Human immunodeficiency virus/acquired immunodeficiency syndrome-associated nephropathy at end-stage renal disease in the United States: patient characteristics and survival in the pre highly active antiretroviral therapy era.

BACKGROUND: The patient characteristics and course of HlV/AIDS-associated nephropathy (HIVAN) are presented for a national sample of end-stage renal disease (ESRD). METHODS: 375,152 patients in the United States Renal Data System were initiated on ESRD therapy between 1 January 1992 and 30 June 1997 and analyzed in an historical cohort study of HIVAN. RESULTS: Of the study population, 3653 (0.97%) had HIVAN. Among patients with HIVAN, 87.8% were African American. HIVAN had the strongest association with African American race compared to other causes of renal failure except sickle cell anemia

Effect of low-dose atropine administration on dobutamine dose requirement in horses anesthetized with detomidine and halothane.

OBJECTIVE: To determine whether a low dose of atropine is associated with decreased requirement for cardiovascular supportive treatment in horses given detomidine prior to maintenance of general anesthesia with halothane. ANIMALS: 3 groups of 10 healthy horses. PROCEDURE: Detomidine (20 micrograms/kg of body weight, i.m.) was administered to all 30 horses. Then, 10 horses received atropine (0.006 mg/kg, i.v.) 1 hour after detomidine administration, 10 horses received atropine (0.012 mg/kg, i.m.) at the time of detomidine administration, and 10 horses served as a control group. Heart rate was

Heating of the brain to maintain normothermia during ischemia aggravates brain injury in the rat.

During brain ischemia temperature spontaneously declines. In animal experiments this decline is frequently prevented by stabilizing the temperature at the pre-ischemic level, using an external heat source. The present study examines whether this procedure influences the severity of ischemic injury. Wistar rats were submitted to 30-min four-vessel occlusion followed by 7 days recirculation. During ischemia and the 1st h of recirculation various systemic and electrophysiological variables were recorded. Seven days after the ischemia brains were perfusion-fixed for light microscopical examination

Comparative clinical study of spironolactone and potassium canrenoate. A randomized evaluation with double cross-over.

In the present study 54 cirrhotic patients were investigated in order to compare the clinical effects of spironolactone (100-200 mg/d) and potassium canrenoate (50-200 mg/d). Diagnosis was established on clinical and laboratory findings for at least 12 months and/or on liver biopsy: no patients had signs of hepatic decompensation. Patients entering the study without previous treatment, after a basal period of observation, were randomly allocated to one of the spirolactones (spironolactone or potassium canrenoate); those already under antialdosteronic treatment underwent a first observation per

[Parenteral replacement of thyroid hormones].

Because of the development in parenteral nutrition, the replacement of thyroid hormones in hypothyroid or athyroid patients under intravenous hyperalimentation has become a new problem to be considered. We tried parenteral replacement of the hormones, intravenously or by enema, in three such patients. Two patients, 54 y-o and 64 y-o females, who underwent laryngo-esophago-thyroidectomy for cervical esophageal cancer or thyroid cancer, had replacement with intravenous l-thyroxine with an initial dose of 100 micrograms/day for 9 and 22 days, respectively. Another patient, a 56 y-o female with dy

[Phenobarbital in newborn infants. Overview].

Over the past years the use of phenobarbital in so called "brain orientated neonatal intensive care" has gradually become established. It is the recommended drug for the treatment of seizures in term neonates. It also should be given to neonates who are being treated with curare like muscle relaxants and whose EEG may show paroxysmal activity. It is recommended to administer phenobarbital before curare is given. This may result in more effective mechanical ventilation. Seizures occurring in the premature baby can hardly be influenced. The prophylactic treatment of premature infants to avoid in

Causal insights of modifiable cardiovascular risk factors for dementia risk - potential for efficient prevention and improved brain health.

BACKGROUND: The 2024 Lancet Commission report identifies 14 modifiable risk factors that may prevent half of dementia. The causal nature of these associations remains however unclear. We aimed to establish robust causal estimates for modifiable cardiovascular risk factors and dementia, offering tangible targets for effective dementia prevention. METHODS: We selected independent variants from the largest genomic consortia to date (N = 439,214 to 3,037,499) for each modifiable risk factor and generated polygenic risk scores for participants of European ancestry in the UK Biobank (N = 408,788).

Integrating environmental enrichment in Turkey production: Molecular, biochemical, welfare and economic perspectives.

Turkeys are a rapidly expanding poultry sector, yet welfare challenges, particularly injurious pecking, could compromises performance and profitability. Environmental enrichment (EE) has been applied to encourage natural behaviours, reduce aggression, and enhance immune and physiological responses. This study was conducted to evaluate the effects of two EE strategies- perches with hanging colored balls (P) and elevated platforms "turkey tree" (TT)- on the growth, behaviour, immune status, and economic efficiency of turkey poults. A total of 4500 one-day unsexed turkey poults were raised for 7

Who Holds the Plate? Psychotherapists' Perspectives on Dietary Behavior, Transdiagnostic Evaluation and Interdisciplinary Collaboration in Eating Disorders.

Background/Objectives: Dietary behavior in eating disorders (EDs) is often framed through either nutritional or psychological perspectives, yet emerging evidence suggests that eating may involve a transdiagnostic, emotionally embedded, and relationally negotiated process. While research highlights the role of emotion regulation difficulties, perfectionism, control, and overvaluation of weight and shape in ED maintenance, less is known about how these processes are interpreted and managed in clinical practice across different cultural contexts. This study explored psychotherapists' perspectives

Functional and effective connectivity between reward and inhibitory control networks underlying subclinical binge eating.

BACKGROUND: Knowledge is growing on the essential role of neural circuits involved in aberrant cognitive control and reward sensitivity for the onset and maintenance of binge eating. AIMS: To investigate how the brain's reward (bottom-up) and inhibition control (top-down) systems potentially and dynamically interact to contribute to subclinical binge eating. METHOD: Functional magnetic resonance imaging data were acquired from 30 binge eaters and 29 controls while participants performed a food reward Go/NoGo task. Dynamic causal modelling with the parametric empirical Bayes framework, a nove

Olfactory bulbectomy-induced impairment of lipid utilization leads to abnormal glucose metabolism in mice.

Increased lipid utilization after food odor perception is a recently identified Pavlovian cephalic phase response in fasted mice that serves to prevent diet-induced glucose intolerance. However, the impact of olfactory dysfunction on metabolic functions remains unclear. Since olfactory bulbectomized (OBX) rodents have been used as a model of irreversible complete anosmia, we investigated whether glucose, lipid, and energy metabolism change over time in OBX mice fed a normal chow diet (NCD) or high-fat diet (HFD). OBX caused constant hyperactivity and triphasic temporal changes in lipid and glu

Adaptive learning from outcome contingencies in eating-disorder risk groups.

Eating disorders are characterised by altered eating patterns alongside overvaluation of body weight or shape, and have relatively low rates of successful treatment and recovery. Notably, cognitive inflexibility has been implicated in both the development and maintenance of eating disorders, and understanding the reasons for this inflexibility might indicate avenues for treatment development. We therefore investigate one potential cause of this inflexibility: an inability to adjust learning when outcome contingencies change. We recruited (n = 82) three groups of participants: those who had rec

Aerobic exercise for eight weeks provides protective effects towards liver and cardiometabolic health and adipose tissue remodeling under metabolic stress for one week: A study in mice.

BACKGROUND: The relationship between exercise training and health benefits is under thorough investigation. However, the effects of exercise training on the maintenance of metabolic health are unclear. METHODS: Our experimental design involved initial exercise training followed by a high-fat diet (HFD) challenge. Eight-week-old male was trained under voluntary wheel running aerobic exercise for eight weeks to determine the systemic metabolic changes induced by exercise training and whether such changes persisted even after discontinuing exercise. The mice were given either a normal chow diet

Elevated Neurobehavioral Responses to Negative Social Interactions in Women With Bulimia Nervosa.

BACKGROUND: Bulimia nervosa (BN) is a complex psychiatric illness that includes binge-purge behaviors and a belief that one's value as a person depends on body shape and weight. Social pressure strongly influences the development and maintenance of BN, but how this manifests neurobiologically within an individual remains unknown. We used a computational psychiatry approach to evaluate neural mechanisms underlying social interactions in BN. METHODS: Behavioral and functional magnetic resonance imaging data were collected from 24 women with BN and 26 healthy comparison women using an iterated s
