Clinical comparison of the over-the-top and flexor carpi ulnaris split approaches for the treatment of anteromedial facet fracture of the coronoid process.

BACKGROUND: The flexor carpi ulnaris (FCU)-split approach and the over-the-top approach have been used frequently for the fixation of anteromedial facet (AMF) fractures of the coronoid process. Clinical studies have not compared functional recovery and complication rates associated with these approaches. This study aimed to compare the clinical effectiveness of the over-the-top and FCU-split approaches for the treatment of AMF fractures of the coronoid process. METHODS: Thirty-two patients who underwent surgery for AMF fractures between January 2013 and August 2019 were divided into the FCU-s

Outcome of Radial Head Arthroplasty in Comminuted Radial Head Fractures: Short and Midterm Results.

BACKGROUND: Comminuted radial head fractures are often associated with secondary injuries and elbow instability. OBJECTIVES: The aim of this retrospective study was to evaluate how well the modular metallic radial head implant EVOLVE® prosthesis restores functional range of motion (ROM) and stability of the elbow in acute care. PATIENTS AND METHODS: Eighty-five patients with comminuted radial head fractures and associated injuries received treatment with an EVOLVE® prosthesis between May 2001 and November 2009. Seventy-five patients were available for follow-up. On average, patients were fol

Revision total elbow replacement.

The range of general and specific adverse event in total elbow arthroplasty is similar in principle and practice to all other revision prosthetic arthroplasty but with three particular challenges: loss of humeral and ulnar bone stock; insufficiency of the extensor 'mechanism'; and the management of the ulnar nerve. Total elbow replacement is presently performed for the management of complex non-reconstructable distal humeral fractures in osteoporotic bone, for post-traumatic arthropathy, and for medically managed inflammatory arthritides in which metaphyseal bone architecture is often preserve

Temporary Cemented Spacer for Complex Radial Head Fractures: A Case Report.

BACKGROUND The management of comminuted and displaced radial head fractures (Mason III), particularly when associated with dislocation (Mason IV) or Essex-Lopresti injury, presents a complex surgical challenge. Treatment options include conservative management, surgical fixation, arthroplasty, or radial head resection; however, no consensus exists regarding the optimal approach for all injury types. Radial head resection is often preferred in complex cases with multiple fragments where reconstruction is unfeasible. While functional and radiological outcomes vary widely, there is a growing tren

Coronal shear fracture of the distal humerus in an 11-year-old patient.

Coronal shear fractures of the distal humerus are uncommon in the pediatric population. We present the case of an 11-year-old girl who sustained a coronal shear fracture of the distal humerus, requiring open reduction and internal fixation with the use of headless screws. At the time of the latest follow-up, she had fully recovered the range of motion on the affected limb, had returned to full physical activities without limitations, radiographs showed a completely healed fracture with restoration of the articular line, and there was no signs of avascular necrosis of the capitellum.

Coronal shear fracture of distal humerus associated with olecranon fracture: A case report and pathomechanism.

Distal humeral coronal shear fractures are rare and account for around 1% of all fractures at the elbow level and 6% of the distal humeral fractures. Many studies have reported on the associated injuries involving the lateral collateral ligament injuries and radial head fractures in up to 40% and 30% of humeral fractures, but this combination of coronal shear fracture of the trochlea and capitellum associated with olecranon fracture is unusual. Here, we report two cases of distal humerus coronal shear fracture with olecranon fracture treated surgically and establish and discuss the pathomechan

[Elbow joint dislocation : Frequent concomitant injuries and current treatment concepts].

Dislocations of the elbow joint are among the most prevalent dislocation injuries in the human body after shoulder joint dislocations and represent a challenge in the clinical routine because of the concomitant injuries and complications. They predominantly affect young adults who become injured during athletic or daily activities. A distinction is generally made between a simple elbow dislocation and a dislocation fracture of the elbow; however, a uniform classification or treatment algorithm has not yet been established, especially for simple elbow dislocations with associated ligamentous, m

Usefulness of digital measurements for functional evaluation of paediatric elbow range of motion.

BACKGROUND: Elbow immobilization due to fractures of the upper limb is frequent in paediatric patients. Proper follow-up is critical to assess elbow functional recovery. Telemedicine can be an option for remote monitoring of these patients. The purpose of this study was to compare personal and virtual evaluation of elbow range of motion after long arm cast withdrawal in paediatric patients. METHODS: An observational cross-sectional study was carried out which included all paediatric patients with elbow immobilization in long arm casts treated at our centre. After cast withdrawal, elbow range

[GUEPAR I total elbow arthroplasty in rheumatoid arthritis: 19 implants followed an average of 67 months].

INTRODUCTION: The GUEPAR I total elbow arthroplasty is a nonconstrained implant used since 1985. Only one multicenter study has reported the mid-term results of this implant in rheumatoid arthritis. We presented a monocentric retrospective study evaluating the results of 19 GUEPAR I total elbow arthroplasty in rheumatoid arthritis with a mean follow-up of 67 months. MATERIALS AND METHODS: Between 1988 and 1996, 19 GUEPAR I total elbow arthroplasties have been performed on 16 patients (3 bilateral). There were 15 women and one man, averaged age 58 years. Radiographically, the elbow was classif

Biomechanics of plate fixation following traditional olecranon osteotomy versus novel proximal ulna osteotomy for visualizing a distal humerus injury.

After a distal humeral injury, olecranon osteotomy (OO) is a traditional way to visualize the distal humerus for performing fracture fixation. In contrast, the current authors previously showed that novel proximal ulna osteotomy (PUO) allows better access to the distal humerus without ligamentous compromise. Therefore, this study biomechanically compared plating repair following OO versus PUO. The left or right ulna from eight matched pairs of human cadaveric elbows were randomly assigned to receive OO or PUO and repaired using pre-contoured titanium plates. Destructive and non-destructive mec

Induced Membrane Reconstruction of a 6-cm Ulnar Defect With Concurrent Revision Bone Grafting of Radial Nonunion Following Gustilo-Anderson Type IIIB Open Forearm Fracture: A Case Report.

The induced membrane technique (IMT) is a two-stage reconstructive procedure for critical-sized bone defects. We report a 31-year-old male who sustained a Gustilo-Anderson grade IIIB open fracture of the radius and ulna following a high-energy dirt bike injury, complicated by Serratia marcescens fracture-related infection and septic nonunion. Intraoperative debridement at Stage 1 revealed a 6-cm defect of the ulna, which was managed with an antibiotic-impregnated polymethylmethacrylate (PMMA) spacer. A concomitant radial nonunion, identified on postdebridement CT, was addressed at Stage 2 with

Bridging Myositis Ossificans After Supracondylar Humeral Fracture in a Child: A Case Report.

Background: Myositis ossificans is an uncommon complication of trauma and surgery, defined as ossifying changes in a non-osseous tissue such as muscles. It happens after tissue injury, with or without fractures. When myositis ossificans occurs around a joint, it can cause ankylosis, leading to complete dysfunction of the joint. Though it has been described in most parts of the body, bridging myositis ossificans involving the elbow joint were scarcely reported. Case Presentation: We report a severe case of myositis ossificans after a supracondylar humerus fracture in a 9-year-old child. In this

Biomechanical evaluation of suture-augmented locking plate fixation for proximal third fractures of the olecranon.

OBJECTIVES: To describe a method of suture augmentation of locking plate fixation (PF) of proximal olecranon fractures and to evaluate the biomechanical effectiveness of the suture augmentation using a human cadaveric model. METHODS: Six matched pairs of cadaveric elbows were used. Proximal one-third fractures of the olecranon were simulated via a transverse osteotomy. Identical locking PF was performed on each elbow using olecranon locking plates. One elbow of each pair was assigned to suture augmentation of the construct. The choice of left/right specimen for augmentation was performed in a

Double-locking precontoured plating system for malunited fractures of the distal end of humerus.

BACKGROUND: Distal humeral malunions are uncommon injuries, often associated with limited elbow motion, pain, instability, weakness, and sometimes ulnar neuritis. The complex anatomy of the elbow joint makes this condition one of the most complex elbow injuries to treat. MATERIALS AND METHODS: Four patients were treated by the same surgeon between 2011 and 2013 using a double-locking precontoured plating system for malunited intra-articular or extra-articular fractures of the distal end of the humerus. RESULTS: At a mean 3 years of follow-up, a significant improvement in the elbow motion and

Terrible triad of the elbow.

The terrible triad of the elbow is a difficult injury with historically poor outcomes. Improved experience, techniques, and implants have advanced to the point where restoration of elbow stability can be expected. Careful attention to each destabilizing element of the injury pattern is essential and places high demands on the surgeon's mastery of the anatomic complexity of the elbow. Technically, the surgeon must bring every skill to bear, as soft tissue techniques, fracture repair, and joint arthroplasty are routinely required to adequately treat these complex constellations of injury.

[Terrible triad injuries].

The complex injury pattern of a posterior elbow dislocation with concomitant radial head and coronoid fractures is usually referred to as a terrible triad injury. Due to the concomitant compromise of several osteoligamentous structures of the elbow joint relevant for stability, these injuries pose a particularly major challenge to the treating trauma surgeons. For this reason, a careful preoperative analysis of all relevant injury components is mandatory in order to make an adequate treatment decision. In most cases, surgical treatment addressing all elements relevant for stability is necessar
