BACKGROUND: Proximal humerus fractures (PHFs) and fracture-dislocations (PHFDs) are frequently encountered in the emergency department (ED). Adequate pain control for these injuries can be challenging, and traditional methods, including opioid analgesics and other blocks may pose significant risks such as medication side-effects and hemidiaphragmatic paresis. The interfascial plane block (IPB) is an emerging, ultrasound-guided technique that targets the fascial plane between the deltoid and subscapularis muscles, potentially offering safer and more focused analgesia for acute shoulder injuries

If left untreated, varus posteromedial rotatory injuries of the elbow result in poor functional outcomes. Surgical treatment allows restoration of elbow kinematics, minimizing the chances of chronic varus instability and early onset osteoarthritis. However, large exposures are associated with extensive soft tissue stripping, a high risk of infection, nerve injury, poor visualization of the articular surface and longer recovery. Consequently, there has been renewed interest in the use of elbow arthroscopy to circumvent these problems. Arthroscopic treatment offers the potential advantage of a s

We aim to describe an approach for reducing the posteriorly dislocated humeral head through the rotator interval via a deltopectoral approach that is frequently utilized for internal fixation of proximal humerus fractures and fracture dislocations. The sheath of the long head of biceps (LHB) and the rotator interval capsule are opened till the glenoid; this enables access to the glenohumeral joint via the rotator interval. A long-handle Cobb elevator is introduced through the rotator interval and, under intraoperative imaging, advanced posteromedially to the dislocated humeral head. After ensu…

PURPOSE: Ankle fractures represent about 10% of all adult fractures, with increasing incidence. Dislocated ankle fractures often require delayed open reduction and internal fixation due to swelling, necessitating temporary stabilization using a cast or an external fixator. This study aims to assess risk factors for insufficient preliminary reduction immobilized by a cast, focusing on medial clearspace and posterior malleolus fragment size, to identify fractures that would benefit from initial stabilization with an external fixator.
METHODS: Patients treated for dislocated ankle fractures at o

This study assessed the effect of the time interval from initial injury to surgery and the presence of associated fracture on functional outcomes after acute posterior elbow fracture dislocation. Twenty-six pediatric patients were evaluated with respect to operation time point (within 24 h vs. later) and associated fracture retrospectively. The Mayo Elbow Performance Index (MEPI) score was used to assess functional results. The MEPI score was 91 (80-100) in patients with one associated fracture and 83 (75-95) (P=0.02) in patients with more than one associated fracture. The MEPI score in patien…

BACKGROUND: Pronation external rotation (PER) ankle fractures are relatively uncommon but serious ankle injuries. Although recent studies have demonstrated good outcomes of PER IV fractures after operative treatment, the effect of dislocation on functional outcomes has not yet been evaluated. The objective of this study was to compare short-term functional outcomes in PER IV ankle fractures with and without dislocation.
METHODS: Our database of ankle fractures surgically treated using an anatomic fixation approach by the senior author from 2003 to 2013 was reviewed. All PER IV ankle fracture

INTRODUCTION: Only a high-energy force can cause thoracic spinal fracture-dislocation injuries, and such injuries should always be suspected in patients with polytrauma. The injury is usually accompanied by neurological symptoms. There are only a few cases of severe thoracic spinal fracture-dislocation without neurological symptoms in the literature, and until now, no case of severe thoracic spinal fracture-dislocation without neurological symptoms and without costal fractures has been reported.

PURPOSE: There is no consensus as to the best treatment of Mason type II fractures without concomitant elbow fractures or dislocation. The aim of this systematic review was to compare the results of operative and nonoperative treatment of these injuries.
METHODS: We systematically screened the databases of PubMed, EMBASE, and Cochrane Library until September 2011 for studies on nonoperative or operative treatment of Mason type II fractures. We defined successful treatment as an excellent or good result according to the Broberg and Morrey score, Mayo Elbow Performance Score, or Radin score. Ex

Fracture complications of convulsions are reported occasionally, e.g.: mono- or bilateral posterior shoulder luxations and luxation fractures, central dislocations of the hip as well as fractures of the femoral neck or compression fractures of vertebrae. A case is reported where the patient sustained four such lesions simultaneously. The epileptic seizure occurred following the sudden interruption of a neurotropic therapy with Carbamazepin (Tegretal). A general osteodystrophy was not found. The simultaneous presence of four such lesions following one convulsion has never before been reported a

Background and Objectives: Anterior sacroiliac fracture dislocation (ASFD), also known as locked pelvis, is a rarely reported diagnosis. The types of ASFDs are often misdiagnosed as lateral compression fractures due to the presence of crescent fractures. In this study, we distinguished ASFD from lateral compression fractures (LC 2) and studied their characteristics. Materials and Methods: This is a retrospective study involving patients from a Level 1 trauma center. Fifty-nine patients under the age of 65 years with crescent fractures caused by a high-energy mechanism were investigated. Result…

BACKGROUND: Dorsal fracture-dislocations of the proximal interphalangeal (PIP) joint can cause chronic pain, stiffness, and loss of motion. We present a minimally invasive method-extension block pinning combined with percutaneous intramedullary reduction and fixation-for unstable dorsal PIP fracture-dislocations and evaluate its radiologic and functional outcomes.
METHODS: Twenty consecutive patients with acute unstable dorsal PIP joint fracture-dislocations and persistent post-reduction subluxation were treated with this technique. In the treated cohort, volar articular involvement ranged fr

INTRODUCTION: Total hip arthroplasty (THA) is widely used for active, elderly patients with femoral neck fractures (FNF). Compared to THA for osteoarthritis, THA for FNF is associated with a higher incidence of dislocation and reoperation. Robotic assistance may improve component positioning and leg-length restoration in THA, but its use in FNF has not been described. The objective of this study was to assess the feasibility and perioperative outcomes of robotic-assisted THA (rTHA) for FNF.
METHODS: A retrospective review identified 93 patients undergoing 94 THAs for FNF from 2016 to 2023. 18

An 11-year-old girl sustained an irreducible anterior radial head dislocation associated with a Salter-Harris type II proximal radial physeal injury and a volar annular ligament avulsion fracture after falling on an outstretched hand. Closed reduction was unsuccessful. During open reduction, the annular ligament was found interposed within the radiocapitellar joint, avulsed with a small bony fragment from the volar aspect of the radial notch. After ligament release, the radial head remained unstable, requiring anatomical repair using a pullout suture technique. At 1-year follow-up, the patient

CASE: A 13-year-old adolescent girl presented with right hip pain and restricted motion, 3 days following closed reduction of post-traumatic hip dislocation. Initial radiograph looked innocuous. A computed tomography scan was performed due to persistent pain, revealed a bony ligamentum teres (LT) avulsion fragment incarcerated within the hip joint. The fragment was excised through a safe surgical dislocation, and an intraoperatively discovered posterior labral tear was repaired. The patient is asymptomatic at 2-year follow-up.
CONCLUSION: This is a rare case of pediatric LT bony avulsion. Cro

OBJECTIVE: To report the outcomes of transoral C2 osteotomy (or partial odontoidectomy) and posterior fixation, regarding efficacy and safety, in patients with severe irreducible atlantoaxial dislocation (IAAD) following odontoid fracture.
METHODS: Transoral C2 osteotomy, soft tissue resection, with or without facet joint release, followed by posterior fixation were performed on 3 patients (2012, 2016, 2023) who were suffering from severe IAAD after an odontoid fracture with spinal cord compression. The radiological and clinical outcomes were then assessed.
RESULTS: All 3 patients had satisf

BACKGROUND: The incidence of traumatic vertebral artery injury (VAI) associated with cervical spine trauma varies widely in published trauma series. The primary aim of this study was to determine the incidence of traumatic VAI in patients who suffered cervical spine injuries by means of routine magnetic resonance imaging, and the secondary objective was to identify any associations with injury mechanism, level of injury, and neurologic injury severity. Materials and methods: A retrospective review was conducted on 96 patients who suffered cervical spine fracture dislocation with or without an…

We present a rare case of open lateral condyle Hoffa's fracture with concurrent patellar intra-articular dislocation, quadriceps rupture and posterior cruciate ligament (PCL) avulsion. The early adolescent male sustained these injuries in a road traffic accident. Diagnostic evaluation and a multidisciplinary approach guided treatment decisions. The patient underwent single-stage open reduction and internal fixation for the lateral condyle Hoffa's fracture and quadriceps tendon repair, while conservative management was chosen for the PCL avulsion. At the 1-year follow-up, the patient exhibited
