Diarrheal disease as a cause of malnutrition.

Although long associated with infectious diseases, malnutrition is recognized as a major effect of specific infections, especially those of the gastrointestinal tract. Synergistic exacerbation of infections and nutritional deficiency commonly begin with weaning, where the impact of repeated infections and possible monocyte mediator release may have an even greater effect on malnutrition of young children than that of deficient diets in many areas. Reviewed here are the detailed host alterations seen with specific enteric infections that lead to malnutrition. These include mucosal dysfunction,

Does non-acetylated salicylate inhibit thromboxane biosynthesis in human platelets?

Ingestion of aspirin (acetyl salicylic acid: ASA) may promote bleeding complications due to inhibition of thromboxane biosynthesis, which results in the prolongation of bleeding time. The effect is believed to be achieved by the irreversible acetylation of the enzyme cyclooxygenase by aspirin. This alteration in platelet function by aspirin prohibits its use in patients with bleeding disorders such as haemophiliacs. Choline magnesium trisalicylate (CMT; Napp Laboratories Ltd) is a non-acetylated salicylate with analgesic and anti-inflammatory effects similar to that of aspirin. However, despit

Use of cholestyramine in treatment of children with familial hypercholesterolaemia.

19 children with heterozygous familial hypercholesterolaemia have been treated with cholestyramine administered twice daily in a total dosage of 8 to 24 g/day (0·3 to 1·1 g/kg body weight per day). Serum cholesterol concentration was reduced by a mean of 36% (range 27 to 47%). The therapeutic effect was similar whether or not dietary fat was restricted, and the reduction in serum cholesterol has been maintained for periods of up to 20 months. Side effects of cholestyramine have been confined to the gastrointestinal tract. Absorption of fat was impaired in some patients, but has not been associ

The effects of starter feed on intestinal fungi and non-targeted metabolomics of blood in calf yaks.

Yaks are the primary ruminant on the Qinghai-Xizang Plateau, playing a central role in the livelihoods of local farmers and herders. They serve as a vital source of food, clothing, shelter, and transportation in the region. Over the course of long-term natural selection, yaks have evolved distinct regulatory mechanisms related to their physiology, nutritional metabolism and feeding behaviors. Despite these adaptive traits, traditional yak breeding faces challenges such as slow growth, delayed gastrointestinal maturation, and low weaning weights, which lead to reduced breeding efficiency. This

Characteristics of the gastrointestinal tract and volatile fatty acids in plateau ruminants.

High-altitude ruminants face environmental stressors such as low temperature, hypoxia, and short grazing seasons, which challenge energy acquisition and nutrient absorption. The gastrointestinal tract (GIT) plays a central role in metabolic adaptation. In this study, we collected contents from the rumen, reticulum, omasum, abomasum, duodenum, jejunum, cecum, and rectum of Pamir yaks. Microbial composition and metabolic functions across GIT regions were investigated through volatile fatty acid (VFA) determination and 16S rRNA gene sequencing. The results showed that the total VFA concentrations

Going with the Flow: Sensorimotor Integration Along the Zebrafish GI Tract.

Sensorimotor integration along the gastrointestinal (GI) tract is crucial for normal gut function yet remains poorly understood in the context of neurodevelopmental disorders (NDDs) such as autism spectrum disorder (ASD). The genetic tractability of zebrafish allows investigators to generate molecularly defined models that provide a means of studying the functional circuits of digestion in vivo. Optical transparency during development allows for the use of optogenetics and calcium imaging to elucidate the mechanisms underlying GI-related symptoms associated with ASD. The array of commonly repo

Fatal Case of Progressive Mpox in a Patient with AIDS-Viral Enteropathy and Malabsorption Demanding the Use of Full Parenteral ARV and Endovenous Cidofovir.

We report a fatal case of disseminated mpox infection that progressed over more than three months in an HIV-infected patient with acquired immunodeficiency syndrome (AIDS). Mucocutaneous, pleuropulmonary, central nervous system, and gastrointestinal involvement was documented. This course of disease resembles progressive vaccinia, a formerly reported disease caused by uncontrolled replication of smallpox vaccination orthopoxviruses in immunosuppressed patients. Severe small bowel involvement jeopardized normal oral tecovirimat and antiretroviral therapy absorption. This problem prompted the us

Trafficking Ion Transporters to the Apical Membrane of Polarized Intestinal Enterocytes.

Epithelial cells lining the gastrointestinal tract require distinct apical and basolateral domains to function properly. Trafficking and insertion of enzymes and transporters into the apical brush border of intestinal epithelial cells is essential for effective digestion and absorption of nutrients. Specific critical ion transporters are delivered to the apical brush border to facilitate fluid and electrolyte uptake. Maintenance of these apical transporters requires both targeted delivery and regulated membrane recycling. Examination of altered apical trafficking in patients with Microvillus I

What do I need to know about aminoglycoside antibiotics?

The aminoglycosides are broad-spectrum, bactericidal antibiotics that are commonly prescribed for children, primarily for infections caused by Gram-negative pathogens. The aminoglycosides include gentamicin, amikacin, tobramycin, neomycin, and streptomycin. Gentamicin is the most commonly used antibiotic in UK neonatal units. Aminoglycosides are polar drugs, with poor gastrointestinal absorption, so intravenous or intramuscular administration is needed. They are excreted renally. Aminoglycosides are concentration-dependent antibiotics, meaning that the ratio of the peak concentration to the mi…

Use of rifaximin in gastrointestinal and liver diseases.

Rifaximin is a broad spectrum oral antibiotic with antimicrobial activity against Gram-positive and Gram-negative aerobic and anaerobic bacteria. It is poorly absorbed and thus has a highly favorable safety profile. Rifaximin has been shown to be effective in the treatment of traveler's diarrhea, functional bloating and irritable bowel syndrome, small bowel bacterial overgrowth and in the prevention of recurrent overt hepatic encephalopathy. In addition, there is emerging evidence for a possible beneficial effect of rifaximin in the treatment of uncomplicated diverticular disease and in the pr

pH-sensitive poly(lactide-co-glycolide) nanoparticle composite microcapsules for oral delivery of insulin.

This study proposes a new concept of pH-sensitive poly(lactide-co-glycolide) (PLGA) nanoparticle composite microcapsules for oral delivery of insulin. Firstly, insulin-sodium oleate complex was prepared by the hydrophobic ion pairing method and then encapsulated into PLGA nanoparticles by the emulsion solvent diffusion method. In order to reduce the burst release of insulin from PLGA nanoparticles and deliver insulin to specific gastrointestinal regions, hence to enhance bioavailability of insulin, the PLGA nanoparticles were further encapsulated into Eudragit(®) FS 30D to prepare PLGA nanopar

Antibacterial activity of synthetic curcumin derivatives: 3,5-bis(benzylidene)-4-piperidone (EF24) and EF24-dimer linked via diethylenetriaminepentacetic acid (EF2DTPA).

Curcumin is well known for its antimicrobial and anti-inflammatory properties. However, since systemic absorption and bioavailability of curcumin from gastrointestinal tract is considerably poor, synthetic curcuminoids are being developed as better alternatives. Two curcumin derivatives: 3,5-bis(benzylidene)-4-piperidone (EF24) and EF24-dimer linked via diethylenetriaminepentacetic acid (EF2DTPA), were included in this study. We investigated the antibacterial activity of EF24 and EF2DTPA against Gram-negative (Escherichia coli) and Gram-positive (Enterococcus faecalis, Staphylococcus aureus) b

Hepatic injury to the newborn liver due to drugs.

The result of the use of drugs in the newborn may be strongly influenced by the peculiar state of the neonate, characterized by the immaturity, at birth, of the processes controlling the absorption, distribution, metabolism and excretion of drugs. Additional important factors that may affect drugs' bioavailability and toxicity are gestational age, birth weight, intrauterine growth restriction, gender and, especially, liver function immaturity. Because of the high susceptibility to infections, antibiotics, in particular ampicillin and gentamicin, are the most widely used drugs in newborns. Eryt

[Drug therapy for the elderly patients with gastric ulcer].

The number of patients with low dose aspirin or non-steroidal anti-inflammatory drug-related gastric ulcers is steadily increasing. The elderly patients have higher risk to develop gastric ulcers during these treatments. For the treatment and prophylactic therapy of the ulcers in the elderly, the following points should be recognized. (1) The drugs that are prescribed for ulcer diseases may interact with drugs that have been administered for the treatment of the pre-existing diseases. (2) The decreased motor function of gastrointestinal tract frequently observed in the elderly may interfere wi

Physiologic changes in pregnancy and their effect on drug disposition.

Changes in maternal physiology occur normally during pregnancy and have the potential to alter the absorption, distribution, and elimination of drugs used therapeutically in pregnant women. These physiologic changes include: plasma volume expansion and increases in extracellular fluid space and total body water; decreased plasma albumin concentration; a compensated respiratory alkalosis; increased cardiac output with regional blood flow changes; increased renal blood flow associated with increased glomerular filtration; changes in hepatic drug metabolizing enzymes; and changes in gastrointesti

[Intensive care management of diffuse septic peritonitis].

The function of the peritoneum in severe abdominal sepsis involves specific properties and defence mechanisms: large surface, efficient barrier, continuous mobility, capacity of migration, multiplication, secretion and absorption. The development of peritonitis supposes an initial lesion of the peritoneal surface by septic or biochemical mechanism. Peritoneal response consists of: septation of the abdominal wall, adhesion of the omentum to damaged surfaces or visceral perforation, massive stepping up of cellular and humoral defence mechanism. In our opinion therapeutic procedures consist of: e

Discriminant analysis for the diagnosis of childhood celiac disease.

A new statistical approach to the analysis of laboratory data has been introduced to optimize the use of absorption tests and gliadin antibody measurement for the diagnosis of childhood celiac disease. Serum antigliadin antibodies, as well as blood xylose, iron, and tryglycerides after oral load, were evaluated in 40 celiac children and 43 age-matched patients affected by other gastrointestinal diseases. Each test evaluated individually gave a considerable rate of false-positive and false-negative results. Discriminant coefficients produced for each test were used to compute a score that allow

Age-dependent response of the rat testes to di(2-ethylhexyl) phthalate.

Spermatogenesis starts soon after birth in the Sprague-Dawley rat but is not fully established until about 56 days of age. When high oral doses of the plasticizer di(2-ethylhexyl) phthalate (DEHP) were given to rats of different ages, testicular damage was observed in immature but not in mature animals. Plasma concentration and urinary excretion data suggested that the gastrointestinal absorption of the DEHP-derived metabolite mono(2-ethylhexyl) phthalate was higher in young animals. Young rats were not more susceptible than older for repeated intravenous infusions of DEHP. It is suggested tha…
