Hypoxic burden as a cause of cardiovascular morbidity in childhood obstructive sleep apnea.

BACKGROUND: To assess whether hypoxic burden (HB) is associated with cardiac autonomic nervous system dysfunction and increased blood pressure (BP) in otherwise healthy children with moderate to severe obstructive sleep apnea (OSA). METHODS: Among 103 children with moderate-to-severe OSA, twenty pairs, matched for age, sex and obstructive apnea-hypopnea index (OAHI) were selected, with low (first quartile) or high (fourth quartile) HB in each pair: median [25th-75th percentiles]; age: 10.8 years [7.3; 13.2] vs. 11.4 [8.6; 13.5]; sex: 7 and 10 girls; z-score of body mass index: 1.50 [0.11; 2.4

Global, Regional, and National Disease Burden and Prediction Analysis of Colorectal Cancer Attributable to Tobacco, Alcohol, and Obesity From 1990 to 2030.

BACKGROUND: Colorectal cancer (CRC) ranks among the highest in incidence and mortality rates globally. A significant portion of Colorectal cancer cases and deaths can be attributed to modifiable risk factors, with smoking, alcohol use, and high body mass index (BMI) being the three most prominent. However, the impact of these risk factors on Colorectal cancer across regions, genders, and age groups remains insufficiently characterized. METHODS: Utilizing data from the Global Burden of Disease (GBD) study 2019, restrictive cubic splines (RCS) and quantile regression analyses are applied to exp

Chronic pain is associated with sleep apnea severity but attenuated by intermittent hypoxemia in people using opioids.

Patients using long-term opioids for pain often have uncontrolled symptoms which might drive ongoing opioid use and contribute to the opioid public health crisis. Opioids also cause sleep disordered breathing (SDB), which might impact pain via arousals and hypoxemia, with potential for a vicious cycle. Adults with noncancer pain using opioids daily for >3 months were recruited. Subjects underwent pain and sleep questionnaires, pain tolerance via 10°C cold pressor testing, and polysomnography. Hypoxic burden was determined from polysomnographic signal analysis. Participants (n = 143) had a medi

Opioid Exposure Differentially Impacts Esophageal Body Contraction Over the Lower Esophageal Sphincter.

BACKGROUND & AIMS: Studies with limited sample sizes have investigated association of chronic opioid use with motility disorders of esophagogastric junction and esophageal body peristalsis. Our aims were to use a large cohort of patients to assess (1) the impact of opioid exposure on clinical and manometric characteristics, and (2) the association of opioid exposure with higher long-term symptom burden. METHODS: Patients recruited from a tertiary medical center who underwent high-resolution manometry (HRM) between 2007 and 2018 were included. Demographics, opiate exposure, clinical symptoms,

Epidemiology of injury and illness in 153 Australian international-level rowers over eight international seasons.

AIM: To report the epidemiology of injury and illness in elite rowers over eight seasons (two Olympiads). METHODS: All athletes selected to the Australian Rowing Team between 2009 and 2016 were monitored prospectively under surveillance for injury and illness. The incidence and burden of injury and illness were calculated per 1000 athlete days (ADs). The body area, mechanism and type of all injuries were recorded and followed until the resumption of full training. We used interrupted time series analyses to examine the association between fixed and dynamic ergometer testing on rowers' injury

Determinants of mortality in a combined cohort of 501 patients with HIV-associated Cryptococcal meningitis: implications for improving outcomes.

BACKGROUND: Cryptococcal meningitis (CM) is a leading cause of death in individuals infected with human immunodeficiency virus (HIV). Identifying factors associated with mortality informs strategies to improve outcomes. METHODS: Five hundred one patients with HIV-associated CM were followed prospectively for 10 weeks during trials in Thailand, Uganda, Malawi, and South Africa. South African patients (n = 266) were followed for 1 year. Similar inclusion/exclusion criteria were applied at all sites. Logistic regression identified baseline variables independently associated with mortality. RESU

Obesity and Prevalence of Latent Tuberculosis: A Population-Based Survey.

OBJECTIVE: Diminution in body weight is a known risk factor that increases the burden of active tuberculosis (TB). However, conflicting evidence exists for the effect of body weight on the risk of latent tuberculosis infection (LTBI). The objective of the present study is to examine the prevalence of LTBI at different body weights, evaluate the extent of association between body mass index (BMI) and LTBI and identify factors mediating this relationship in an adult population. METHODS: We conducted a cross-sectional study to estimate the relationship between BMI and LTBI in participants from t

Cardiovascular autonomic nervous system responses and orthostatic intolerance in astronauts and their relevance in daily medicine.

BACKGROUND: The harsh environmental conditions during space travel, particularly weightlessness, impose a major burden on the human body including the cardiovascular system. Given its importance in adjusting the cardiovascular system to environmental challenges, the autonomic nervous system has been in the focus of scientists and clinicians involved in human space flight. This review provides an overview on human autonomic research under real and simulated space conditions with a focus on orthostatic intolerance. METHODS: The authors conducted a targeted literature search using Pubmed. RESUL

Radiotherapy for ablation of ventricular tachycardia: Assessing collateral dosing.

Ablation of cardiac tissue using focused radiotherapy to control ventricular tachycardia (VT) is a novel therapeutic consideration in the management of ventricular arrhythmias associated with structural heart disease. Segments of ventricular myocardium that act as substrates for ventricular tachycardia can be identified by the use of invasive mapping techniques or non-invasive ECG incorporating multi-electrode body surface recordings and cardiac imaging. Early case series have demonstrated a consistent decrease in VT burden and sufficient acute safety to expand to more detailed multicenter stu

[Role of interleukin-6 (IL-6) in the pathogenesis of combined radiation/thermal injuries].

Combined injury such as whole body gamma-irradiation at the dose of 7 Gy+10% body surface full-thickness thermal burn were investigated in (CBA x C57BL6)F1 mice. Enhanced level of IL-6 in mice serum at 6-24 hs following combined injury was established. The potential inhibiting activity of pentoxifylline (POF) as an influence to IL-6 levels, and measure of several acute phase response signs has been evaluated. It was established, that single POF injection don't modify IL-6 production, don't change leukocytosis and early hyperfermentemia (as alaninaminotransferase levels indicated). But serum al

Hypertension among Guatemalan women: the role of food insecurity and overweight/obesity.

INTRODUCTION: Food insecurity (FI) and obesity are associated with cardiovascular risk markers such as hypertension, yet it remains unclear whether their coexistence increases hypertension risk beyond their independent contributions. We hypothesized that women experiencing both FI and OWT/OB ('coexisting burden') would have higher odds of hypertension, compared to those experiencing only OWT/OB, only FI, or neither. We aimed to examine the association of FI and overweight/obesity (OWT/OB) with hypertension and systolic blood pressure (SBP) among Guatemalan women of reproductive age (15-49 year

Adolescent Somatic Symptoms Under Psychosocial Adversity: Differential Links of Risk and Protective Factors to Physical and Psychological Burden in Türkiye.

Background/Objectives: Adolescent somatic symptoms such as headache, irritability, and sleep difficulties are common and closely tied to adolescent mental health, yet they are typically studied as a single dimension and as individual complaints detached from the psychosocial adversity surrounding the adolescent. Drawing on the nationally representative 2022 Türkiye Child Survey (n = 3523, ages 13-17), we examined whether these symptoms are patterned by the psychosocial risk and support surrounding the adolescent. Methods: Measurement models, multiple correspondence analysis, survey-weighted or

Quantifying intra-individual variability in anatomical sites of pain in longitudinal studies.

Longitudinal pain studies present an opportunity to understand intra-individual variations in pain. However, current methods to quantify the anatomical extent of pain fail to capture variations across multiple body sites over time. Our aims were to (1) describe patterns of intra-individual variation in pain sites over time, (2) develop a metric that quantifies intra-individual variation in pain sites over time, and (3) determine whether this metric is related to other indicators of pain burden. We used data from a longitudinal cohort of 72 participants (51 female; median (IQR) age 43 (37-51) y

A common model for the breathlessness experience across cardiorespiratory disease.

Chronic breathlessness occurs across many different conditions, often independently of disease severity. Yet, despite being strongly linked … the experience of breathlessness. These factors were assigned the following descriptive labels: 1) body burden, 2) affect/mood, 3) breathing burden and 4) anger/frustration. Stratifying patients by their scores across the four factors revealed two groups corresponding to high and low burden. These two groups were not related to the primary disease diagnosis …

Periodontal inflamed surface area and C-reactive protein as predictors of HbA1c: a study in Indonesia.

UNLABELLED: Periodontitis may exert an infectious and inflammatory burden, evidenced by increased C-reactive protein (CRP). This burden may impair blood glucose control (HbA1c). The aim of our study was to analyze whether periodontitis severity as measured with the periodontal inflamed surface area (PISA) and CRP predict HbA1c levels in a group of healthy Indonesians and a group of Indonesians treated for type 2 diabetes mellitus (DM2). A full-mouth periodontal examination, including probing pocket depth, gingival recession, clinical attachment loss, plaque index and bleeding on probing, was p
