Implementation of a Program to Enhance Cognitive Reserve in Patients With Multiple Sclerosis (EM Reserva Program).

BACKGROUND: Cognitive reserve (CR) may help mitigate cognitive decline in people with multiple sclerosis (PwMS); however, few interventions have targeted CR enhancement, and none have focused on individuals without baseline cognitive impairment. The EM Reserva program is a multimodal intervention that combines cognitive leisure activities, aerobic exercise, and structured social engagement and is designed to strengthen CR in individuals with relapsing-remitting MS (RRMS). METHODS: This pragmatic, single-center, observer-blinded randomized controlled trial included PwMS aged 18-55 years with R

Successful cognitive rehabilitation for acute right frontal aslant tract injury with impaired working memory and inattention using diffusion tensor tractography.

BACKGROUND: Language impairments caused by left frontal aslant tract (FAT) damage have a good prognosis. However, the prognosis of cognitive dysfunction after right FAT injury is unknown. The present study describes successful cognitive rehabilitation for impaired working memory and inattention together with diffusion tensor tractography (DTT). METHODS: A patient in his 40s with acute stroke underwent magnetic resonance imaging, computed tomography perfusion scans, DTT, and 11 neuropsychological tests for attention, frontal lobe, memory, visuospatial, intelligence, and cognitive functions. Hip fracture and cognitive impairment in older adults-integrated approaches to rehabilitation: a narrative review.

Fragility fractures, particularly hip fractures, represent a major public health concern among older adults and are associated with high morbidity, mortality, functional decline, and socioeconomic burden. Cognitive impairment is common in older adults with hip fractures and contributes to increased fracture risk, poor postoperative outcomes, delayed recovery, and higher rates of institutionalization. This review aimed to examine rehabilitation strategies for older adults with hip fractures, with a specific focus on considerations for those with cognitive impairment. Evidence suggests that indi

Cognitive remediation for depressed inpatients: Results of a pilot randomized controlled trial.

OBJECTIVE: Neurocognitive deficits that persist despite antidepressive treatment and affect social and vocational functioning are well documented in major depressive disorder. Cognitive training approaches have proven successful in ameliorating these deficits in other psychiatric groups, but very few studies have been conducted in unipolar depressive patients by now. In contrast to previous studies solely including outpatients, effects of a cognitive remediation intervention on neurocognitive functioning of depressed inpatients were assessed by the present study. METHOD: A randomized controll…

Cognitive-behavioral treatment of social phobia: new advances.

Cognitive-behavioral treatment (CBT) for social phobia is an effective treatment for many patients, but some patients do not benefit from the treatments and many remain symptomatic. Therefore, researchers have been examining techniques that may improve treatment outcome. In this paper, recent psychopathology and treatment outcome research, much of which supports the expectation that a second-generation CBT treatment may further improve outcome, are discussed. Finally, the authors present a number of CBT techniques that are tailored for the individual treatment of patients with social phobia. T

Understanding functional brain reorganization for naturalistic piano playing in novice pianists.

Learning to play the piano is a unique complex task, integrating multiple sensory modalities and higher order cognitive functions. Longitudinal neuroimaging studies on adult novice musicians show training-related functional changes in music perception tasks. The reorganization of brain activity while actually playing an instrument was studied only on a very short time frame of a single fMRI session, and longer interventions have not yet been performed. Thus, our aim was to investigate the dynamic complexity of functional brain reorganization while playing the piano within the first half year o

Dissociable functional responses along the posterior-anterior gradient of the frontal and parietal cortices revealed by parametric working memory and training.

While the storage capacity is limited, accumulating studies have indicated that working memory (WM) can be improved by cognitive training. However, understanding how exactly the brain copes with limited WM capacity and how cognitive training optimizes the brain remains inconclusive. Given the hierarchical functional organization of WM, we hypothesized that the activation profiles along the posterior-anterior gradient of the frontal and parietal cortices characterize WM load and training effects. To test this hypothesis, we recruited 51 healthy volunteers and adopted a parametric WM paradigm an…

Can We Train Multisensory Integration in Adults? A Systematic Review.

The ability to efficiently combine information from different senses is an important perceptual process that underpins much of our daily activities. This process, known as multisensory integration, varies from individual to individual, and is affected by the ageing process, with impaired processing associated with age-related conditions, including balance difficulties, mild cognitive impairment and cognitive decline. Impaired multisensory perception has also been associated with a range of neurodevelopmental conditions, where novel intervention approaches are actively sought, for example dysle

Virtual reality lead extraction as a method for training new physicians: a pilot study.

BACKGROUND: It is estimated that the demand for transvenous lead extraction (TLE) has reached an annual extraction rate of nearly 24,000 patients worldwide. Despite technologic advances, TLE still has the potential for significant morbidity and mortality. Complication rates with TLE directly parallel operator experience. However, obtaining adequate training during and postfellowship can be difficult. Given the potential for catastrophic complications and the steep learning curve (up to 300 cases) associated with this procedure, we sought to validate a virtual reality (VR) lead extraction simul

Brain stimulation prevents neural downregulation and optimizes learning.

Non-invasive brain stimulation, such as transcranial random noise stimulation (tRNS), has been shown to enhance cortical excitability and fa … igm, we investigated how tRNS modulates learning-related plasticity in the attention network during cognitive training (N = 37, age range=18-35 years old). In the sham condition we observed a significant decline in task-evoked BOLD activity within the attention network after the training and no behavioral improvement, suggesting neural changes associated with cognit …

Neural substrates of individual differences in learning generalization via combined brain stimulation and multitasking training.

A pervasive limitation in cognition is reflected by the performance costs we experience when attempting to undertake two tasks simultaneously. While training can overcome these multitasking costs, the more elusive objective of training interventions is to induce persistent gains that transfer across tasks. Combined brain stimulation and cognitive training protocols have been employed to improve a range of psychological processes and facilitate such transfer, with consistent gains demonstrated in multitasking and decision-making. Neural activity in frontal, parietal, and subcortical regions has…

Immersive training programs for schoolchildren: a systematic review.

Immersive technologies have been recently adopted by researchers to enhance students' learning skills, but few studies still exist on the schoolchildren population. Therefore, through a literature search on the main research databases (EBSCO, Web of Science, Scopus, PubMed) and after the selection process, 11 eligible papers were judged suitable for the systematic review. Comprehensively, the studies employed a sample of 401 children, of which 234 (58 %) were typical learners and 167 children had a specific learning disorder (42 %). The trainings targeted learning abilities (reading, writing,

Neurofeedback training for tourette syndrome: an uncontrolled single case study.

Gilles de la Tourette syndrome (TS) is characterized by motor and vocal tic manifestations, often accompanied by behavioral, cognitive and affective dysfunctions. Electroencephalography of patients with TS has revealed reduced Sensorimotor Rhythm (SMR) and excessive fronto-central Theta activity, that presumably underlie motor and cognitive disturbances in TS. Some evidence exists that neurofeedback (NFB) training aimed at enhancing SMR amplitude is effective for reducing tics. The present report is an uncontrolled single case study where a NFB training protocol, involving combined SMR uptrain

Treatment of panic disorder.

Panic disorder with or without agoraphobia occurs commonly in patients in primary care settings. This article assesses multiple evidence-based reviews of effective treatments for panic disorder. Antidepressant medications successfully reduce the severity of panic symptoms and eliminate panic attacks. Selective serotonin reuptake inhibitors and tricyclic antidepressants are equally effective in the treatment of panic disorder. The choice of medication is based on side effect profiles and patient preferences. Strong evidence supports the effectiveness of cognitive behavior therapy in treating pa

Effects of a 6-month dual-task, power-based exercise program on cognitive function, neurological and inflammatory markers in older adults: secondary analysis of a cluster randomised controlled trial.

Functional power-based exercise training can improve physical performance in older adults and cognitive training can improve measures of cognition, but their combined effects on cognition and related risk factors (neurological and inflammatory markers) remains uncertain. This 6-month cluster randomised controlled trial evaluated the effectiveness of dual-task functional power training (DT-FPT) on cognition and circulating neurological and inflammatory markers in older adults at increased falls risk, and whether intervention responses varied by apolipoprotein-E (ApoE) and brain derived neurotro…

Enhancing Working Memory Capacity in Persian Cochlear Implanted Children: A Clinical Trial Study.

INTRODUCTION: Sensory deprivations such as hearing impairment that affect sensory input have a secondary impact on cognitive functions such as working memory (WM). WM capacity is an important cognitive component that processes language-related activities. Moreover, several studies have shown a deficit in WM in children with a cochlear implant (CI). We aimed to assess the performance of children with CIs in pre- and post-training sessions and compare their scores on a battery of WM tests to investigate the efficacy of a WM training program. MATERIALS AND METHODS: Twenty-five children aged 7-10
