Brain Stimulation to Modulate Food Intake and Eating Behavior.

PURPOSE OF REVIEW: Appetitive behaviors are mediated through homeostatic and reward signaling of brain circuits. There has been increasing interest in the use of neuromodulation techniques aimed at targeting brain regions such as the lateral prefrontal and subcortical regions associated with dysregulation of eating behaviors. RECENT FINDINGS: Invasive brain stimulation techniques have demonstrated promising results in treating severe and enduring anorexia nervosa and morbid obesity. In addition, non-invasive techniques have been shown to successfully reduce food craving, hunger ratings, and c…

A 2015 Update on The Natural History and Diagnosis of Nicotine Addiction.

This review will describe the biological and behavioral aspects of nicotine addiction as it develops during adolescence and advances over a lifetime. Symptoms of physical nicotine dependence typically appear when adolescents are smoking only a few cigarettes each month. Physical dependence develops through a set sequence of symptoms that correlate with changes in brain structure in addiction circuits. Smokers often describe their symptoms of physical dependence as "wanting," then "craving," and eventually "needing" to smoke. These symptoms appear whenever the smoker goes too long without smoki…

Substance use disorders and the orbitofrontal cortex: systematic review of behavioural decision-making and neuroimaging studies.

BACKGROUND: Orbitofrontal cortex dysfunctions have been frequently documented in people with substance use disorders. The exact role of this … orbitofrontal cortex after detoxification. The association between hyperactivity of this region and craving or cue reactivity was not consistent across studies. CONCLUSIONS: The orbitofrontal cortex has an important role in addictive behaviours. Further studies are needed to elucidate the underlying neuronal substrates of cue reactivity, craving and deci …

Mood state and recent cocaine use are not associated with levels of cocaine cue reactivity.

Eighty-one cocaine-dependent outpatients were assessed for their reactions to cocaine-related cues in a laboratory setting. All subjects contributed a urine sample prior to the session. Compared with non-drug control cues, the cocaine stimuli produced increases in physiological arousal, self-reports of high, craving, and withdrawal, and self-reports of negative mood. Subjects who tested cocaine-positive on the day of testing differed only in skin resistance responding from those who tested cocaine-negative. Changes in cue-induced physiological and self-report measures were also not associated …

Interoceptive Processing in Substance Use Disorders (SUDs): From the Neuroanatomy to Insights from Computational Models and Predictive Coding Frameworks.

Substance use disorders (SUDs) represent complex public health challenges characterized by a blend of genetic, cognitive, environmental, and psychosocial factors. This chapter explores the critical role of interoceptive processing - the internal sensing of physiological states - in the neurobiology and treatment of SUDs. Interoceptive dysfunctions are highlighted as central to the craving, emotional regulation, and decision-making processes that underpin addictive behaviors. The importance of the insula in sustaining drug use, particularly nicotine, underscores a broader involvement of interoc…

Music therapy for people with substance use disorders.

BACKGROUND: Substance use disorder (SUD) is the continued use of one or more psychoactive substances, including alcohol, despite negative effects on health, functioning, and social relations. Problematic drug use has increased by 10% globally since 2013, and harmful use of alcohol is associated with 5.3% of all deaths. Direct effects of music therapy (MT) on problematic substance use are not known, but it may be helpful in alleviating associated psychological symptoms and decreasing substance craving. OBJECTIVES: To compare the effect of music therapy (MT) in addition to standard care versus …

Alcohol withdrawal symptoms predict corticostriatal dysfunction that is reversed by prazosin treatment in alcohol use disorder.

Chronic alcohol use increases risk of alcohol withdrawal symptoms (AW) and disrupts stress biology and resilient coping, thereby promoting excessive alcohol intake. Chronic alcohol intake and multiple alcohol detoxifications are known to impair brain medial prefrontal cortex (mPFC) and striatal functioning, regions involved in regulating stress, craving and alcohol intake. In two related studies, we examined whether AW predicts this functional brain pathology and whether Prazosin versus Placebo treatment may reverse these effects. In Study 1, patients with Alcohol Use Disorder (AUD) (N = 45) w…
