AIMS: To test the hypothesis that acute increased biventricularly (BiV) paced heart rate (pHR) results in decreased muscle sympathetic nerve activity (MSNA), and that dyssynchronous pacing (AAI) attenuates this effect, in heart failure patients receiving cardiac resynchronization therapy (CRT).
METHODS AND RESULTS: Fourteen CRT patients (NYHA II-III, 12 males, mean EF 28 ± 14%) were recruited. Three different pHRs (50-90 b.p.m.) were randomly programmed in BiV- and AAI-pacing modes. Muscle sympathetic nerve activity (total sympathetic nerve activity/min (units) and number of bursts/100 RR) we…

BACKGROUND: Ventricular ectopic beats (VEBs) are usually considered a benign condition that can be managed with conservative measures. Heart rate variability (HRV), which is one of the most important methods for assessing autonomic activity, is a noninvasive, quantitative method of analyzing autonomic effects on the heart. We aimed to investigate the risk of arrhythmia in patients with VEBs and without cardiovascular disease by using HRV parameters.
METHODS: Patients with frequent VEBs (more than 30 times in 1 hour, according to the Lown classification) were identified. Identified patients we…

Congenital heart defects affect the efficiency and functionality of the heart, and autonomic control of heart rate and of circulation can display a pathologic behavior in order to compensate for the hemodynamic alterations due to disease. While previous works have investigated heart rate variability (HRV) in specific pathologies, e.g. tetralogy of Fallot, the goal of this study was to assess HRV in children with a congenital heart malformation taking into account the effects of cyanotic and acyanotic defects, and comparing pathologic children with age matched controls. HRV, approximate entropy…

The following studies were conducted to determine if the ability of the intrinsic cardiac opioid, met-enkephalin-arg-phe to interrupt vagal bradycardia can be generalized to include the disruption of vagal effects on atrial contraction and coronary blood flow. Anesthetized dogs were instrumented to measure heart rate and left atrial contractile force or heart rate and coronary blood flow. The response of each variable was recorded at rest and during vagal stimulation. During the evaluation of vagal effects on contractile activity and coronary blood flow, heart rate was maintained constant by e…

BACKGROUND: Obstructive sleep apnea (OSA) imposes cardiovascular stress through intermittent hypoxia and event-related responses. Whether heart rate surge adds prognostic information beyond hypoxic burden (HB) remains unclear.
OBJECTIVES: The authors aimed to evaluate heart rate surge for cardiovascular mortality and its incremental value beyond HB in OSA.
METHODS: We analyzed 4,790 participants from a community-based cohort with overnight polysomnography. Heart rate surge was defined as peak heart rate within the event-aligned window minus mean heart rate during the 60-second pre-event base…

Detecting exercise intensity thresholds is key for optimizing endurance training prescriptions for performance and recovery. Over the past several years an index of heart rate variability representing fractal correlation properties (DFA a1) has been investigated with respect to exercise intensity threshold demarcation and durability. We intend to show that sufficient data has accumulated to recommend it for widespread consumer use in tested populations. Summary: Validated consumer hardware, software, along with strong study results, support using DFA a1 monitoring to evaluate training zones an…

Heart rate variability (HRV) represents a rich physiological signal that captures the computational dynamics of autonomic regulation. Emerging evidence suggests that atypical autonomic control contributes to the neurocognitive phenotype of neurodevelopmental disorders (NDD), including autism spectrum disorder (ASD) and attention-deficit/hyperactivity disorder (ADHD). However, the characterization of HRV alterations in these populations remains incomplete. This review was motivated by the broader research context of the EMPOWER project, funded through the Horizon Europe program, which investiga…

Variations in fetal heart rate variability (HRV) may serve as early indicators of risk for later psychopathology. These individual differences could further interact with the postpartum environment to shape developmental outcomes. The present study investigated the relation between fetal HRV and toddler psychopathology in the context of postnatal parenting stress as an index of infants' early parenting environment. Mother-infant dyads (N = 159) were followed from the third trimester through 18 months postpartum. Fetal HRV was collected remotely during a 20-minute baseline at 34-40 weeks' gesta…

BACKGROUND The Lin Oral Appliance (LOA) is a novel oral device intended to reduce snoring. Heart rate variability (HRV) can noninvasively represent autonomic function. We investigated whether LOA use influences autonomic modulation in adolescent athletes using HRV indices. MATERIAL AND METHODS This prospective study analyzed HRV in time and frequency domains. RMSSD, SDSD, and p.NN50 represented parasympathetic activity; low-frequency (LF)/high-frequency (HF) ratio represented sympathetic balance. Two-way repeated-measures analysis of variance evaluated associations of sex and test sequence. As…

This study examined associations between multi-day ultra-short resting heart rate (HR) parameters and cardiometabolic health under free-living conditions. Sixty-seven adults (18-59 years, 53% male) completed seven consecutive days of seated 1-min RR interval recordings after waking using a validated chest strap monitor. Log-transformed root mean square of successive differences (LnRMSSDM) and resting HR (HRM) were averaged across days and standardized. Cardiometabolic health status was defined as the number of metabolic syndrome (MetS) components present. Associations were assessed using Poiss…

Acyl ghrelin increases cardiac output (CO) and contractility in patients with heart failure with reduced ejection fraction (HFrEF). In healthy humans, acyl ghrelin increases gastric emptying rate (GER) and hunger, a potential add-on benefit for HFrEF patients suffering from cachexia with symptoms of delayed gastric emptying and loss of appetite. The aim of this study was to determine if post-prandial acyl ghrelin infusion increases GER and hunger in HFrEF patients compared to CO. HFrEF patients (n = 29) arrived fasted and received a 500-kcal breakfast followed by 1.5 g paracetamol. They next r

Chronic kidney disease (CKD) is associated with increased cardiovascular morbidity and mortality, partly mediated by autonomic dysfunction. Heart rate recovery (HRR), particularly during the first minute after exercise (HRR1), reflects early parasympathetic reactivation and provides an integrative measure of cardiovascular autonomic regulation. However, data across non-dialysis CKD stages remain limited. In this prospective observational study, 120 patients with CKD stages 1-4 and 30 age- and sex-matched healthy controls underwent symptom-limited treadmill exercise testing. HRR was calculated …

Heart rate variability (HRV) is a widely utilized approach for investigating cardiovascular health and human performance. Methodological approaches, analyses, and interpretations of HRV can vary extensively, and scientific debates on HRV reliability and utility remain. These debates have been amplified in recent years by the rapid growth and ubiquity of wearable devices and their incorporation into scientific research studies. The present guidelines paper takes an evidence-based approach to propose more consistent and rigorous measurement and interpretation of HRV within human cardiovascular r…
