Technique and indications of elbow arthroscopy. A twelve-cases report.

AIM: Describe the technique of the elbow arthroscopy and discuss their indications through a series of 12 cases and a review of the literature. METHODS: We report a retrospective study of a short series of 12 cases of elbow arthroscopy performed on 4 years. The indications of this technique were loose bodies in 7 cases, parcellar displaced fracture of radial head in 3 cases, fracture of coronoid process in one case and osseous impingement in one case. RESULTS: We have noted no complications and at a mean follow up of 18 months (6 to 40 months) our results were almost satisfactory. CONCLUSIO…

Responsiveness of the DASH questionnaire for surgically treated tennis elbow.

This study evaluates if the Disabilites of the Arm, Shoulder and Hand (DASH) questionnaire is an adequately responsive outcome measure in lateral extensor release for tennis elbow. To measure responsiveness (sensitivity to clinical change), 21 patients with operated tennis elbow completed the DASH questionnaire pre-operatively and again postoperatively. The effect size (2.03) and standardized response (1.54) mean that, as well as the correlation with patient satisfaction and the Roles & Maudsley score (10), the DASH questionnaire is a useful tool to evaluate the outcome of surgical treatment o…

Ultrasonographic swelling ratio in the diagnosis of ulnar neuropathy at the elbow.

High-resolution ultrasound can demonstrate focal nerve enlargement in entrapment neuropathies. We hypothesized that a ratio between the nerve cross-sectional area at the site of maximal enlargement and at an unaffected site may improve diagnostic accuracy in ulnar neuropathy at the elbow (UNE), when compared to a single measurement at the site of maximal enlargement. Ultrasound was used to measure the cross-sectional area of the ulnar nerve at three sites in 30 normal, healthy controls and 26 individuals with UNE. In individuals with UNE, the ratio was 2.9:1 when the site of maximal swelling w…

[The elbow joint - a diagnostic challenge : anatomy, biomechanics, and pathology].

The elbow is one of the most commonly injured joints in sports activities. In particular, weight lifters, golfers, tennis players, and pitchers are affected. Injuries in sports involving overhead throwing are commonly based on the pathophysiologic model of valgus extension overload syndrome. The injuries are commonly complex and demand a good knowledge of the symptoms, the exact anatomy, and the biomechanics to arrive at a precise radiologic diagnosis. The characteristic patterns of injury that occur in specific sports activities are related to a combination of increased varus or valgus and ex…

Osteoarthritis of the elbow: results of arthroscopic osteophyte resection and capsulectomy.

Primary osteoarthritis of the elbow involves formation of bony spurs, loose bodies, and capsular contracture. This article presents the results of an arthroscopic technique for treatment of elbow arthritis in a series of patients. A retrospective chart review identified 41 patients with primary osteoarthritis in 42 elbows who underwent arthroscopic osteophyte resection and capsulectomy and who had more than 2 years of follow-up. Preoperative motion, pain, and Mayo Elbow Performance Index scores were compared with those at the latest follow-up. At an average follow-up of 176.3 weeks, significan…

Arthroscopic and open options for surgical treatment of chondromatosis of the elbow.

PURPOSE: The purpose of this study was to evaluate the treatment-specific outcome of surgical therapy of chondromatosis of the elbow with special attention on the complication and recurrence rate as well as induction of secondary osteoarthritis. METHODS: A consecutive patient group from 1989 to 2003 was evaluated retrospectively and followed up clinically and radiologically. Special attention was paid to locking and catching, swelling, pain, loose bodies, and signs of osteoarthritis. In the case of a causal pathology, mostly osteoarthritis, the condition was classified as secondary; otherwise…

A new case of hairy elbows syndrome (hypertrichosis cubiti).

Hairy Elbows Syndrome (Hypertrichosis Cubiti; OMIM# 139600) is a rare syndrome, and characterized by the presence of long vellus hair localized on the extensor surfaces of the distal third of the arms and proximal third of the forearm bilaterally. Occasionally hypertrichosis of other body regions may accompany hairy elbows. About half of the reported patients have short stature. Aside from short stature other relatively rare abnormalities related with this syndrome were also described. Most of the reported cases were sporadic, but autosomal dominant as well as autosomal recessive inheritance p…

Open reduction for late-presenting posterior dislocation of the elbow.

PURPOSE: To evaluate results of open reduction for late-presenting (more than 3 weeks) posterior dislocation of the elbow in 10 patients. METHOD: Elbow stiffness was the main indication for surgery. The mean age of the patients was 34 (range, 13-65) years; the mean time since injury was 4 (range, 2-6) months. All patients had non-functional elbow movement for any activity of daily living. Three patients had associated fractures around the elbow joint. RESULTS: At a mean follow-up of 19 (range, 11-28) months, 8 patients regained a functional range of movement for activities of daily living an…

Divergent dislocation of the elbow in an 11-year-old child.

In this report divergent dislocation of the elbow in an 11-year-old boy and its outcome is presented. His right upper limb was trapped in a water irrigation machine and sustained an extreme pronation thrust through the hyperextended elbow. He had also suffered from a segmental fracture of both bones of the forearm and a fracture of the humerus. The median and ulnar nerves were functionless at his presentation. Considering the 16 cases described in the literature, different types of divergent dislocation of the elbow and the plausible mechanisms in producing this type of injury are reviewed.

Dislocation of the elbow with fractures of the coronoid process and radial head.

INTRODUCTION: The aim of the study was to evaluate the factors relevant to prognosis after operative treatment of an elbow dislocation/fracture involving the coronoid process and the radial head. In 30-50% of cases, elbow dislocations are accompanied by concomitant bony injuries. Here, the ulnar coronoid process and the radial head are particularly crucial to the stability of the elbow joint. MATERIALS AND METHODS: In a retrospective study, 27 out of 37 patients who were treated surgically in our clinic between 1990 and 1999 for elbow dislocation with involvement of the coronoid process and t…

Lateral epicondylitis is one of the most common upper extremity pain syndromes. We report the results of patients in whom conservative treat … f the Arm, Shoulder and Hand questionnaire was 9 (range, 0-37). Preoperatively, 7 patients had full elbow range of motion; however, in 3 patients, there was a loss of extension at the elbow ranging from 7 degrees to 20 degrees preoperatively. The range of elbow motion was full in all patients postoperatively. No patient demonstrated posterolateral pain …

Reconstruction with an osteochondral autograft for advanced osteochondritis dissecans of the elbow.

UNLABELLED: The treatment of large, advanced osteochondritis dissecans of the elbow is controversial. To ascertain whether better results could be obtained using osteochondral autografts, we retrospectively reviewed the results in 10 young athletes (mean age, 14.3 years; range, 12-17 years) who were followed up for a mean of 25.5 months (range, 18-45 months). After abrasion of the fragments, cylindrical osteochondral bone plugs were transferred from a lateral femoral condyle. They were assessed clinically by the Japanese Orthopaedic Association elbow score and radiologically by radiocapitellar…

Occult periarthrosis of the shoulder: a possible progenitor of tennis elbow.

Nineteen outpatients, nine women and ten men, all right-handed, ranging in age from 33 to 87 yrs, with an average age of 53.5 yrs, presented with complaints of lateral epicondylitis (i.e., tennis elbow). Each was subsequently identified as having a heretofore unrecognized loss of ipsilateral shoulder internal rotation. The authors suggest that in this scenario the occult shoulder periarthrosis is interlinked in a pathokinetic chain potentially predisposing to the presenting symptoms of tennis elbow. During a tennis serve, excessive wrist flexion may be substituted as to compensate for a restri…

Radiography of the elbow for evaluation of patients with osteochondritis dissecans of the capitellum.

OBJECTIVE: This study was performed to determine the ability of a routine radiographic examination of the elbow to detect osteochondritis dissecans of the capitellum and associated intra-articular loose bodies. DESIGN AND PATIENTS: The study group consisted of 15 patients with osteochondritis dissecans of the capitellum confirmed by surgery or magnetic resonance imaging. Seven of the 15 patients had associated intra-articular loose bodies confirmed by surgery. All 15 patients had anteroposterior and lateral radiographs and magnetic resonance imaging of their symptomatic elbow. Nine of the 15 …

Bilateral intraneural perineurioma presenting as ulnar neuropathy at the elbow.

We describe a 36-year-old woman with progressive bilateral ulnar neuropathy. Sonographic and magnetic resonance imaging studies revealed extensive focal ulnar nerve enlargement at the elbow. Histological studies gave evidence of an intraneural perineurioma. Because intraneural perineurioma usually appears as a single mass lesion at sites other than typical entrapment sites, this mode of presentation is unusual. We discuss the nature of this benign tumor and the differential diagnosis of nerve enlargement. Knowledge of possible causes of nerve thickening is crucial when performing imaging in pa…
