In sub-Saharan Africa, where blood supply is critically inadequate, severe haemorrhage is a leading cause of maternal deaths. The aim of this review was to estimate the impact of lack of blood on maternal deaths and identify reasons and potential solutions. Selective non-operative management of splenic injury in children is generally considered to be safe, and the majority of those with isolated injuries do not require blood transfusion. Eighty-four children were treated for blunt splenic trauma from 1988 to 1997 in the Department of Paediatric Surgery, The Medical Faculty of Uludag University, Bursa, Turkey. A premature male infant with hyaline membrane disease received a 34 ml transfusion of packed erythrocytes from a CPDA1 blood unit on day 2 with no adverse consequences. On day 34 he was given a transfusion, and the platelet count decreased. Intravenous immune globulin therapy was initiated and the platelet count steadily increased. Human leukocyte (HLA) antigen and platelet antibody testing showed that the thrombocytopenia was due to passive transfer of platelet-specific antibody from the blood donor.

Haemolytic transfusion reactions occurred in 4 patients under plastic surgery due to incompatibility of Rh blood groups. 4 patients with anti-Rga successfully transfused in 1979 and 1980 with Rg(a+) donor units are herein reported since the literature lacks information on transfusion results in patients with this alloantibody. The transfusions of both Rg(a+) whole blood and packed red blood cell units caused no discernible immediate or delayed transfusion reactions. Clinically, predicted hematocrit increases were attained and sustained and the laboratory findings showed no evidences of shortened survivals of donors' red blood cells.

From 1955 through 1978, 150 immunohemolytic accidents of variable gravity have been documented, representing an incidence of 7.1 cases out of 100 000 transfused blood units. These accidents can be classified as follows: 83 cases due to ABO incompatibility, 42 cases due to an irregular antibody in the recipient (six times due to anti-D), 20 cases induced by passive maternal antibodies in the newborn and 5 cases which were attributed to injected anti-A or anti-B antibodies. We examined the relationship between blood volume, mean haemoglobin maintained and transfusion requirement in an extensive series (166 cases) of uniformly treated patients with homozygous beta-thalassaemia. The blood volume is increased in all the subjects and this increase appears inversely proportional to the mean haemoglobin level maintained.
