OBJECTIVE: A utriculus masculinum is encountered in 12% of hypospadias. However, patients rarely present with symptoms, such as post-void incontinence or infection. We describe a combined laparoscopic-cystoscopic approach to overcome the challenge posed by the location of the utriculus deep within the pelvis and to accurately identify the confluence of the utriculus with the urethra.

An unusual case of a 5-year-old boy with duplication of the glans, a rare variety of diphallia, is reported. The two glandes were arranged one above the other, the dorsal glans had only a blind pit while the ventral glans had a patent urethra with glanular hypospadias. A 6 year old Haflinger gelding was presented to the reporting clinics with a history of chronic dysuria. A large cystic calculus (12 x 9 x 9 cm) was diagnosed cystoscopically. Lithotripsy was carried out endoscopically in the standing, sedated patient with a Holmium:YAG surgical laser (2100 nm, 0.5-3.5 J/pulse, 3-60 pulses/sec.). In total urethral diverticula in children represent a very rare urological disorder. Primary localisation is the dorsal side of the urethra, ventral located diverticula are extremely rare. We report on a ten year old girl with a ventral, urethral diverticulum presenting with the symptoms of persisting enuresis. The diverticulum was excised through a median laparatomy. Placement of suprapubic catheter facilitated adequate postoperative drainage and postoperative cystourethrography confirmed the absence of extravasation.

We report on a new endoscopic approach to the removal of a migrated urethral stent following hypospadias repair. A flexible balloon-tipped guide wire allows the effective cystoscopic retrieval of a urethral stent, suggesting potential applications to the removal of other foreign bodies in the urinary tract.

Inserting a Foley catheter into a traumatized urethra with false channels or a post-transurethral resection of prostate (TURP) undermined bladder neck can be very difficult if not impossible at times. Likewise, replacing a Foley catheter that has fallen out in the early postoperative period after a radical prostatectomy can be difficult and can cause significant injury to the fresh anastomosis. We describe a technique using a Peel-Away sheath that fits over a cystoscope or a resectoscope, and facilitates accurate insertion of a Foley catheter without trauma to the urethra.

In a preliminary study of 32 women with stress urinary incontinence, a modification was added to the standard suprapubic urethropexy to decrease the incidence of postoperative urinary retention. Immediately before the elevating sutures are tied, a urethroscope is inserted into the midurethra and the bladder neck is visualized while the sutures are being tied with just enough tension to close the bladder neck. Experience with 20 endoscopic vesical neck suspensions (Stamey procedure) is compared to 20 matched anterior cystourethropexies (Marshall-Marchetti-Krantz procedure) performed by the same staff. The present procedure is a transvaginal vesical neck suspension, described in 1959 by Pereyra, modified in 1973 by Stamey and further modified in 1975 by Mason. The long-term success rate for relief of urinary stress incontinence was 90 per cent with either procedure. Advantages and disadvantages of the endoscopic vesical neck suspension are discussed.
