[The surgical treatment of fractures of the base of the fifth metacarpal bone].

Fractures of the base of the fifth metacarpal bone are relatively uncommon injuries. In contrast to similar fractures of the first metacarpal bone, various methods of treatment have been advocated. Some believe that cast immobilization or immediate unrestricted motion is a reliable method, while others recommend surgery and pinning or plate fixation. Fractures of the base of the fifth metacarpal bone are inherently unstable and inadequate reduction may result in pain, functional disability and osteoarthritic changes. We retrospectively reviewed 14 cases with intra-articular or extra-articular

30-year retrospective review of frontal sinus fractures: The Charity Hospital experience.

BACKGROUND AND OBJECTIVES: The optimal method of treating frontal sinus fractures has remained undetermined. The purpose of this retrospective review is to compare the mechanism of injury, diagnostic techniques, methods of fracture management, complications, and long-term results in different types of frontal sinus fractures. METHODS AND MATERIALS: Over 4,000 records of patients with frontal bone or skull fractures, treated during a 30-year period in an academic nonprofit tertiary Level I trauma center, were considered. From these, the authors selected 150 patients with frontal sinus fracture

Functional Ankle Reconstruction Technique After Total Calcanectomy.

BACKGROUND: Although comminuted fractures, osteomyelitis, large skin ulcers, and malignant tumors are rarely seen in the calcaneus, it is a problematic region to treat because it is not an actual compartment and has insufficient blood supply. Few foot and ankle surgeons would recommend total calcanectomy in various cases of malignant tumors, comminuted fractures, ulcerations of the heel often seen in diabetic patients, and chronic osteomyelitis. After calcanectomy, if functional reconstruction is not performed, the patient will experience loss of function, pain, wound problems, talonavicular d

Hemiarthroplasty for irreparable distal humeral fractures: medium-term follow-up of 42 patients.

We report our experience of performing an elbow hemiarthroplasty in the treatment of comminuted distal humeral fractures in the elderly patients. A cohort of 42 patients (three men and 39 women, mean age 72; 56 to 84) were reviewed at a mean of 34.3 months (24 to 61) after surgery. Functional outcome was measured with the Mayo Elbow Performance Score (MEPS) and range of movement. The disabilities of the arm, shoulder and hand questionnaire (DASH) was used as a patient rated evaluation. Complications and ulnar nerve function were recorded. Plain radiographs were obtained to assess prosthetic lo

Single-Incision Double-Plating Approach in the Management of Isolated, Closed Osteoporotic Distal Femoral Fractures.

INTRODUCTION: Distal femoral fractures in elderly population had recorded an increase in incidence in the last 2 decades. Lateral distal femoral locking plating is considered one of the best options especially when dealing with comminuted fractures but varus collapse of the medial femoral condyle occurs frequently in patients with osteoporosis. Anatomical reduction of the fracture with stable rigid fixation using double-plating approach allows early mobilization of geriatric population and prevents varus collapse minimizing the comorbidities in such fractures. PATIENTS AND METHODS: Between Se

Watchful Eye Prevents Catastrophe in Case of Malignant Hyperthermia: A Case Report.

CASE: A 49-year-old woman presented with a displaced left posterior wall acetabular fracture-dislocation and a left comminuted intra-articular distal radius fracture sustained in a motor vehicle accident. Fifteen minutes after anesthesia induction for operative fixation of the fractures, the patient experienced severe hypertension, increased end-tidal carbon dioxide, and fever. A diagnosis of malignant hyperthermia (MH) was made before incision. The patient was medically stabilized, and her acetabular fracture-dislocation was placed in skeletal traction until she was deemed fit for surgery 4 d

The Missing Piece: A Subtrochanteric Hip Fracture Presenting with Abdominal Pain: A Case Report.

CASE: A 33-year-old man presented with acute abdominal and left hip pain after being struck by a motor vehicle. Radiographs revealed a comminuted left subtrochanteric femoral fracture. Computed tomography demonstrated a large pelvic hematoma and a radiopaque mass in the abdomen. Laparoscopy revealed the mass to be a large bone fragment lying within the greater omentum. The bone fragment, which seemed to be a femoral fracture fragment, was removed, and the fracture was repaired with a short cephalomedullary nail. CONCLUSION: Rarely, subtrochanteric fracture fragments can penetrate the pelvis o

Open tibia fracture with compartment syndrome.

In a series of 104 open tibia fractures, six patients developed compartment syndromes involving all four compartments of the lower leg, four of which developed after initial debridement and reduction. The presence of an open tibia fracture with a displaced, comminuted, fibula fracture should suggest the possibility of a developing compartment syndrome. Clinical symptoms include: increasing muscle pain which can be exaggerated by stretching; loss of sensation; decrease in muscle strength; and palpably swollen compartments. Double fasciotomy incisions may lead to adequate decompression, but resu

The Ilizarov fixator in trauma: a 10-year experience.

We reviewed the clinical results of 332 fractures treated with the Ilizarov external fixator between 1984 and 1993. The locations of the involved bones were: tibia, 247 (including 28 with tibial pylons); femur, 47; humerus, 21; forearm, 12 and calcaneus, 5. The clinical outcomes of this series were retrospectively evaluated by radiological and clinical rating systems. In the tibial fractures, results in 71.1% were categorized as excellent or good, without deep infection; 63.3% of the tibial pylon fractures, mostly open, also showed excellent or good results. In the femoral fractures, which wer

Concomitant Distal Radius Fracture with Scaphoid Fracture-Dislocation and Carpal Instability: A Case Report.

INTRODUCTION: Concomitant ipsilateral fractures of the distal radius and scaphoid are uncommon and are typically associated with high-energy trauma. The combination of a comminuted intra-articular distal radius fracture with scaphoid fracture-dislocation and carpal instability represents a particularly rare and complex injury pattern. Early recognition of associated carpal injuries is important to restore and maintain anatomical alignment. CASE REPORT: A 32-year-old right-hand-dominant man presented with severe pain, swelling, and deformity of the left wrist following a high-energy motorcycle

[Endoprosthetics of the radial column after trauma].

Non-displaced fractures of the radius head are in most cases treated conservatively. Open reduction and anatomical internal fixation of displaced radius head fractures is the method of choice. In comminuted fractures of the radius head (Mason type III and type IV) replacement with a radius head prosthesis achieves joint stability and prevents secondary complications, such as valgus elbow deformity and proximal radial migration. Modern anatomically formed prostheses show promising results in the medium-term view. Typical complications after radius head replacement are limited range of motion in

Femoral nailing with transverse locking screws through the femoral neck.

We treated 55 patients with subtrochanteric or comminuted femoral fractures with a transverse locking femoral nail (Alta, Howmedica). This nail was positioned more cephalad than usual so that at least one fixation screw was located in the femoral neck or head. Forty-seven fractures (20 subtrochanteric and 27 diaphyseal) were available for follow-up averaging 12 months (3-35). The time to radiographic healing averaged 7 months (3-21). There was one delayed union, one non-union, two superficial infections, one broken proximal screw, one compartment syndrome of the thigh, and one peroneal nerve p

Distal humeral fractures in the adult.

Distal humeral fractures in the adult often are complicated by pseudarthrosis, pain, and limitation of motion. Of 29 adult patients with distal humeral fractures treated in teaching hospitals, 22 obtained acceptable results. Twenty-two percent of those treated by open methods and 36% of those treated by closed methods had unacceptable results. Ninety percent of the patients starting physiotherapy within six weeks of injury recovered well, while over half of those starting physiotherapy after six weeks had unsatisfactory results. Even comminuted fractures healed when treated by internal fixatio

Injury patterns in motorcycle accidents.

A retrospective study of 260 cases of motorcycle accidents which occurred over a 4 1/2 year period is reported. Approximately 8% died of their injuries. The vulnerability of the tibia to open fractures is noted, and the frequent association of these open injuries with severe contamination and devitalization of tissues resulting in a high complication rate. Severely comminuted intraarticular fractures of the distal tibia and radius were numerous, and one is tempted to name the injury the "motorcycle radius." Traumatic amputations were few and all involved the lower extremity. Fractures of the c

Less is more: lag screw only fixation of lateral malleolar fractures.

Displaced fractures of the lateral malleolus are typically treated with plate osteosynthesis with or without the use of lag screws, and immobilisation in a plaster cast for up to 6 weeks. Fixation through a smaller incision with less metal, such as lag screw only fixation, would theoretically lead to decreased infection rates and less irritation caused by hardware. The purpose of this study was to evaluate the benefits and success of lag screw only fixation of the lateral malleolus in non-comminuted oblique fractures of the lateral malleolus. A total of 25 patients who had non-comminuted unsta

Design and Validation of a Smile-Necklace Plate for Treating Inferior Patellar Pole Avulsion Fractures: A Review and Hypothesis.

The patella's inferior pole transmits force generated by contraction of the quadriceps muscle to the tibial tuberosity through the attached patellar ligament, thus completing knee extension. Therefore, fractures of the patella's inferior pole disrupt the coherence of mechanical transmission in the lower extremities. There appears to be no consensus among trauma centers regarding the treatment of infrapatellar pole fractures, primarily because there is no consistent design or application of internal fixation for this type of fracture. We designed a new internal implant similar to the smile neck

A prospective study of 245 open digital fractures of the hand.

We report a prospective study of 245 open digital fractures of the hand in 201 patients. Management followed clear guidelines set out in protocol. The incidence of infection (2.04%) and non-union (2.45%) was low. Proximal phalangeal fractures and comminuted fractures did worst and associated significant soft tissue injuries had a particularly deleterious effect on the outcome. A classification based on the different soft-tissue components involved is proposed. In fractures associated with simple lacerations or isolated digital nerve injury only, one can expect about 40% good results and about

In vitro/ in vivo evaluation of double crosslinked bone glue with different degrees.

Successful bone fragment fixation is a crucial factor in bone fracture healing, the fixation of crushed bone fragments could hinder bone fracture healing. Thus, ideal bone glues to effectively adhere and splice comminuted bone fragments are needed in clinical. Herein, an osteoinductive and biodegradable double cross-linked bone glue (GelMA-oDex-AMBGN) was constructed through Schiff's base reaction between commercial GelMA (with different substitution degrees of amino groups) and Odex mixed with amine-modified mesoporous bioactive glass nanoparticles (AMBGN), followed by crosslink with blue lig
