INTRODUCTION: Transcatheter aortic valve replacement (TAVR) is increasingly used to treat aortic stenosis, including in low-risk patients. However, post-procedural cardiac conduction abnormalities (CCA), often requiring permanent pacemaker implantation (PPI), remain a concern. This study investigates how the volume and distribution of aortic leaflet calcium deposits influence the risk of post-TAVR CCA.
METHODS: An electromechanically coupled four-chamber beating heart model was used to simulate TAVR with a self-expandable Evolut® 26 mm device. Five virtual patient scenarios were modeled with

Atrioventricular (A-V) conduction, ventriculo-atrial conduction and mechanism of tachycardia were studied by programmed electrical stimulation before and after the administration of ouabain in 15 patients suffering from paroxysmal supraventricular re-entrant tachycardia. In 13 patients the tachycardia circuit was confined to the A-V node. In two patients the stimulation study showed that an accessory pathway was used in a ventriculo-atrial direction during tachycardia. Ouabain lengthened the effective and functional refractory period of the A-V node and A-V nodal transmission time in all patie

Becker muscular dystrophy (BMD) is a primary myopathy caused by mutations in the dystrophin gene on chromosome Xq21.2, which often affects the myocardium and cardiac conduction system. However, there is limited experience with cardiac disease in BMD. This review aims to summarize and discuss recent advances and future perspectives regarding the clinical presentation, diagnosis, treatment, and outcome of cardiac involvement in BMD. Data were retrieved by searching for relevant articles in the PubMed and Google Scholar databases. Cardiac disease in BMD is characterized by myocardial fibrosis, ca

BACKGROUND AND OBJECTIVE: Left bundle branch area pacing (LBBAP) is increasingly used for treating bradycardia and heart failure. However, real-world data are limited. The Conduction-System Pacing Italian Network Group (C-SING) collected prospective data on LBBAP procedures in a nationwide context.
METHODS: Observational data from 28 Italian sites were analysed for consecutive LBBAP procedures, comparing outcomes based on operator experience levels.
RESULTS: From January 2022 to December 2023, 1250 patients (median age 78, 66.2 % male) underwent LBBAP attempt. Most frequent indications were

Sotalol is a beta-blocker with class III antiarrhythmic properties that has recently been used in children for the treatment of supraventricular and ventricular arrhythmias. However, little is known about its electrophysiologic effects on the immature heart. Using intracardiac electrocardiographic recordings and stimulation techniques, 15 canine neonates (8-15 days) and 15 adult mongrel dogs were studied with cumulative doses of sotalol (0.5, 1, 2, and 4 mg/kg plus an additional dose of 8 mg/kg for neonates). Heart rate decreased significantly in the two groups, but more in adult dogs (-43% in

Wild-type transthyretin amyloidosis (ATTRwt) is an age-associated systemic disorder characterized by extracellular deposition of misfolded transthyretin amyloid fibrils, leading to progressive organ dysfunction. Cardiac involvement is common and may result in restrictive cardiomyopathy, arrhythmias, and conduction disturbances, including left bundle branch block (LBBB). Although pharmacological therapy with transthyretin stabilizers, such as tafamidis, has been shown to reduce mortality in patients with ATTRwt cardiac amyloidosis (ATTRwt-CA), the role of device-based therapies, such as cardiac

Transcatheter aortic valve replacement (TAVR) is an expanding, catheter-based technology that allows the implantation of a prosthetic valve without requiring open heart surgery for the treatment of severe aortic stenosis (AS). The frequency of coronary artery disease (CAD) in patients (pts) with severe AS undergoing surgical treatment ranges from 30 to 50 %. This tends to be higher in pts undergoing TAVR with a prevalence of 49-76 % and is more prevalent with older age and the fact that TAVR is commonly performed in high-risk groups with more advanced cardiovascular disease. Surgical disruption of the small (approximately 0.7 x 1.0 cm) epicardial fat pad situated at the junction of the inferior vena cava (IVC) and inferior surface of the left atrium (ILA) interrupts both right and left vagal input to the atrioventricular nodal (AVN) region of the canine heart. This intervention eliminates AV block during supramaximal stimulation of both cervical vagi, without interfering with sinus bradycardia normally associated with sinoatrial nodal (SAN) suppression. Independent modulation of SAN and AVN activities by the parasympathetic system is thereby revealed. Histology of

Left bundle branch block (LBBB) describes a disruption or delay in electrical conduction through the left bundle branch of the His-Purkinje system (HPS). While new-onset LBBB commonly emerges in individuals with a history of hypertension, cardiomegaly or coronary artery disease, it can also occur in the absence of other cardiovascular abnormalities. Whether LBBB is a cause or a consequence of heart failure remains unclear. In this work, we used six different electromechanical in silico heart models with an integrated HPS to simulate both a healthy control and a pure LBBB case for each geometry

The cause of severe disturbances of the cardiac conduction system is seldom possible to establish clinically at pacemaker implantation, apart from cases of acute myocardial infarction or digitalis intoxication and in relatively rare cases of inflammatory disorders such as sarcoidosis and systemic sclerosis. Since cardiac manifestations, mainly conduction disturbances, occur in patients with ankylosing spondylitis, the prevalence of this disease was determined using radiologic screening for sacroiliitis in a population of 223 men who had permanently implanted pacemakers. Sacroiliitis was found

Although Whipple's disease is clinically recognized for its features of arthritis and diarrhea, pericarditis and cardiac valvular disease are frequently present and a significant number of such patients die suddenly and unexpectedly. This report includes special postmortem cardiovascular examinations in a 55-year-old man who died of Whipple's disease. Pericarditis and valvular disease were extensively present. There was also focal myocardial degeneration, including the sinus node, A-V node and His bundle. Typical Schiff-positive bacilli were found in the tunica media and endothelium most numer

BACKGROUND: Electrocardiographic diagnosis of acute myocardial infarction in the setting of cardiac pacing represents diagnostic challenge. There are no focusing data, neither reporting about diagnostic sensitivity of 12‑lead ECG with left bundle branch area pacing (LBBAP) during acute myocardial infarction (AMI).
CASE SUMMARY: We present 12‑lead ECG morphology in a patient with permanent LBBAP during AMI.
DISCUSSION: Abnormal repolarization changes induced by ventricular pacing can lead to delay in diagnosis in patients with AMI. LBBAP and overall conduction system pacing may facilitate a t

Cardiac sarcoidosis (CS), a rare complication of systemic sarcoidosis, can have subtle or no symptoms. It is characterized by granuloma formation in the myocardium, which can occur in isolation or alongside systemic sarcoidosis. Clinical manifestations include conduction system disorders (e.g., atrioventricular block and ventricular tachyarrhythmia), heart failure, and sudden cardiac death. Timely evaluation and screening for CS are crucial, especially in systemic sarcoidosis patients with limited symptoms. We present the case of a 50-year-old African-American male diagnosed with cardiac sarco

Cardiac physiologic pacing (CPP), encompassing cardiac resynchronization therapy (CRT) and conduction system pacing (CSP), has emerged as a pacing therapy strategy that may mitigate or prevent the development of heart failure (HF) in patients with ventricular dyssynchrony or pacing-induced cardiomyopathy. This clinical practice guideline is intended to provide guidance on indications for CRT for HF therapy and CPP in patients with pacemaker indications or HF, patient selection, pre-procedure evaluation and preparation, implant procedure management, follow-up evaluation and optimization of CPP

Pacing induced cardiomyopathy (PICM) is most commonly defined as a drop in left ventricle ejection fraction (LVEF) in the setting of chronic, high burden right ventricle (RV) pacing. Recent data suggest, however, that some individuals may experience the onset of heart failure symptoms more acutely after pacemaker implantation. Definitions of PICM which emphasize drop in LVEF may underestimate the incidence of deleterious effects from RV pacing. Treatment of PICM has primarily focused on upgrade to cardiac resynchronization therapy (CRT) when LVEF has dropped. However, emerging data suggest tha

We present two symptomatic sisters who had a positive family history of sudden death. None of them had structural heart disease. In the 25-year-old proband, complex ventricular arrhythmia, cardiac conduction system disease, and skeletal muscle weakness were found. Genetic examination showed a pathogenic intronic variant in the desmin gene in the proband only. In the elder sister with palpitations, complex ventricular arrhythmia (>46 000 ectopic beats) was removed by radiofrequency ablation. This family case shows that complex ventricular arrhythmia may have different background within one fami

A unicuspid aortic valve is a rare congenital malformation that frequently presents with valvular dysfunction and dilatation or aortic aneurysm, requiring combined aortic valve surgery and aortic repair. Some patients show severe valve calcification extending into the interventricular septum, possibly resulting in damage to the conduction system during debridement for valve replacement. We present a rare case of severe aortic stenosis with a unicommissural unicuspid aortic valve diagnosed by preoperative transesophageal echocardiography in a 36-year-old man. After composite graft replacement o
