Intraarticular radial head (IARH) fractures are uncommon pediatric injuries with unpredictable yet poor outcomes. The aim of this study was to evaluate clinical outcomes of IARH fractures in pediatric and adolescent patients, by testing our hypothesis that surgically managed fractures would have less risk for an unplanned second surgery and better elbow range of motion at the final follow-up. A retrospective review of 53 IARH fractures was performed. Demographic and clinical data were recorded. Concomitant and associated injuries were documented. Initial management and any attempted reduction

Chronic posterior elbow soft tissue defects often require soft tissue reconstruction because of exposure of the underlying triceps tendon and proximal ulna. Current options for soft tissue coverage require sacrifice of a local muscle or microsurgery. The purpose of this study is to evaluate patient and surgical outcomes after reconstruction of small-sized to medium-sized (<50 cm) posterior elbow defects with bipedicle advancement flaps. A retrospective chart review was performed for 3 patients who underwent posterior elbow reconstruction with bipedicle flaps. The etiology of the soft tissue el

Traumatic distal humerus epiphysiolysis (TDHE) is a rare injury in infants with an incidence of about 1:35,000 births. It is primarily a birth injury, but it is also seen in cases of battered child syndrome. Because of its rare occurrence and the diagnostic difficulties, the lesion may be overlooked or misdiagnosed on initial presentation. The diagnosis is made on the basis of a high index of suspicion, clinical signs and symptoms and awareness of radiological changes. Traumatic humeral dislocation will produce the same radiological picture as TDHE and is consequently a common, incorrect initi

Methods of treatment for mild juvenile osteochondritis dissecans (OCD) of the capitellum include activity modification and periods of immobilization, followed by therapy and range of motion exercises. Surgical treatment may be indicated if there are persistent symptoms, a loose body, the child is approaching skeletal maturity, or if magnetic resonance imaging reveals a fracture through the articular cartilage. We describe a technique of arthrotomy, using an arthroscope as a visual aid, for the treatment of large, full-thickness OCD lesions of the capitellum in young patients. The arthroscopic

This article describes the use of ultrasound (US) in the evaluation of the elbow. US is able to visualize several abnormalities affecting tendons, muscles, ligaments and bursae around the elbow joint as well as to delineate the nature of soft-tissue swelling, such as a space-occupying lesion or synovial enlargement. Occult fractures, osteophytes and intra-articular loose bodies can be depicted with this technique as well. At the cubital tunnel, US allows accurate imaging of the ulnar nerve and to document changes that occur in compressive syndromes. Over other imaging modalities, US offers sev

Open arthrotomy has been the standard of treatment for posttraumatic flexion contractures of the elbow. Utilizing standard arthroscopic techniques combined with electrocautery, anterior capsulotomy was performed without complication. Immediate improvement in extension was demonstrated. Multiple cadaveric dissections evidenced the safety of the approach as the humeral capsular attachment is isolated from the neurovascular structures by the brachialis muscle. This technique serves to underscore the utility of elbow arthroscopy in patients with dysfunction previously only amenable to open joint p

Separation of the distal humeral epiphysis in the neonate or young child can be diagnosed confidently by combining roentgenographic and clinical information. Such diagnosis is critical, as two abnormalities included in the differential diagnosis--irreducible elbow dislocation and fracture to the lateral humeral condyle--often require open reduction, unlike distal humeral epiphyseal separation, which can be treated conservatively. We describe two patients and review the pertinent literature.

OBJECTIVE: To explore the effect of arthroscopy in diagnosis and treatment of osteochondritis dissecans of elbow joint in adolescent.
METHODS: From May 2003 to February 2006, 11 patients with osteochondritis dissecans of elbow joint were diagnosed and treated with arthroscopy. There were 6 males and 5 females, aging from 13 to 19 years. The left joints were involved in 4 cases and right joints in 7 cases. Seven patients had obvious history of injury. The interval of injury and operation was from 7 months to 12 years. The score of VAS was 80 +/- 10, the range of flexion and extend of joint 80

BACKGROUND: Because the population of female baseball players has been increasing worldwide in recent years, it is important for female players to reduce the risk of elbow joint injuries, which occur frequently in baseball pitching. Although many biomechanical studies related to baseball pitching have clarified elbow joint loads, the participants in most of these studies were male baseball players, necessitating the expansion of investigations to female baseball players, particularly young female players.
PURPOSE: The objective of the present study was to elucidate the pitching mechanics rela

BACKGROUND: The earliest distal humeral hemiarthroplasty (or elbow hemiarthroplasty [EHA]) implants, which date back to the late 1940s, were performed in patients with severe elbow joint injuries as an alternative to arthrodesis. After some clinical reports and case studies with a short follow-up, published in the 1990s, a new "anatomically convertible" EHA model was introduced in 2005 and became a common surgical option to treat complex elbow fractures and their sequelae. We describe the mid- and long-term outcomes of EHA performed to treat acute intra-articular fractures or their sequelae.

Twenty fresh cadaveric elbows were used to evaluate the proximity of neurovascular structures to the six arthroscopic portals of the elbow at different positions. After distention of the joint, 4-mm Steinmann pins were introduced into the elbow from the portal's entry points. After surgical dissection, the proximity of the neurovascular structures to the pins was measured in 5 different positions. The radial nerve showed significant proximity to the anterolateral portal in full elbow flexion, full elbow extension, and forearm supination with 10%, 20%, and 10% nerve-pin contacts, respectively.

Of 23 patients with an elbow dislocation associated with at least one displaced fracture of the radial head, 19 patients had the radial head extirpated two days range, 0-16 days) after injury. In four elbows, redislocation occurred. All four redislocations were associated with a displaced fracture of the coronoid process. A follow-up examination was performed in 19 patients between three and 34 years after the injury. Severe osteoarthritis with reduced joint space developed in 12 elbows. Reduced range of motion was the most common complaint, and reduced extension was the most common finding. T

We reviewed the results of 10 patients (10 elbows) who underwent surgical reconstruction for clinically symptomatic posterolateral rotatory instability of the elbow. The symptoms resulted from previous dislocation or a hyperextension or varus stress injury of the elbow. Two elbows had underlying preexisting varus deformity. Surgical reconstruction was performed with a tendon graft in 6 elbows and reattachment of the lateral ulnar collateral ligament to the humerus in 4. Postoperatively, no patient had residual instability or a positive pivot shift test in the elbow. Results were graded as exce

BACKGROUND: Push-ups are performed ubiquitously as a means of strengthening/training and evaluating the upper extremity. However, push-ups performed on the hands with the wrist extended can cause wrist, elbow and shoulder injuries, which may result in disability. Conversely, individuals with a prior wrist or elbow injury are unable to perform push-ups. Our purpose was to evaluate movement patterns in two push-up styles (neutral and extended wrist) at the elbow and shoulder joints. We hypothesized that force-transfer will differ between the two styles, allowing the adjustment of training protoc

The Monteggia injury, a combination of an ulnar fracture and proximal radial dislocation, is an uncommon but high-stakes paediatric injury that is frequently missed at initial presentation. Early recognition is crucial, as delayed diagnosis is associated with malunion, instability, restrictions in rotation, and deformity. The aim of this review is to highlight key concepts relevant to exams and clinical practice, including the epidemiology, anatomy, classification, and evidence-based management of both acute and neglected Monteggia injuries.
