Acute sports-related spinal cord injury: contemporary management principles.

Improvements in helmet and equipment design have led to significant decreases in overall injury incidence, but no available helmet can prevent catastrophic injury to the neck and cervical spine. The most effective strategy for preventing this type of injury appears to be careful instruction, training, and regulations designed to eliminate head-first contact. The incidence of football-related quadriplegia has decreased from a peak of 13 cases per one million players between 1976 and 1980 to 3 per million from 1991 to 1993, mostly as a result of systematic research and an organized effort to eli

Pitfalls in computed tomography of the cervical and lumbar spine.

In 10 patients, evaluated by computed tomography studies of the spine, radiological findings alone may have led to misinterpretation of the clinical condition. Five patients demonstrated CT findings of cervical disc herniation or protrusion, while clinical examination resulted in the diagnosis of neuralgic amyotrophy. In contrast, 3 patients with clinical signs of compression of lumbar nerve roots had negative findings in CT studies of the lumbar discs. Additional slices at the level of the vertebral body, however, demonstrated free sequestration of disc substance. Two patients with signs of c

Long-term vocal outcomes of refined nerve-muscle pedicle flap implantation combined with arytenoid adduction.

The objective of this study is to evaluate long-term efficacy of refined nerve-muscle pedicle (NMP) flap implantation combined with arytenoid adduction (AA) for treatment of unilateral vocal fold paralysis (UVFP). The authors retrospectively reviewed 33 patients with UVFP who received refined NMP flap implantation with AA and were followed up over a 1-year period. Evaluation of vocal fold vibration (regularity, amplitude, and glottal gap), aerodynamic analysis (maximum phonation time [MPT] and mean airflow rate [MFR]), and perceptual evaluation (Grade and Breathiness) were performed preoperati

[Autoplasty of recurrent laryngeal nerve in the thyroid gland surgery].

Efficacy of autoplasty of recurrent laryngeal nerve (RLN) for laryngeal reinnervation in surgery of differentiated cancer of thyroid gland was studied. Prospectively 8 patients were examined, in whom laryngeal reinnervation, using the RLN autoplasty, for the abduction laryngeal paralysis was done. The examination was performed before and after the operation, it included videolaryngoscopy, acoustic analysis and the patient's self-estimation of psychosocial consequences of the voice-formation disturbance. Improvement of a vocal cords spacious positioning was noted in 38% patients, and was confir

Ultrasound-guided locoregional anaesthesia for carotid endarterectomy: a prospective observational study.

INTRODUCTION: Ultrasound guidance is increasingly used for invasive anaesthetic procedures to improve efficacy, facilitate performance and reduce risk of complications. Herein, we present a simple approach to ultrasound-guided locoregional anaesthesia for patients undergoing eversion carotid endarterectomy. METHODS: At the level of the base of the carotid bifurcation, the needle was inserted at the lateral border of the sternocleidomastoid muscle and, guided by ultrasound, advanced 0.5-1 cm posterolateral to the carotid artery, where ropivacaine (7.5 mg ml(-1)) was injected. During retraction

Cervicothoracic extradural arachnoid cyst: possible association with obstetric brachial plexus palsy.

The association of cervicothoracic extradural arachnoid cysts and obstetric brachial plexus palsy has not previously been reported. We report two patients with this association. The first patient is a 9-month-old boy with left obstetric brachial plexus palsy that developed bilateral leg weakness at 6 months of age owing to compression of the spinal cord by a C6 to T8 left cervicothoracic extradural arachnoid cyst. The second patient is a 3-year-old girl with bilateral brachial plexus palsy and spastic paraparesis who had magnetic resonance imaging at 3 days of age that showed intraspinal cord

Sleep disturbances are related to glymphatic dysfunction in blepharospasm.

Research has shown a close relationship between sleep and glymphatic function, with impaired glymphatic function potentially contributing to sleep problems in a bidirectional way. However, its role in sleep disturbances associated with dystonia remains unknown. This cross-sectional study aimed to investigate whether glymphatic function is impaired in dystonia and to explore its relationship with sleep disturbances. We conducted structural magnetic resonance imaging and diffusion tensor imaging (DTI) on two large cohorts: patients with blepharospasm (BSP, n = 45) and cervical dystonia (CD, n =

Dural sac shrinkage signs on magnetic resonance imaging at the thoracic level in spontaneous intracranial hypotension-its clinical significance.

BACKGROUND: Spontaneous intracranial hypotension (SIH) is secondary to a cerebrospinal fluid leak at the spinal level without obvious causative events. Several signs on brain and cervical spine magnetic resonance (MR) imaging (MRI) have been associated with SIH but can be equivocal or negative. This retrospective study sought to identify characteristic SIH signs on thoracic spinal MRI. METHODS: Cranial and spinal MR images of 27 consecutive patients with classic SIH symptoms, who eventually received epidural autologous blood patches (EBPs), were analyzed. RESULTS: The most prevalent findings

Multi-modal endovascular management of traumatic pseudoaneurysm and arteriovenous fistula of the ascending cervical artery: A technical report and review of literature.

OBJECTIVE: The management of traumatic pseudoaneurysm (PA) with concomitant arteriovenous fistula (AVF) arising from the thyrocervical trunk is challenging and rarely reported. Here, the usefulness of a multi-modal endovascular strategy for management of traumatic PA and AVF arising from the thyrocervical trunk is presented. A literature review describing unique clinical features and management strategies of traumatic vascular lesions of the thyrocervical trunk is included. METHODS: A 58-year-old man presented with two PAs arising from the ascending cervical artery (AsCA) and a robust AVF bet

An isolated double-crush-syndrome in posttraumatic thoracic outlet syndrome - A case report.

Thoracic outlet Syndrome (TOS) includes all disorders caused by compression of all neurovascular Structures in the upper thoracic outlet (Ross and Owners, 1966, Bürger and Arterien, 2014, Sanders and Annest, 2017). The Double-Crush-Syndrome (DBS) defines multilevel lesions along a neurovascular trunk caused by mechanical compression in different areas. Pectoralis-minor-syndrome (PMS) is also classified in the disorders of he upper thoracic outlet and was also known as hyperabductionsyndrome or subcoracoidal-syndrome. Between 2015-2019 our department treatet 488 patients suffering from neurolog

The influence of a depressed scapular alignment on upper limb neural tissue mechanosensitivity and local pressure pain sensitivity.

BACKGROUND: A depressed scapular alignment could lead to prolonged and repetitive stress or compression of the brachial plexus, resulting in sensitization of neural tissue. However, no study has investigated the influence of alignment of the scapulae on sensitization of upper limb neural tissue in otherwise asymptomatic people. In this case-control study, we investigate the influence of a depressed scapular alignment on mechanosensitivity of the upper limb peripheral nervous system as well as pressure pain thresholds (PPT). METHODS: Asymptomatic individuals with neutral vertical scapular alig

Brachial neuritis: an under-recognized cause of upper extremity paresis after cervical decompression surgery.

STUDY DESIGN: Case series. OBJECTIVE: To identify an alternative etiology for the development of upper extremity weakness after cervical spine surgery. SUMMARY OF BACKGROUND DATA: The development of proximal upper extremity paresis after cervical decompression surgery is commonly diagnosed as postoperative C5 palsy. Symptoms most commonly consist of weakness involving the deltoid and/or biceps brachii muscles, and in many patients there is also associated pain in the shoulder region with or without sensory deficits. Interestingly, the onset of symptoms is often delayed until days to weeks af

Relationship between the lateral dissection lines of various radical hysterectomies and anatomical structures in the female pelvis: an educational video from the Sapporo Cadaver Surgical Workshop.

OBJECTIVE: Radical hysterectomy with pelvic lymph node dissection is the treatment of choice for cervical cancer. All gynecologic oncologists should master this technique. The surgical procedure requires a wide dissection of the female pelvis. Performing radical hysterectomy without a thorough knowledge of the anatomy of the female pelvis can lead to serious complications. For novice surgeons to safely perform radical hysterectomy, mastering the 3-dimensional anatomy of the pelvis and aligning the dissection lines of radical hysterectomy to the female pelvic anatomy is crucial.
