Diastatic damage of the caecum is only described in left colon neoplastic obstructions. However, diverticular sigmoid stenosis can likely cause cecal diastatic distention. In emergency, ileo-cecal resection removing the area of diastatic damage or externalizing the cecal perforation can be an interesting alternative to subtotal colectomy. The left colonic stenosis is treated later so the colon can be spared with better long-term functional outcome than after subtotal colectomy.

Hirayama disease is a rare juvenile amyotrophy that is often misdiagnosed as an unrelated, relentlessly progressive disease. Canaliculops is a non-inflammatory dilatation of the canaliculus which can be secondary to trauma, punctal agenesis, herpetic infections or chronic drug use. This condition can affect either canaliculi and has shown female preponderance. Treatment described so far has consisted of cyst marsupialization with no literature on the post-operative epiphora in these cases. We describe two cases of canaliculops with punctal agenesis where we have tried to establish lacrimal patency.

Marfan syndrome (MFS) is an autosomal dominantly inherited connective tissue disorder, mostly caused by mutations in the fibrillin-1 (FBN1) gene. Marfan syndrome is a connective tissue disorder that may be complicated during pregnancy by aortic dissection. Caesarean section may be selected to avoid the haemodynamic challenges of vaginal birth. The common occurrence of dural ectasia in patients with Marfan syndrome is known to be associated with failed neuraxial anaesthesia. This report describes the administration of spinal anaesthesia to a woman with Marfan syndrome, and discusses why a hypobaric bupivacaine and opioid mixture, warmed to 37°C and injected intrathecally with the patient seated, produced successful surgical anaesthesia.

OBJECTIVE: To evaluate the efficacy of endoscopic balloon dilation for subglottic stenosis.
METHODS: The clinical outcomes of 9 pediatric patients treated with balloon dilation were retrospectively analyzed. There were 8 male and 1 female. Their age ranged from 7 months to 7 years (mean 2.5 years). The degree of stenosis was classified according to Myer-Cotton classification as follows: grade Ⅱ (n=7) and grade Ⅲ (n=2). The causes of the stenosis were postintubation lesions (n=8) and laryngeal penetrating trauma (n=1).
RESULTS: The patients had undergone 1 to 3 dilations with an average of 1.

We report a case of advanced aortic sinus aneurysm without the aggravation of aortic valve regurgitation after a simple reduction procedure for dilated aortic root base and sinotubular junction (STJ) in a Marfan patient with annuloaortic ectasia and severe aortic valve regurgitation. The aortic valve competence was well preserved by limited change of distances among the interleaflet triangles after reduction of the dilated aortic root base and STJ in the Marfan patient, although the sinus dilatation was aggravated.
