Surgery for non-Meckel's small-bowel diverticular perforation: two case reports and a literature review.

BACKGROUND: Similar to colonic diverticula, small-intestinal diverticula are often asymptomatic, but may cause life-threatening acute complications. Non-Meckel's small-bowel diverticular perforation is rare, and the rate of mortality is high. However, there is currently no consensus regarding its therapeutic management. CASE PRESENTATION: Case 1: A 73-year-old Japanese man with localized lower abdominal pain was referred to our hospital. Enhanced computed tomography (CT) revealed diverticulitis of the small intestine, which was managed conservatively. Four days after admission, abdominal pain

Visualization of segmental arterialization with arrival time parametric imaging using Sonazoid-enhanced ultrasonography in portal vein thrombosis: A case report.

A 55-year-old male was admitted in mid-April 2011 with a fever of >39°C and pain in the lower right abdomen. A medical examination revealed sepsis originating from colonic diverticulitis. Abdominal B-mode ultrasonography (US) performed on admission detected thrombi in the superior mesenteric vein and in the right branch of the hepatic portal vein. Arrival time parametric imaging (At-PI) using Sonazoid-enhanced US showed arterialization of the entire right lobe of the liver. The treatment for the sepsis and portal thrombi that had been started upon admission dissolved the thrombi by day 22, wit

Bowel obstruction: evaluation with CT.

Eighty-four computed tomographic (CT) scans from patients referred for bowel obstruction between January 2, 1988, and December 31, 1989, were retrospectively evaluated. A pair of radiologists without knowledge of patient histories determined the presence or absence of bowel obstruction. Sixty-four patients ultimately proved to have intestinal obstruction, and 20 did not. Diagnosis was established by means of surgery (n = 39), barium studies (n = 17), and clinical course (n = 28). Causes of obstruction included adhesions (n = 37), metastases (n = 6), primary tumor (n = 7), Crohn disease (n = 4)

Epiploic Appendagitis in Three Young Men: CT Diagnosis, Clinical Mimics, and Conservative Management.

Epiploic appendagitis (EA) is an uncommon, self-limiting inflammation of the small fat-filled sacs along the colon, occurring in approximately 0.3-1% of suspected appendicitis cases and 2-7% of suspected diverticulitis cases. EA can be primary (due to torsion or venous thrombosis) or secondary (associated with adjacent inflammatory conditions). We present a series of three male patients with primary EA, aged 25-37 years, who presented with sudden, localized abdominal pain variably involving the left lower quadrant, right lower quadrant, or flank. All were hemodynamically stable and afebrile; l

Colovesical Fistula due to Sigmoid Diverticulitis.

INTRODUCTION: The incidence of colonic diverticulosis has risen significantly. Diverticular disease is the most frequent cause of colovesical fistulas, which are uncommon complications of diverticulitis. Clinical signs, such as fecaluria and pneumaturia, are typically required to confirm its presence. Finding the cause of the disease so that the proper therapy can be started is the primary goal of a diagnostic workup rather than observing the fistula tract itself. Case Presentation. We present a 43-year-old man complaining of frequent urinary tract infections for six months. On CT abdomen and

Diagnosis of appendiceal diverticulitis by multidetector computed tomography.

PURPOSE: Because appendiceal diverticulitis (AD) has a high risk of perforation, precise diagnosis is important for patients with suspected acute appendicitis (AA). In this study, we aimed to reveal the characteristics of computed tomography (CT) images of AD. METHODS: CT images were comparatively analyzed in patients who underwent appendectomy and had histopathologically proved AD (n = 23) or AA (n = 365) to reveal specific findings of CT image for AD and determine sensitivity and specificity of CT for AD. RESULTS: Univariate analysis showed that maximal diameter of the appendix in AD was s

Gossypiboma presenting as coloduodenal fistula--report of a rare case with review of literature.

The term gossypiboma is used to describe a mass of cotton matrix left behind in a body cavity intraoperatively. The most common site reported is the abdominal cavity. It can present with abscess, intestinal obstruction, malabsorption, gastrointestinal hemorrhage, and fistulas. A 37-year-old woman presented with pain in the right hypochondrium for 2 months following open cholecystectomy. As she did not improve with proton pump inhibitors, an esophagogastroduodenoscopy (EGD) was done, which showed a possible gauze piece stained with bile in the first part of the duodenum. Contrast-enhanced compu

An Unusual Presentation of Diverticular Colovesical Fistula: Acute Renal Failure With Bilateral Emphysematous Pyelonephritis.

Colovesical fistula (CVF) is an abnormal connection between the urinary system and the bowel. It is a known complication of acute diverticulitis. Common symptoms include pneumaturia, fecaluria, and abdominal pain, amongst others. In this case, we report a rare case of acute renal failure due to a relatively asymptomatic colovesical fistula causing left-sided emphysematous pyelonephritis complicated by renal abscess. After appropriate surgical treatment and antibiotic therapy, the CVF, pyelonephritis, and abscess resolved. Unfortunately, renal function did not return to baseline, likely due to

[Abdominal actinomycosis. Ileo-vaginal fistula as a clinical manifestation. Case report and review of the literature].

In 1985, a resection of the sigmoid colon was performed on an 81-year-old patient with recurrent and stenotic sigmoid diverticulitis. Vaginal hysterectomy was carried out at the same time for adenomycosis. After an uneventful recovery, approximately 1 year later the patient was presented with fetial leucorrhoea. Radiologically, a fistula was apparent between the terminal ileum and the vagina. Adhesions between the terminal ileum and the vaginal stump were surgically resolved and an ileum segment resection performed. Postoperative recovery was smooth and the patient has remained symptom-free si

Experience with Laparoscopic and Robotic Colon Surgery Together with Other Major Minimally Invasive Procedures for Unrelated Pathologies.

Background: Indications for combined colon surgery together with other procedures include oncologic multivisceral resections and abdominal trauma. It is unclear if combining minimally invasive (MI) colon surgery with unrelated other procedures increases the risk for complications. Patients and Methods: The surgical database from two institutions during a 10-year period was queried for combined colon surgeries together with other interventions. All open cases, combined cases performed for one pathology and MI colectomies together with a minor procedure, were excluded. Results: Median age of the

Segmental left colectomy: a modified caudal-to-cranial approach.

BACKGROUND: We have designed a modified caudal-to-cranial approach to perform laparoscopic left colectomy preserving the inferior mesenteric artery for benign colorectal diseases. METHODS: IRB approval and informed consent have been obtained. A dissection is conducted to separate the descending mesocolon of the Gerota's plan from the medial aspect to the peritoneal lining to the left parietal gutter. The peritoneal layer is incised parallel to the vessel and close to the colonic wall. The dissection is continued anteriorly up to reach the resected parietal gutter. A passage into the mesentery

[Restoration of colonic continuity after Hartmann's operation].

A retrospective analysis of data from 69 patients treated by Hartmann's operation between 1981 and 1991 determined prognostic factors for colon continuity re-establishment and the mortality of this second intervention. The 15 patients who died during the first month after the Hartmann's operation were excluded from the study, the 54 survivors including 32 men and 22 women, mean age 68 +/- 12 years (range 19 to 87 years). The initial indication for surgery was: complicated sigmoid diverticulis (n = 26), cancer of colon (n = 14) or other site (n = 14). Colon continuity was re-established in 23 p

Case report: concurrent sigmoidectomy and schwannoma resection in a patient with recurrent diverticulitis.

Benign peripheral nerve sheath tumors are growths that develop in the protective sheath that surround peripheral nerves. In rare circumstances, they can cause mass effect on surrounding structures intra-abdominally. We report an otherwise healthy 62-year-old female who presented for constipation and recurrent bouts of diverticulitis. The patient had known about her nerve sheath tumor but did not want it removed. The initial plan was to perform a robotic Sigmoidectomy; however, this was not feasible given the size of the tumor intra-operatively. The procedure was subsequently aborted. A multi-d

Non-operative management of gallstone sigmoid ileus in a patient with a prostatic cancer.

Gallstone ileus is an uncommon complication of calculus cholecystitis through the formation of a biliary enteric fistula. The risk of mechanical obstruction caused by gallstones is increased with its size, in addition to chronic constipation, neoplasm and diverticulitis, to name a few. Here, we present a case of an 89-year-old male patient who presented with signs of bowel obstruction, which was found to be a gallstone impacted in the sigmoid colon. Considering the patient's stable condition and his comorbidities, a conservative approach was opted including IV fluids, fleet enema and bowel res

Complete Genome Sequence of bla IMP-6-Positive Metakosakonia sp. MRY16-398 Isolate From the Ascites of a Diverticulitis Patient.

A novel species of carbapenemase-producing Enterobacteriaceae (CPE) was isolated from a patient diagnosed with sigmoid colon diverticulitis. At first, laboratory testing suggested it was Klebsiella oxytoca or Pantoea sp.; however, a complete genome sequence of the isolate, MRY16-398, revealed that it could be novel species, most similar to [Kluyvera] intestini, of which taxonomic nomenclature is still under discussion. Orthologous conserved gene analysis among 42 related bacterial strains indicated that MRY16-398 was classified as the newly proposed genus Metakosakonia. Further, MRY16-398 was

Diverticulitis-induced pylephlebitis possibly misdiagnosed as biliary duct obstruction.

CASE: A 59-year-old Asian man presented to our emergency department with hypogastrium pain, loss of appetite, and diarrhea. On admission, he was hypotensive and jaundiced. Laboratory test results revealed thrombocytopenia, hypercreatininemia, and hyperbilirubinemia. Color Doppler sonography showed no blood flow in the right and left branches of the portal vein, which seemed similar to biliary obstruction. Enhanced computed tomography showed portal vein thrombi, consistent with pylephlebitis; a broad-spectrum antibiotic and an anticoagulant were administered. OUTCOME: The patient died of multi

[Surgical therapy of complicated diverticular disease].

From 1975-1989 55 patients were operated on for complicated diverticular disease at our unit. Intraoperative we found the following complications: 21 walled of perforations, 22 stenosis of the sigmoid colon combined with obstruction of the small and/or large bowel, 8 free perforations with generalized, faecal peritonitis, 7 diverticular fistulae (5 colovesical, 1 colojejunal and 1 colocutaneous fistula) and diverticular bleedings. In 33 cases we performed a resection with primary anastomosis (8 times with protecting stoma). 17 times the Hartmann's procedure was carried out and 5 times a transv

Colovesical and rectovesical fistulas.

Colovesical or rectovesical fistulas develop because of a congenital abnormality, a traumatic incident, or an underlying disease, such as inflammation or cancer. Inflammation, either diverticulitis or Crohn's disease, was the cause of fistula in 73 (67%) of 109 patients with colovesical or rectovesical fistulas seen at the Mayo Clinic, Rochester, Minn, between 1965 and 1980. Sixty-six of the 73 patients had colovesical fistulas, and 64% of them underwent resection and anastomosis in one stage. Cancer of the cervix, prostate, bladder, colon, or rectum was responsible for 27 fistulas, equally di

Prospective evaluation of functional outcomes after laparoscopic sigmoidectomy with high tie of the inferior mesenteric artery for diverticular disease in consecutive male patients.

BACKGROUND: To date, there has been no consensus concerning the vascular approach during sigmoid colectomy for diverticular disease. The aim of this study was to determine the functional impact of elective laparoscopic sigmoidectomy performed with high ligation of the inferior mesenteric artery for diverticulitis in consecutive male patients. METHODS: Twenty-five consecutive patients of median age 53 years were enrolled in a prospective single-centre pilot study at a tertiary teaching hospital. Main outcome measures were functional results. Patients were asked to complete standardized, valida
