Viable pregnancies in women with different mullerian duct anomalies.

Some of the fascinating developmental anomalies encountered by Obstetricians and Gynecologists are mullerian duct disorder of various types. The diagnosis is usually made during workup for infertility, recurrent pregnancy loss or cyclic pelvic pain. Because of embryologic developmental structural share and proximity, it is not uncommon to find mullerian duct, urinary tract and skeletal anomalies concurrently. Three parous women with different mullerian duct malformations preoperatively diagnosed as ovarian tumor, ectopic pregnancy and footling breech are reported Intraoperatively, the first ca

[A surgically treated case of partial anomalous pulmonary venous connection to the right atrium, diagnosed by magnetic resonance imaging].

We report a successful surgical repair of a 33-year-old male with partial anomalous pulmonary venous connection (PAPVC) to the right atrium and pulmonary valve stenosis. This case was diagnosed using Magnetic Resonance Imaging (MRI). The findings of MRI clearly showed an abnormal development of the atrial septum far to the left, and the right lower pulmonary vein connected to the right atrium. An embryologic explanation of PAPVC can be provided in many cases by persistent communication between pulmonary veins and systemic veins. However, the MRI findings of this case support the suggestion of

Thyroid ima artery (the artery of Neubauer) - how much do we know?

The blood supply of the thyroid gland has been the subject of numerous original studies, case reports and meta-analyses. The number of surgical procedures carried out on the thyroid gland has significantly increased over the last few decades. The cadaveric report discusses the case of a thyroidea ima artery (TIA) which originated from the brachiocephalic artery before its terminal subdivision, giving off numerous branches to the infrahyoid muscles, trachea, and thyroid gland. Based on the current literature, we discuss the prevalence of TIA, its embryology, and possible clinical aspects of thi…

Multimodality imaging features of Canal-of-Nuck hydrocele in adults.

Canal of Nuck abnormalities are common in pediatric population but may rarely present in adults. Hydrocele of the Canal of Nuck is the most common presentation in adult population. These may be symptomatic or found incidentally on imaging. We review the embryology and present imaging features of Canal of Nuck hydroceles in this case-based review article, and highlight the features on various imaging modalities, such as ultrasound, CT (Computed Tomography), MRI (Magnetic Resonance Imaging) and PET (Positron Emission Tomography). Radiologists should be aware of these findings in their practice a…

An unexpected cyst of the ear lobule.

Branchial cleft anomalies (BCA) are among the most common congenital anomalies found in the pediatric head and neck. The embryology of these congenital anomalies is well understood, which allows clinicians to anticipate their diagnosis when a pediatric patient presents with a head or neck mass. The predictable anatomy of the various types of BCA allows for improved surgical planning to prevent recurrence and ensure complete resection. This report details an unusual location of a first BCA located in the ear lobule of a 10-month old male. There has been no documented first BCA at the ear lobule…

A Review of the Genetics and Pathogenesis of Syndactyly in Humans and Experimental Animals: A 3-Step Pathway of Pathogenesis.

Embryology of normal web space creation and the genetics of syndactyly in humans and experimental animals are well described in the literature. In this review, the author offers a 3-step pathway of pathogenesis for syndactyly. The first step is initiated either by the overactivation of the WNT canonical pathway or the suppression of the Bone Morphogenetic Protein (BMP) canonical pathway. This leads to an overexpression of Fibroblast Growth Factor 8 (FGF8). The final step is the suppression of retinoic acid in the interdigital mesenchyme leading to suppression of both apoptosis and extracellula…

Normal neuroanatomical variants that may be misinterpreted as disease entities.

Variations of normal development and benign incidental anomalies are frequently observed on diagnostic neuroimaging. It is important these are recognised for what they are, as misinterpretation may result in unnecessary further investigation, follow-up imaging and anxiety. In this article, we review benign intracranial anomalies commonly referred to our unit for specialist neuroradiology advice or multidisciplinary discussion, concerning cysts of the pineal gland and pituitary fossa, vascular anomalies, and perivascular spaces. This article outlines the embryology and development, the various …

Neurosurgical management and pathology of lumbosacral lipomas with tethered cord.

Lumbosacral lipomas are the most common form of occult spinal dysraphism. The development of lumbosacral lipomas is from the premature disjunction of the neural tube from the surrounding ectoderm, leaving the neural plate open posteriorly and allowing for the infiltration of mesodermal tissue, including fatty tissue. Since lumbosacral lipomas are a common cause of spinal cord tethering that can lead to progressive neurological deficits, prophylactic neurosurgery for lumbosacral lipomas, including untethering of the spinal cord, is recommended. We briefly review the embryology, classification, …

MR Imaging of Müllerian Fusion Anomalies.

Müllerian duct anomalies, also called congenital uterine anomalies, are developmental structural disorders of the female genital tract. These anomalies are clinically relevant in patients with a history of infertility and pregnancy-related complications. The American Society for Reproductive Medicine classification system is the most well known, although newer systems, such as from the European Society of Human Reproduction and Embryology/European Society for Gynaecological Endoscopy, are becoming more widely accepted. MR imaging remains the optimal imaging modality due to its superior multipl…

Intrapancreatic accessory spleen: An eluding diagnosis.

Intrapancreatic accessory spleen (IPAS) is a benign anomaly of splenic embryology and a rare cause of pancreatic pseudotumour. Here, we report a case of a 70-year-old Malay lady whose IPAS was discovered incidentally during her surveillance computed tomography for her underlying left lower lung fibrosis. Radiologically, the lesion mimicked a neuroendocrine pancreatic tumour and was only diagnosed pathologically as IPAS after surgery. In conclusion, recognising IPAS as a differential for enhancing pancreatic mass allows us to exhaust all non-invasive diagnostic means to diagnose this benign les…

Radiographic assessment of congenital malformations of the upper extremity.

Congenital and developmental malformations of the upper extremity are uncommon and their diagnosis can challenge radiologists. Many complex classification systems exist, the latest of which accounts for the complex embryology and pathogenetic mechanisms that govern the formation of these anomalies. Using appropriate descriptors allows for more specific diagnosis and improved consultation with referring pediatricians and surgeons, helping to guide medical and surgical interventions and, if indicated, further investigation for associated abnormalities and underlying syndromes. We review the imag…

Macrostomia: A Practical Guide for Plastic and Reconstructive Surgeons.

Macrostomia is a rare and debilitating congenital anomaly with incompletely understood etiopathogenesis. Despite the phenotypic variability in macrostomia, plastic surgeons should demonstrate competence in the diagnosis and management of this condition. The anatomy, embryology, classification, and clinical presentation of macrostomia are reviewed in this manuscript. A historical overview of surgical repair is presented that forms the basis for understanding modern techniques of repair. Finally, an effective method of macrostomia repair is presented along with review of 5-year results. It is ou…

Nonobliteration of the Processus Vaginalis. Sonography of Related Abnormalities in Children.

The objective of this pictorial essay is to systematically classify processus vaginalis– related disorders in the light of embryology and present illustrative sonograms with corresponding diagrams. Failure of the processus vaginalis to obliterate during gestation results in a wide spectrum of anomalies, including communicating and noncommunicating hydroceles and inguinal and inguinoscrotal hernias, along with other related disorders of the genital system. There are varying classifications in the literature regarding the aforementioned entities. Proper and timely diagnosis of these entities is …

Anatomical variants and pathologies of the vermix.

The appendix may demonstrate a perplexing range of normal and abnormal appearances on imaging exams. Familiarity with the anatomy and anatomical variants of the appendix is helpful in identifying the appendix on ultrasound, computed tomography, and magnetic resonance imaging. Knowledge of the variety of pathologies afflicting the appendix and of the spectrum of imaging findings may be particularly useful to the emergency radiologist for accurate diagnosis and appropriate guidance regarding clinical and surgical management. In this pictorial essay, we review appendiceal embryology, anatomical v…

Oesophageal atresia and tracheo-oesophageal fistula.

Oesophageal atresia with tracheo-oesophageal fistula is a relatively common congenital anomaly occurring in around 1:2500 births. The aetiology and embryology of the condition remain unclear, whilst associations with other significant anomalies are common. Studies in rodent models are contributing to our understanding of the condition. Advances in surgical care and neonatal management have improved survival considerably to around 90%. Long-gap and isolated oesophageal atresia present significant management challenges. Post-operative and long-term complications including oesophageal stricture, …

Congenital cervical cysts, sinuses, and fistulae in pediatric surgery.

Congenital cervical anomalies are essential to consider in the clinical assessment of head and neck masses in children and adults. These lesions can present as palpable cystic masses, infected masses, draining sinuses, or fistulae. Thyroglossal duct cysts are most common, followed by branchial cleft anomalies and dermoid cysts. Other lesions reviewed include median ectopic thyroid, cervical teratomas, and midline cervical clefts. Appropriate diagnosis and management of these lesions requires a thorough understanding of their embryology and anatomy. Correct diagnosis, resolution of infectious i…
