Selection of approach and bypass for fetal-type posterior cerebral artery aneurysm: illustrative cases.

BACKGROUND: Unlike in aneurysms of the adult-type posterior cerebral artery (PCA), in aneurysms of the fetal-type PCA, parent artery occlusion (PAO) results in vascular insufficiency and major ischemic strokes. Preservation or reconstruction of fetal-type PCAs is necessary to prevent these complications. Furthermore, it is necessary to select an appropriate bypass method and approach for revascularization of the PCA. OBSERVATIONS: The authors report 2 cases of aneurysms of fetal-type PCAs that were successfully treated with PAO with revascularization. A 38-year-old man with a large unruptured

Solitaire Stent Retriever Mechanical Thrombectomy in a 6-Month-Old Patient with Acute Occlusion of the Internal Carotid Artery Terminus: Case Report.

BACKGROUND: Endovascular mechanical thrombectomy (MT) performed with stent retriever devices have proven efficacious in the treatment of adult patients with acute cerebral ischemia resulting from large-vessel occlusion. Recently, reports describing the use of MT in pediatric large-vessel occlusion have increased. However, these case reports are generally limited to older children and teenagers. CASE DESCRIPTION: We report the use of a Solitaire (Medtronic, Minneapolis, Minnesota, USA) stent retriever for thrombectomy and revascularization of the right middle cerebral artery) in a 6-month-old

Moyamoya angiopathy with dolichoectatic internal carotid arteries, patent ductus arteriosus and pupillary dysfunction: a new genetic syndrome?

We report on 2 children with moyamoya angiopathy and bilateral dolichoectatic internal carotid arteries in combination with iris hypoplasia with bilateral fixed dilated pupils and a history of patent ductus arteriosus. Both were symptomatic with moyamoya angiopathy and underwent bilateral extracranial-intracranial (EC-IC) bypass operations for cerebral revascularization. This is the first report on moyamoya angiopathy and bilateral dolichoectatic internal carotid arteries with simultaneous occurrence of ocular and cardiovascular malformations. There have been descriptions of cerebral vascular …

Characterizing pre- and post-operative cerebral blood flow and transit time in pediatric moyamoya patients using multi-delay ASL and DSC MRI.

Cerebral blood flow (CBF) and transit time are essential biomarkers for assessing brain health. While dynamic susceptibility contrast (DSC) MRI has been widely applied to measure these metrics, it is limited in the pediatric population due to the need for contrast agents. Arterial spin labeling is a non-invasive and quantitative MR modality, and multi-delay ASL can measure CBF and transit time simultaneously. Although multi-delay ASL has been used in adult neuroimaging studies, its application in children requires investigation. Moyamoya disease, a progressive steno-occlusive cerebrovascular d

Management challenges of tongue piercing-associated mitral valve endocarditis with systemic embolization in a 16-year-old patient.

BACKGROUND: Infective endocarditis (IE) is a rare but serious condition in adolescents. While most cases are associated with dental procedures, oral piercings can also serve as an entry point for bacterial infection. A 16-year-old female with no prior medical history presented with severe pain in the left hypochondrium, accompanied by fever and rigors. She had undergone tongue piercing several months earlier and nasal piercing 1 week prior. Echocardiography revealed mitral valve IE with severe regurgitation. Streptococcus mitis was identified as the etiologic pathogen. Computed tomography scan

[Asymptomatic intracerebral hemorrhage under strict blood pressure control due to postoperative cerebral hyperperfusion in a patient with moyamoya disease].

We report a case of moyamoya disease manifesting as asymptomatic intracerebral hemorrhage due to postoperative cerebral hyperperfusion, despite the prophylactic intensive blood pressure control. This 35-year-old man initially suffered from right upper quadrantanopsia and was found to have cerebral infarction in the left occipital lobe due to moyamoya disease. He also manifested preoperatively a small intracerebral hemorrhage at the left caudate nucleus. The left cerebral hemisphere showed apparent hemodynamic compromise, thus he underwent left superficial temporal artery-middle cerebral artery

Acute In-Stent Thrombosis after Carotid Angioplasty and Stenting: A Case Report and Literature Review.

BACKGROUND: Based on the results of a recent randomized controlled trial, carotid artery stenting (CAS) was regarded as a relatively safe, less invasive treatment of internal carotid artery stenosis. However, cerebral thromboembolic events are the most common complications of CAS. Especially acute stent thrombosis following CAS will be fatal without prompt diagnosis and revascularization. CASE REPORT: We report a case of acute stent thrombosis in whom carotid revascularization was performed successfully via arterial thrombolysis and balloon postdilation. A 79-year-old man with hypertension wa

C2-P2 Bypass: Technical Assessment of Petrous Carotid Artery to Posterior Cerebral Artery Interpositional Bypass Through the Combined Transcochlear-Subtemporal Approach as a Part of Microsurgical Treatment for Dolichoectatic Vertebrobasilar Artery Aneurysms.

BACKGROUND AND OBJECTIVES: Managing dolichoectatic vertebrobasilar artery aneurysms requires a multifaceted approach. Revascularization of the posterior circulation with a high-flow bypass is part of the flow reversal paradigm. Performing a robust high-flow bypass and addressing the aneurysm through the same approach smooths the operative intervention. This study assessed the anatomic feasibility of accessing the basilar trunk and aneurysm simultaneously to revascularize the posterior circulation using a petrous internal carotid artery (pICA)-posterior cerebral artery (PCA) interpositional byp

Natural history of endogenous collateral vessels after revascularization surgery in patients with moyamoya angiopathy.

OBJECTIVE: Moyamoya angiopathy (MMA) is characterized by the plasticity to develop endogenous collateral blood vessels to compensate for progressive steno-occlusion of proximal intracranial arteries. Bypass surgery has been anecdotally reported to induce regression of these collateral vessels, but a detailed analysis of their natural history is lacking. Here, the authors characterize these collaterals after bypass surgery. METHODS: A single-center retrospective analysis of the medical records of 81 predominantly Caucasian MMA patients (121 hemispheres) treated with a combined superficial temp

Comparison of Exoscopic and Microscopic Superficial Temporal Artery to Middle Cerebral Artery Bypass.

PURPOSE: Exoscopes have been used as a reliable surgical tool in the neurosurgical field. They could improve surgical field visibility with 4K three-dimensional (4K-3D) monitors and provide alleviation for the physical strain by allowing surgeons to maintain a neutral posture. In this study, we report our initial experience with exoscopic superficial temporal artery (STA)-middle cerebral artery (MCA) bypass compared to the conventional microscope at our institution. METHODS: This study included 58 patients with ischemic status, including internal carotid artery (ICA) stenosis, ICA occlusion,

The orbital grading system in pediatric moyamoya: an interrater reliability analysis of angiographic outcomes.

OBJECTIVE: The Matsushima grading system is widely used to evaluate angiographic revascularization outcomes after pial synangiosis for moyamoya vasculopathy by quantifying collateral ingrowth. The authors hypothesized that the orbital grading system (OGS), a new scale developed for adults to measure collateral ingrowth, would provide greater consistency and interrater reliability. They aimed to compare the performance of these scales for the first time in the pediatric population. METHODS: The authors performed a retrospective analysis of patients with moyamoya vasculopathy with follow-up cat

Bilateral STA-MCA bypass for Moyamoya angiopathy associated with severe erythrodermic psoriasis.

OBJECTIVE: We report on the uncommon association between severe erythrodermic psoriasis and moyamoya angiopathy (MMA), a progressive cerebrovascular disorder characterized by steno-occlusive changes in the circle of Willis. Concomitant moyamoya and severe erythrodermic psoriasis is a rare pathology, with unknown pathogenesis. MMA with severe erythrodermic psoriasis, even in the setting of stroke, is often managed with non-curative medical intervention alone, due to concerns for surgical instability. Here we show with appropriate surgical consideration and medical management, patients can under

Spontaneous Thrombosis of Giant Dissecting Fusiform Middle Cerebral Aneurysm After Double-Barrel Superficial Temporal Artery-Middle Cerebral Artery Bypass: A Case Report of Decision-Making in a Limited Resource Environment.

BACKGROUND: The treatment of giant fusiform middle cerebral artery (MCA) aneurysms remains daunting owing to their tendency to be associated with precarious end-vessel anatomy and the need for complex microsurgical techniques to appropriately address the aneurysm and the vasculature at risk. Extracranial-intracranial bypass revascularization remains a valuable tool for treating these complex lesions. In the present report, we have described a rare occurrence in which the creation of a double-barrel superficial temporal artery (STA-MCA) bypass facilitated spontaneous obliteration of the aneurys

Orphaned Middle Cerebral Artery Side-to-Side In Situ Bypass as a Favorable Alternative Approach for Complex Middle Cerebral Artery Aneurysm Treatment: A Case Series.

OBJECTIVE: Middle cerebral artery (MCA) aneurysms have continued to be primarily managed microsurgically. In cases of complex MCA aneurysms, revascularization could facilitate effective aneurysm treatment. The MCA candelabra provides excellent candidates for in situ side-to-side bypass. In the present case series, we have described applications of MCA in situ side-to-side bypass for the management of complex MCA aneurysms, along with a review of the pertinent data. METHODS: A retrospective review of a prospectively maintained neurosurgical patient database was performed to identify all patien

Acute Preoperative Infarcts and Poor Cerebrovascular Reserve Are Independent Risk Factors for Severe Ischemic Complications following Direct Extracranial-Intracranial Bypass for Moyamoya Disease.

BACKGROUND AND PURPOSE: Severe ischemic changes are a rare but devastating complication following direct superficial temporal artery to MCA bypass in patients with Moyamoya disease. This study was undertaken to determine whether preoperative MR imaging and/or cerebrovascular reserve assessment by using reference standard stable xenon-enhanced CT could predict such complications. MATERIALS AND METHODS: Among all adult patients undergoing direct bypass at our institution between 2005 and 2010 who received a clinically interpretable xenon-enhanced CT examination, we identified index cases (patie
