Imaging of vestibular function and disorders and its clinical relevance.

PURPOSE OF REVIEW: To discuss recent advances in imaging of the structural organization and functional connectivity of central vestibular di … hways via the corpus callosum. About 30% of patients with vestibular migraine have a mild bilateral endolymphatic hydrops of the inner ear, especially in the vestibulum rather than the cochlea. Classical vestibular paroxysmia is caused by neurovascular cross-compression, which is most reliably detected by vestibular nerve angulation in DTI-MRI. SUMMARY: The general me …

Association Between Vestibular Aqueduct Morphology and Meniere's Disease.

OBJECTIVE: To investigate the relationship between vestibular aqueduct (VA) morphology and Meniere's disease (MD) using ultrahigh-resolution … ol groups were analyzed. The correlations between VA morphology and the degrees of hearing loss and endolymphatic hydrops (EH) were also analyzed. RESULTS: VA was classified as grades 1-3 in 11, 17, and 12 ears in the MD group and 5, 26, and 37 ears in the control group, respectively. The patency differed significantly between the groups (p < 0.01). The width of the o …

Current diagnostic procedures for diagnosing vertigo and dizziness.

Vertigo is a multisensory syndrome that otolaryngologists are confronted with every day. With regard to the complex functions of the sense o … as for example intratympanic gadolinium application for Menière's disease with visualization of an endolymphatic hydrops also influence current medical standards. Recent methodical developments significantly contributed to the possibilities that nowadays vertigo can be better and more quickly clarified in particular in otolaryngology.

Gadolinium distribution in cochlear perilymph: differences between intratympanic and intravenous gadolinium injection.

INTRODUCTION: Three-dimensional fluid-attenuated inversion recovery (3D-FLAIR) imaging 24 h after intratympanic gadolinium injection (IT method) or 4 h after intravenous injection (IV method) has been used to visualize endolymphatic hydrops in Ménière's disease. The aims of this study were to evaluate the difference in gadolinium distribution in cochlear perilymph between the two methods by comparing the enhancement of the basal and apical turns and clarify the pharmacokinetics in cochlear perilymph. METHODS: A total of 24 ears of 22 patients who underwent the IT method (gadolinium-diethylene…

The peaked audiometric configuration in Meniere's disease: disease related?

A retrospective analysis of audiograms was completed for persons from 3 different patient groups. These 3 groups were (1) persons with unila … d with Meniere's disease, as suggested by research results from animals with experimentally induced endolymphatic hydrops. Peaked audiometric configurations were also observed in roughly 9% of the general clinical population and in 12.5% of ears of persons with acoustic tumors. SP/AP area ratio in the diagnosis of Ménière's disease.

Conventional analysis of the click-evoked electrocochleogram includes measurement of the amplitudes of the cochlear summating potential (SP) and auditory nerve action potential (AP) to derive the SP/AP amplitude ratio. It is now well documented that an amplitude ratio where SP is elevated relative to AP may be a positive finding for endolymphatic hydrops in patients suspected of having Ménière's disease (MD). However, the incidence of an elevated SP/AP amplitude ratio in the general MD population is only approximately 60%. This relatively low value necessitates efforts to improve the diagnosti…

[Menière's disease and isosorbide as an oral hyperosmotic agent (author's transl)].

According to a nationwide survey in 1977 and average of 54.7% of Menière's disease patients were unable to function normally in their daily lives. These patients mostly complained of repeated attacks of vertigo (male 40.3%, female 29.2%) (Watanabe et al. 1980). For the purpose of improving the repeated attacks of vertigo, Isosorbide as an oral hyperosmotic diuretic was first administered orally to 21 patients with Menière's disease from November 1979 to January 1981 for reducing the endolymphatic hydrops that can be presumed to be of pathogenetic importance. The electrocochleographic examinati…

Different forms of dizziness occurring after cochlear implant.

Dizziness after cochlear implant (CI) was studied in a series of 94 consecutive adult patients receiving a cochlear implant, 46 (49.0%) of whom experienced dizziness post-operatively. In 29 patients, post-operative dizziness occurred soon after surgery and subsided within one month. Dizziness of the continuous type, lasting more than 6 months, was a complaint in only two patients. In addition to these already known forms of dizziness, spells of vertigo occurring later than one month after cochlear implant were experienced by 15 patients (delayed-V). The spells of delayed-V occurred suddenly an

Inflammatory and immune pathways in Ménière's disease: a multi-database bibliometric and visualized analysis of research trends.

BACKGROUND: Ménière's disease (MD) is a chronic inner ear disorder characterized by recurrent episodes of vertigo, fluctuating sensorineural … gy, and vestibular medicine. Research hotspots have progressively shifted from clinical phenotypes, endolymphatic hydrops, and local therapeutic interventions toward immune-inflammatory mechanisms, encompassing cytokine networks, innate immune signaling, allergy-related pathways, and imaging-based phenotypes. Among these, the IL-1β/TNF-α axis, the NF-κB/TLR signaling p …

Comparison of radiological abnormalities between the jugular bulb and the vestibular aqueduct in patients with Ménière's disease.

OBJECTIVE: Anatomical variations of the inner ear may contribute to the development of Ménière's disease (MD), which is a complex inner ear disorder histopathologically characterized by idiopathic endolymphatic hydrops (ELH). Abnormalities of the vestibular aqueduct (VA) and the jugular bulb (JB) have been suggested as predisposing factors. Yet, few studies have investigated the correlation between JB abnormalities and VA variations as well as its clinical relevance in these patients. In this retrospective study, we investigated the differences in the incidence of radiological abnormalities of…

MATERIALS AND METHODS: Adults (>16 yr old) with SNHL were recruited at the Otolaryngology Department at Marqués de Valdecilla University Hospital (Spain). Asymmetric caloric function in the presence of normal horizontal head impulse tests is most commonly associated with Meniere's disease and may function as a diagnostic marker.

Vestibular sensory functional status of cochlear implanted ears versus non-implanted ears in bilateral profound deaf adults.

Patients with hearing loss who underwent cochlear implantation can present symptomatic or asymptomatic vestibular damages earlier or later after the surgery. The vestibular permanent lesions could be acute, produced by surgical trauma or could be progressive due to local morphological changes made by the presence of the portelectrode in the inner ear (fibrosis related, ossification, basilar membrane distortion, endolymphatic hydrops). Besides histopathological findings in inner ear of cochlear implanted patients, the vestibular permanent damages could be found by assessment of clinical vestibu…
