Advanced imaging techniques and artificial intelligence in pleural diseases: a narrative review.

BACKGROUND: Pleural diseases represent a significant healthcare burden, affecting over 350 000 patients annually in the US alone and requiring accurate diagnostic approaches for optimal management. Traditional imaging techniques have limitations in differentiating various pleural disorders and invasive procedures are usually required for definitive diagnosis. METHODS: We conducted a nonsystematic, narrative literature review aimed at describing the latest advances in imaging techniques and artificial intelligence (AI) applications in pleural diseases. RESULTS: Novel ultrasound-based techniqu

Methimazole-Induced Pleural Effusion in the Setting of Graves' Disease.

Methimazole is a thionamide drug that inhibits the synthesis of thyroid hormones by blocking the oxidation of iodine in the thyroid gland. We report a case of methimazole-induced recurrent pleural effusion. A 67-year-old female with recently diagnosed Graves' disease on methimazole 20mg daily was admitted with dyspnea and new onset atrial fibrillation with rapid ventricular rate. Chest X-ray revealed a unilateral right pleural effusion, which was consistent with a transudate on thoracocentesis. She was managed as a case of congestive heart failure and methimazole dose was increased to 30 mg da

Gastrointestinal and hepatic manifestations of sarcoidosis.

Sarcoidosis is a multisystem disease characterized by noncaseating granulomas in the affected organs, including skin, heart, nervous system, and joints. Diagnosis of sarcoidosis is generally based upon a compatible history, demonstration of granulomas in at least two different organs, negative staining and culture for acid fast bacilli, absence of occupational or domestic exposure to toxins, and lack of drug-induced disease. Involvement of the hollow organs is rare. Rather than being due to sarcoidosis, some reported mucosal lesions may simply have incidental granulomas. Extrinsic compression

Acute stress effects on local Il-1beta responses to pathogens in a human in vivo model.

The combined effects of stress and antigen on interleukin-1beta (Il-1beta) have rarely been studied locally at the site of microbial challenges in vivo, so far. We here propose a model for the analysis of such effects in humans and examine its utility for acute stress trials. Twelve students (6 male, 6 female) refrained from oral hygiene in two antagonistic quadrants for 28 days to allow for increasing bacterial stimulation of the respective gingival sites due to accumulation of microbial plaque. Good oral hygiene was maintained in the remaining quadrants. At day 27 and 28 students were subjec

Aetiology and management of acute cardiac tamponade.

OBJECTIVE: To review current concepts in the aetiology and management of patients with cardiac tamponade. DATA SOURCES: A review of articles reported on acute cardiac tamponade. SUMMARY OF REVIEW: Cardiac tamponade is defined as a haemodynamically significant cardiac compression caused by pericardial fluid. The fluid may be blood, pus, effusion (transudate or exudate) or air and treatment involves correction of the cardiac diastolic restriction by removing pericardial fluid during either pericardiocentesis or thoracotomy. Pericardiocentesis is usually performed for urgent management of an ac

Vascular endothelial growth factor (VEGF) in inflammatory and malignant pleural effusions.

BACKGROUND: Investigation and management of pleural effusions is an important clinical problem yet the pathogenesis of pleural fluid accumulation is poorly understood. Vascular endothelial growth factor (VEGF) is a potent inducer of capillary permeability that is produced by both malignant and inflammatory cells. A study was undertaken to determine whether VEGF has a potential pathogenic role in the development of pleural effusions and whether VEGF receptors are present on human pleural mesothelial cells. METHODS: Normal and inflamed pleura were examined immunohistochemically for the presence

Culture positivity of anterior chamber exudate in cases of fungal keratitis.

PURPOSE: To describe the nature of anterior chamber (AC) exudates in cases of fungal keratitis, and to determine whether these are inflammatory or infective. METHODOLOGY: Twenty-six patients with culture-proven fungal keratitis and anterior chamber exudate, worsening due to medical management and who underwent therapeutic penetrating keratoplasty (TPK), were included in the study. Patients with mixed infections, prior surgical intervention, perforation, or absence of hypopyon/AC exudates were excluded from the study. AC exudates were plated on one/two solid culture media during surgery after

Association of Endotracheal Aspirate Culture Variability and Antibiotic Use in Mechanically Ventilated Pediatric Patients.

IMPORTANCE: Endotracheal aspirate cultures are commonly collected from patients with mechanical ventilation to evaluate for ventilator-associated pneumonia or tracheitis. However, the respiratory tract is not sterile, making differentiating between colonization from bacterial infection challenging, and results may be unreliable owing to variable specimen quality and sample processing across laboratories. Despite these limitations, clinicians routinely interpret bacterial growth in endotracheal aspirate cultures as evidence of infection, sometimes regardless of organism significance, prompting

Micropore Particle Technology Promotes Wound Healing, Whereas Polyhexamethylene Biguanide Causes Tissue Degeneration: A Case Report.

CASE REPORT: A 72-year-old woman with a nontraumatic spinal cord injury developed eschar on her lower right back. An underlying abscess was identified, which upon surgical debridement left a large wound extending down to the hip bone. In addition, the hip suffered from chronic osteomyelitis and was exposed at the bottom of the wound. The wound was initially treated for 5 weeks with Manuka honey but deteriorated further. Next, micropore particle technology (MPPT) was used. It cleared the wound of necrotic tissue based on autolytic debridement and removed the soft tissue infection; over a 3-mont

Novel mediators and mechanisms in the resolution of infectious inflammation: evidence for vagus regulation.

Excessive chronic inflammation is linked to many diseases and considered a stress factor in humans (Robbins Pathologic Basis of Disease. Philadelphia: W.B. Saunders Co., 1999, Proc Natl Acad Sci USA, 2008, 105: 17949, Immunity, 44, 2016, 44: 463, N Engl J Med, 2011, 364: 656). Today, the resolution of inflammation is widely recognized as a cellular biochemically active process involving biosynthesis of a novel superfamily of endogenous chemical signals coined specialized pro-resolving mediators (SPMs; Nature, 2014, 510:92). Herein, we review recent evidence, indicating a role for the vagus ner

Pleural fluid carcinoembryonic antigen as a biomarker for the discrimination of tumor-related pleural effusion.

OBJECTIVES: To assess the value of tumor biomarkers in the determination of tumor-related pleural effusion (PE). Cytological examination of the pleural fluid has a limited sensitivity in diagnosing tumor PE. METHODS: Pleural fluid and serum samples were obtained from 1137 patients and PE etiology was determined by multiple diagnostic techniques. Carcinoembryonic antigen (CEA) levels were measured in both pleural fluid and serum samples. Levels of cancer antigen (CA) 125, glucose, and lactate dehydrogenase (LDH) were assessed in pleural fluid specimens. RESULTS: 601 PE patients were diagnosed

Bacterial and Respiratory Viral Interactions in the Etiology of Acute Otitis Media in HIV-infected and HIV-uninfected South African Children.

BACKGROUND: Bacteria and respiratory viruses are implicated in the pathogenesis of acute otitis media (AOM); however, data from low-middle income countries are sparse. We investigated the etiology of AOM in HIV-infected (HIV+), HIV-uninfected (HIV-) and HIV-exposed clinically asymptomatic for HIV-infection (HEU) South African children. METHODS: Children ≥3 months to <5 years of age with AOM were enrolled between May 2009 and April 2010 (NCT01031082). Middle ear fluid samples were cultured for bacteria; antibacterial susceptibility was done and serotyping undertaken for Streptococcus pneumonia

Diagnostic accuracy and safety of semirigid thoracoscopy in exudative pleural effusions in Denmark.

BACKGROUND: To assess the diagnostic accuracy and the safety of medical thoracoscopy (MT) performed with the semirigid thoracoscope. METHODS: We retrospectively evaluated patients who underwent MT with semirigid thoracoscope under local anesthesia for unexplained exudative pleural effusion from March 1, 2009 to September 1, 2013 in Denmark. RESULTS: Sixty-nine patients were retrospectively studied. In 13 patients it was not possible to perform the scheduled MT, in 9 cases due to an insufficient pneumothorax, in 3 due to an insufficient pleural effusion, and in 1 due to a purulent pleuritis.

Assessing joint effusion and bone changes of the head of the mandible in MR images of symptomatic patients.

The aim of the present study was to investigate the relationship between degenerative bone changes of the head of the mandible and the presence of joint effusion (JE). This study was based on sagittal magnetic resonance imaging (MRI) reports of 148 temporomandibular joints (TMJs) of 74 patients complaining of pain and/or dysfunction in the TMJ area. The mandible heads were surveyed for osteoarthritis characteristics, which were classified as osteophytosis, sclerosis or erosion. The presence of JE was checked whenever high signal intensity was observed in the articular space. The results eviden

Subcarinal collection following mediastinoscopy: a normal post-procedural CT finding.

AIM: To determine the frequency with which a subcarinal collection is present at computed tomography (CT) following mediastinoscopy and to determine the CT features of the collection. MATERIALS AND METHODS: All patients who underwent uncomplicated mediastinoscopy during a 1-year period were retrospectively identified. This list was cross-referenced to determine those patients who also underwent CT within 15 days after the procedure. Each post-mediastinoscopy CT examination was assessed in consensus by three fellowship-trained thoracic radiologists for the presence of subcarinal abnormalities,
