Recurrent obstetric management mistakes identified by simulation.

OBJECTIVE: To develop a simulation-based curricular unit for labor and delivery teams involved in obstetric emergencies to detect and address common mistakes. METHODS: A simulation-based curricular unit for hands-on training of four obstetric emergency scenarios was developed using high-tech mannequins and low-tech simulators. The scenarios were eclamptic seizure, postpartum hemorrhage, shoulder dystocia, and breech extraction. The obstetric teams consisted of at least one resident and two midwives. Checklists of actions expected from the teams were handed out to the course's tutors who obser

Elective cesarean delivery for term breech.

OBJECTIVE: To compare the maternal complications of elective cesarean delivery for breech at term with those after vaginal or emergency cesarean delivery. METHODS: We conducted a population-based, retrospective cohort study of 15441 primiparas who delivered singleton breech at term. Information was obtained from the Danish Medical Birth Register, the Register of Death Causes, and the Denmark Patient Register. RESULTS: Elective cesarean delivery was associated with lower rates of puerperal fever and pelvic infection (relative risk [RR] 0.81; 95% confidence interval [CI] 0.70, 0.92), hemorrhag

Extremely preterm vaginal breech delivery en caul.

OBJECTIVE: To describe an alternative method of vaginal birth to the conventional assisted delivery for extremely preterm breech infants within intact amnions, and to compare the immediate neonatal outcomes with those delivered by cesarean. METHODS: Retrospective review of singleton breech deliveries under 26 weeks' gestation after spontaneous labor with intact membranes. Nine "en caul" vaginal births after tocolysis and six cesarean deliveries performed for the sole indication of preterm labor were identified between 1996 and 2001. RESULTS: The vaginal group's mean gestation and cervical di

Vaginal breech delivery: results of a prospective registration study.

BACKGROUND: Most countries recommend planned cesarean section in breech deliveries, which is considered safer than vaginal delivery. As one of few countries in the western world Norway has continued to practice planned vaginal delivery in selected women. The aim of this study is to evaluate prospectively registered neonatal and maternal outcomes in term singleton breech deliveries in a Norwegian hospital during a ten years period. We aim to compare maternal and neonatal outcomes in term breech pregnancies subjected either to planned vaginal or elective cesarean section. METHODS: A prospective

Risk factors for neonatal brachial plexus paralysis.

PURPOSE: The study was aimed to identify risk factors for neonatal brachial plexus paralysis. METHODS: A retrospective case-control study was designed. A comparison was performed between cases of brachial plexus paralysis, with all consecutive deliveries during the same 5 months period, without brachial plexus paralysis. Statistical analysis was performed using the SPSS package. RESULTS: The prevalence of brachial plexus paralysis was 1.62/1,000 (9/5,525) vaginal births. Independent risk factors for brachial plexus paralysis were shoulder dystocia (OR = 525; 95% CI 51-4,977, P < 0.001), vacu

Detecting developmental dysplasia of the hip.

Recent guidelines from the American Academy of Pediatrics mandate the universal screening of all newborns for developmental hip dysplasia, in an effort to promote prompt recognition and treatment of this condition. The effectiveness of universal screening hinges on the appropriate education, training, and experience of health care providers. This article reviews the normal embryologic, natal, and postnatal development of the hip. Historical risk factors that may have an impact on this developmental sequence are outlined. A step-by-step guide to the examination of the hip is offered, along with

Delivery of the very low birthweight breech: what is the best way for the baby?

A retrospective analysis was done to determine whether vaginally vs. cesarean section-born breech infants in the very low birthweight range are at increased risk for morbidity and mortality. Eighty-three viable singleton breech infants weighing 700-1,600 g, who were delivered in our medical center during the period 1980 through 1993, were followed for up to 5 years of age. Analysis of data after correction for weight, gestational age and other confounding variables such as antepartum complications revealed that survival rates in the 700-1,000 g group were similar in both routes of delivery. [Ultrasound of extremity lesions caused by birth trauma].

With regard to injuries of the extremities caused by birth trauma, ultrasound allows simultaneous direct evaluation of the surface of osseous elements and of cartilaginous and soft-tissue structures as well. Thus, not only fractures, but also concomitant articular fluid collections, respectively haematomas and/or dislocation of an epiphysis are demonstrated in their initial extent and in their course of healing. Additionally, closed repositions, for instance of a displaced epiphysis, can be exactly monitored by ultrasound. A reduced number of roentgenograms as well as renunciation of other ima

[Infants of low birth weight after deliveries, morbidity and mortality (author's transl)].

Morbidity and mortality of 908 immature births (newborn infants under 2501 g; still births with crown-heel lengths 35 cm plus) which occured at the University Clinic, Mannheim, Department of obstetrics and Gynecology between the years 1966 and 1971 were studied and compared with a random selection of 782 newborn infants with birth weights of at least 2500 g. From a total of 832 live born immatures, one quarter had an initial Apgar score of 1--7. 67% of these were under 1000 g, 22,07% had birth weights between 1501 and 2000 g and 12,72% between 2001 and 2500 g. Immediately post-partum 89,7% of

The role of multidisciplinary team discussions in the management of thoraco-omphalopagus conjoined twins: A case report from a low-resource setting.

INTRODUCTION AND IMPORTANCE: Conjoined twins represent a rare medical phenomenon that poses significant clinical and ethical challenges, particularly in resource-limited settings. CASE PRESENTATION: We present a case of conjoined twins delivered at 39 weeks gestation to a 27-year-old refugee mother. The delivery was by emergency cesarean section due to footling breech presentation. Initial assessments revealed shared cardiac anatomy, with Twin A exhibiting a more favorable anatomical structure compared to Twin B, who presented with severe cardiac malformations. Follow-up imaging, including CT…

Examining Cesarean Delivery Rates by Race: a Population-Based Analysis Using the Robson Ten-Group Classification System.

The Robson Ten-Group Classification System is widely considered to be the gold standard for comparing cesarean section (CS) delivery rates, despite limited adoption in the United States (US). When reporting overall CS rates, Blacks and other minorities are typically reported to have high CS rates but comparing overall CS rates may be misleading as CS may be more common in some higher risk populations. Improved understanding of how CS rates differ by race among standardized groups could highlight differences in care and areas for improvement. The current study examines racial differences in ces

Challenges in the Caesarean Section of a Severely Kyphotic Parturient.

Caesarean section in a severely kyphotic patient presents with unique challenges. We report a case of obstructed labor in case of a pregnant lady with severe kyphosis of spine that was managed by caesarean section. Lateral recumbent position with adequate assistance and paramedian or vertical skin incision was used and found to provide good exposure. Baby was delivered by lower segment uterine incision by reverse breech extraction. Postpartum hemorrhage was managed with uterotonics and bilateral uterine artery ligation. Tubal ligation though advised was refused by the patient. Prolonged cathet

Congenital dysplasia of the hip: the lax (subluxatable) newborn hip.

During a 4-year period, 15,149 newborns were examined for hip abnormalities. Ninety-two infants (0.61%) were found to have subluxatable (lax, but not dislocatable) hips. The profile of this group was similar to patients with dislocated hips, with an increased frequency of females, breech presentations, left-sided involvement, and associated postural deformities. A significant number showed acetabular dysplasia or persistent hip laxity despite treatment with an abduction brace. We concluded that the newborn subluxatable hip arises from the same antecedents as frank dislocation, is not always be…

Routine ultrasound screening for neonatal hip instability. Can it abolish late-presenting congenital dislocation of the hip?

We have undertaken routine ultrasound screening for neonatal hip instability in Coventry since June 1989. Of the 14,050 babies scanned during the first three years, 847 (6%) had ultrasound abnormalities. A grading system, based on the percentage of femoral head coverage, is presented. The proportion of abnormal hips decreased gradually so that by nine weeks, 90% had normal ultrasound appearances. Abnormality was more common in babies with a family history of CDH and in breech presentations. All babies with clinically abnormal hips had an abnormal first ultrasound examination. Five babies not d…

The abnormalities of the presentation in twin pregnancy and perinatal mortality.

The author conducted a retrospective review of the obstetric patient material in his department between January 1, 1975 and December 31, 1992, concerning the clinical implications associated with various lies and presentations of twins. The material included 329 twins (1.07% of all births). Breech, or transverse lies complicated 186 (56.53%) twin gestations. The frequency of premature newborns was among 186 malpresented twin pairs 85 (45.7%) and 101 (54.3%) were mature. The percentage of malpresented twin fetuses increased with maternal age and parity. Abnormal presentations were more frequent
