Bilateral Subacromial-Subdeltoid Rice Bodies in the Shoulder: A Surgical Case Report.

INTRODUCTION: Rice bodies (RBs) are pale and glossy appearing small fibrinous nodules that form due to synovial or tenosynovial joint inflammation. RBs are significant as they are common in orthopedic practices causing nonspecific symptoms such as pain, swelling, range of motion limitations, crepitus, and catching sensations. These loose bodies occur often within the bursa as a symptom of chronic bursitis and are commonly associated with rheumatoid, inflammatory, or tuberculous arthritis. Reports on RBs are present; however, few bilateral cases within the shoulder appear in the literature. Cas…

External to internal glenohumeral strength ratio in non-traumatic rotator cuff pathologies.

BACKGROUND AND STUDY AIMS: A correct agonist -antagonist strength relationship for shoulder external and internal rotation is necessary for … ed and unaffected shoulders, respectively. The ratio was 0.87 (SD 0.23) in patients with subdeltoid bursitis, 0.88 (SD 0.16) in rotator cuff tendinopathy and 0.87 (SD 0.22) in patients with rotator cuff tears. CONCLUSIONS: The ER/IR ratio appears to be similar between the affected and unaffected shoulders of subjects with nontraumatic cuff pathologies.

Excisional Debridement for Chronic Achilles Insertional Tendinosis: A Novel Technique and Case Review.

BACKGROUND: Operative treatment of chronic Achilles insertional tendinosis (AIT) involves tendon debridement, removal of the retrocalcaneal bursitis, and excision of the calcaneal exostosis, often followed by repair of the Achilles tendon and deep tendon transfer. The literature describes a variety of techniques without a single standard of care. METHODS: This is a retrospective review of 57 patients treated with an excisional debridement of the central portion of the Achilles tendon. The novelty of this technique is that instead of complete detachment of the tendon from its insertion, only t…

Can Haglund's Syndrome Be Misdiagnosed as Low Back Pain? Findings from a Case Report in Physical Therapy Direct Access.

Background: Haglund's syndrome (HS) is a painful condition that is caused by an exostosis of the posterior superior part of the calcaneus coupled with Achilles tendinopathy and retrocalcaneal bursitis. Both for the proper musculoskeletal assessment and for the differential diagnosis process of possible concurrent diseases deriving from other anatomical areas, the diagnosis of HS is still a challenge. Case Presentation: A 41-year-old male amateur runner was diagnosed and treated for low back pain and referred leg pain by his general practitioner. Due to ineffective results, he self-presented to…

Sacral stress fracture in a professional hockey player: a case report.

Lumbosacral pain is common in the general population and among athletes. Many athletes are diagnosed with low back strain and treated with nonsteroidal anti-inflammatories, rest, and muscle relaxers. However, the differential for low back pain in athletes is broad and includes many potential etiologies such as: lumbar disk disease, facet arthropathy, spondylolysis, sacroiliitis, tendinopathies, ligament sprains, hip pathology, bursitis, intraabdominal processes, and neoplasm. Sacral stress fractures are included among the many possibilities. Stress fractures are rare in the general population,…

Polymyalgia rheumatica: diagnosis and treatment.

Polymyalgia rheumatica (PMR) typically manifests as inflammatory pain in the shoulder and/or pelvic girdles in a patient over 50 years of ag … disease, or infection. Ultrasonography may assist in the diagnosis by showing bilateral subdeltoid bursitis. Glucocorticoids are the mainstay of the treatment of PMR. Although the optimal starting dosage and tapering schedule are not agreed on, a low starting dosage and slow tapering may decrease the relapse rate. Methotrexate is probably useful when gluc …

Ultrastructure of the subacromial bursa in painful shoulder syndromes.

In order to determine if inflammation of the subacromial bursa or "bursitis" is as common an occurrence as believed in painful conditions of the shoulder, eight bursae were obtained during surgery from 3 cases of calcifying tendinitis, 2 of tight coracoacromial ligament, 2 of rotator cuff tear and 1 of acromioclavicular osteoarthritis. In the cases of calcifying tendinitis, tight coracoacronial ligament and rotator cuff tear the morphological changes mainly consisted of a numerical increase in cells throughout the bursal wall along with proliferation of endothelial cells in the vascular channe…

Ultrasonographic Diagnosis of an Ischiocondylar Adductor Magnus Partial Tear: An Uncommon Cause of Ischial Pain.

Pain arising from the ischial region is commonly attributed to proximal hamstring injury or ischial bursitis, whereas injury to the ischiocondylar portion of the adductor magnus remains less commonly considered because of its deep location and complex anatomy. We report a 63-year-old woman who developed persistent proximal posterior thigh pain after sprinting, followed by progressive sitting pain despite conservative treatment. High-resolution musculoskeletal ultrasonography revealed focal structural abnormalities of the ischiocondylar portion of the adductor magnus at the tendon insertion and…

Heel Pain: Diagnosis and Management.

Heel pain accounts for 2 million office visits annually and is associated with higher body mass index, manual jobs, and a sedentary lifestyle. Retrocalcaneal bursitis is more common in women 40 to 60 years of age with Haglund deformity and a thickened Achilles tendon. Calcaneal apophysitis (Sever disease) is the most common cause of heel pain in active children and adolescents; in the outpatient setting, it accounts for up to 16% of musculoskeletal conditions in children. In os trigonum syndrome, athletes such as soccer players, gymnasts, and dancers who are engaged in repetitive plantar flexi…

Atypical variant of Baastrup's disease with lumbar stenosis and cauda equina syndrome.

BACKGROUND: Classical Baastrup's disease is a degenerative disorder of the lumbar spine characterized by the approximation of adjacent spinous processes due to excessive lordosis. This results in edema, sclerosis, cyst, bursitis, and midline epidural fibrosis and is often overlooked as a cause of low back pain. Here, we report a patient with atypical Baastrup's disease and lumbar spinal stenosis who presented with a cauda equina syndrome. CASE DESCRIPTION: A 67-year-old male presented with low back pain of 1 year's duration. This exacerbated over the past 3 weeks, becoming associated with the…
