Primary phycomycosis of the brain in heroin addicts.

Two heroin addicts had fatal primary phycomycosis of the brain. The first case is unique. A 28-year-old man displayed unusual symptoms of unilateral hydrocephalus confirmed by both computed tomography (CT) scanning of the brain and craniotomy. The nature of the fungal granuloma was verified histopathologically. The second case, a 40-year-old man, possessed bilateral basal ganglionic lesions in which the CT scanning suggested abscesses. Postmortem examination confirmed the presence of phycomycotic abscesses. There was no evidence of systemic mycoses in both cases. Review of the clinical feature

[Cerebral trunk abscess due to Neisseria meningitidis in a 28-year-old immunocompetent patient].

Meningococcal disease can manifest in very different clinical situations. Meningococcal meningitis and fulminant meningococcemia (or purpura fulminans) are the most common forms of these infections, but many other manifestations can be seen including septic arthritis, pericarditis, pleurisy and conjunctivitis. Brain abscesses have also been described; they are a rare complication of meningococcal disease. We report the case of a 28-year-old immunocompetent patient who developed meningococcal infection associated with brain abscesses and oligo-arthritis.

Vibrio parahaemolyticus- and V. alginolyticus-associated meningo-encephalitis in a bottlenose dolphin (Tursiops truncatus) from the Adriatic coast of Italy.

A case of Vibrio parahaemolyticus- and V. alginolyticus-associated meningo-encephalitis in a bottlenose dolphin (Tursiops truncatus) found stranded along the Adriatic coast of Italy in 2016 is herein reported, along with a minireview on V. parahaemolyticus and V. alginolyticus infections in aquatic mammals. Macroscopically, two abscesses were found in the dolphin's forebrain, along with an extensive, bilateral, parasitic broncho-pneumonia. Histologically, a suppurative-to-pyogranulomatous meningo-encephalitis involved the brain but not the cerebellum. Microbiological investigations yielded iso

A case report of ocular nocardiosis secondary to Nocardia asiatica in a patient with acquired immunodeficiency syndrome.

PURPOSE: To describe the clinical presentation, diagnosis, management, and outcome of endogenous ocular nocardiosis caused by Nocardia asiat … inflammation in the setting of disseminated infection, particularly when accompanied by multifocal brain abscesses, should prompt early consideration of nocardiosis. Early recognition and empiric trimethoprim-sulfamethoxazole therapy may facilitate systemic recovery and preserve vision in less affected eyes.

The role of CT in the evaluation of patients with intracranial CNS infectious-inflammatory disorders.

Infections of the CNS may be caused by diverse etiologies. These include bacteria, viruses, mycoses, protozoa, helminths. In these CNS infec … mortality and morbidity. Since the introduction of CT, clinical outcome in patients with cerebritis-brain abscess has markedly changed. In this paper the role of CT in the evaluation of intracranial CNS infection will be reviewed.

Sarcoidosis complicated by cirrhosis and hepatopulmonary syndrome.

Sarcoidosis is a multisystem disorder commonly affecting the lungs, but also the liver, with cirrhosis and portal hypertension occurring in fewer than 1% of cases. Although hepatopulmonary syndrome (HPS) is seen in 15% to 20% of patients with cirrhosis of varying causes, it has rarely been associated with sarcoidosis. Also, although a brain abscess is not uncommon in patients with discrete pulmonary arteriovenous malformations, it is rarely seen in patients with the much smaller intrapulmonary vascular dilations that characterize HPS. A patient with an unusual series of uncommon sarcoidosis co…

Cerebral Abscess Mimicking Intracerebral Hemorrhage: A Case Report.

Hemorrhagic pyogenic cerebral abscesses are a rare clinical entity that can present diagnostic challenges and lead to treatment delays. We report a case of a 46-year-old female who initially presented with a sudden onset of focal neurological deficits, raising a suspicion of a cerebrovascular etiology. Further diagnostic workup revealed a hemorrhagic pyogenic brain abscess. The patient was managed successfully with a conservative approach, receiving medical treatment after declining surgical intervention, and was discharged with only minimal neurological residuals. Although hemorrhagic pyogeni…

Clinicoradiological improvement of intracranial tubercular abscess with medical management alone.

Tubercular abscess is a rare presentation of central nervous system tuberculosis (TB), which commonly presents as tuberculoma or meningitis. Most of the published cases of tubercular brain abscess have been dealt with by surgical treatment along with medical management. We here report a case of intracranial tubercular abscess in a 17-year-old girl presenting with fever, headache and right hemiparesis who showed significant clinicoradiological improvement with medical management alone. Thus, in a country like India where TB is highly endemic, TB should be considered as a possible aetiology for …

Meningitis and Ventriculitis due to Nocardia araoensis Infection.

A 73-year-old man was admitted to our hospital with disturbance of consciousness, fever and headache. Cerebrospinal fluid (CSF) analysis revealed pleocytosis with neutrophil predominance, increased protein and low glucose. CSF and blood cultures yielded negative results. Antibiotics and antituberculous drugs were started for meningitis. An antimycotic was also added. The patient died from transtentorial hernia 99 days after admission. Autopsy revealed meningitis, ventriculitis and brain abscess, and Nocardia araoensis was detected in pus from the left lateral ventricle. This appears to represe…

Current approaches to management of cerebral fungal infection in pediatric patients with hematologic disorders.

We report 2 pediatric cases of cerebral fungal infection. A patient with severe aplastic anemia developed an Aspergillus species brain abscess and pulmonary aspergillosis after peripheral blood stem cell transplantation. Despite administration of micafungin, amphotericin B, and flucytosine, the patient died 2 months after the transplantation because of underlying pulmonary aspergillosis. Another patient with acute myelogenous leukemia developed a huge brain abscess with histopathologic findings suspicious of mucormycosis. This patient was cured with combination therapy of antifungal agents and…

[Pulmonary nocardiosis with cerebral abscess successfully treated by medication alone--a case report].

Nocardiosis is an illness caused by members of the genus Nocardia. Although it is usually an opportunistic infection in immunocompromised hosts, it can also occur in patients without a definable predisposing condition. Three species are recognized as being responsible for what most frequently seen. The primary site of infection is overwhelmingly the lungs. Brain abscess is by far the most common site for nocardial metastasis from pulmonary lesion. Although surgical intervention must always be considered in the treatment of nocardial brain abscess, it can clearly be cured by medical therapy alo…

[An abscess gone astray].

A 69-year-old female patient with known recurrence of esophageal cancer presented with fever and elevated inflammatory markers. In the recent past, a suspicious mediastinal lymph node had been biopsied via bronchoscopy. Consequently, a lymph node abscess developed, leading to blood stream infection (pathogen: Streptococcus anginosus) via a fistula into the superior vena cava. Through a preexisting atrial septal defect, the infection also spread to the brain, causing multiple brain abscesses.

Fulminant citrobacter meningitis with multiple periventricular abscesses in a three-month-old infant.

Citrobacter, a Gram-negative enteric bacillus, is a rare cause of septicemia and meningitis, seldom reported beyond the neonatal period. It is characterized by a fulminant clinical course and a high incidence of complications, including brain abscesses. We studied a three-month-old infant with Citrobacter meningitis, who developed acute communicating hydrocephalus and multiple periventricular brain abscesses while on treatment. The patient died, despite intensive antibiotic treatment directed towards the causative organism, C. diversus.

An Unexpected Case of Disseminated Amebiasis with Cerebral Involvement and Successful Recovery in a Non-Endemic Context.

BACKGROUND Amebiasis is a parasitic infection caused by the protozoan Entamoeba histolytica. Amebic brain abscesses are a rare form of invasive amebiasis frequently lethal due to the difficulty of its diagnosis and inadequate treatment. Cerebral amebiasis poses a therapeutic challenge as evidenced by the scarcity of papers reporting complete recovering after treatment. CASE REPORT We report the case of a 39-year-old Spanish man, with a history of alcohol and drug abuse. He had never traveled outside of Europe, no reported oral-anal sexual contact, and no history of immunosuppressant medication…

Intracranial abscess from facial cellulitis.

Cellulitis, one of most common diseases of everyday life, is often overlooked for its significance. Although cellulitis does not cause or lead to serious problems usually, its possibility to cause life-threatening problem should be known. In present case, a patient who had received acupuncture treatment a week earlier presented to the clinic with symptoms of facial cellulitis. The disease resolved within few weeks under empirical antibiotic treatment but recurred after 3 months. Under close history review of the patient, we found out that the patient had received craniectomy 20 years ago. The

99mTc-HMPAO labeled leukocyte SPECT in intracranial lesions.

BACKGROUND: Scintigraphy with 111In (indium-111)-oxine or 99mTechnetium-hexamethylpropyleneamine oxime (99mTc-HMPAO) labeled leukocytes has been used to differentiate brain abscess from brain tumor. However, there are false positive or false negative results from planar scintigraphic images. So a more specific and sensitive scintigraphic technique needs to be developed. METHODS: Planar and single photon emission computed tomography (SPECT) images were obtained and reviewed in 14 patients with intracerebral ring-enhancing lesions on computed tomography (CT) or magnetic resonance imaging (MRI).…
