PURPOSE OF REVIEW: Sutureless aortic valve replacement (AVR) has emerged as an alternative to traditional AVR for patients with aortic stenosis who present a higher surgical risk, such as the elderly, or those with small or highly calcified aortic roots. With transcatheter aortic valve implantation - the other major AVR alternative - being used in increasingly lower-risk patients, the place of sutureless valves in the AVR landscape needs to be defined. In this review, we discuss recent data and expert opinion as it pertains to the subject of sutureless AVR.

Paravalvular leak (PVL) remains a significant problem after transcatheter aortic valve replacement. PVL is associated with long-term morbidity and mortality. Percutaneous management of post-TAVR PVL seems to be feasible and should be adopted by the structural heart interventionalist.

PURPOSE OF REVIEW: Bicuspid aortic valve stenosis is regarded as a relative contraindication for transcatheter aortic valve replacement (TAVR) according to current guidelines, as patients with bicuspid anatomy (BiAV) were excluded from landmark clinical trials. In this review, we will discuss the feasibilty, safety and outcomes of TAVR in BiAV, based upon the current literature and our own experience.
RECENT FINDINGS: The currently available data come from observational registries with low sample size, and only in a few were the results of TAVR in BiAV compared with those achieved in the cohor

Valve-in-valve transcatheter aortic valve implantation (ViV-TAVI) is an established therapy for a degenerated surgical bioprosthesis. TAVI-in-TAVI following ViV-TAVI has not been previously performed. We report a high-risk patient presenting with severe left ventricular failure secondary to undiagnosed critical aortic stenosis due to degeneration of the implanted transcatheter heart valve more than a decade after initial ViV-TAVI for a failing stentless aortic valve homograft. Successful TAVI-in-TAVI reversed the clinical and echocardiographic changes of decompensated heart failure with no evi

CASE SUMMARY: A patient who underwent mechanical aortic and mitral valve replacement developed three paravalvular leaks 10 months later. We located the tracks by puncturing the apex cordis under transoesophageal echocardiography guidance alone and puncturing the femoral artery guided by fluoroscopy. Three paravalvular leaks were occluded with a hybridization method simultaneously. The patient was followed up for 24 months and maintained a good condition.
CONCLUSION: Multiple paravalvular leaks after double valve replacement can be occluded in patients by the use of different approaches under

A 74-year-old patient presented to the emergency department with aggravating asthenia and persistent fever over the course of the last 2 weeks. He benefited 3 years prior from a self-expandable transcatheter aortic valve implantation (TAVI) for symptomatic severe aortic valve stenosis, as he refused open heart surgery. The blood workup showed leukocytosis and high C-reactive protein levels. However, the microbiological analysis remained negative. During his hospital stay, a transesophageal echocardiogram was performed, which showed thickening of the transcatheter heart valve leaflets and a veg

BACKGROUND: Selecting between mechanical and bioprosthetic valves depends on multiple clinical and patient-specific criteria. This study aims to compare mechanical and bioprosthetic valve replacement outcomes in Egypt to support evidence-informed decision-making.
METHODS: We conducted a narrative literature review to identify studies reporting outcomes of mechanical and bioprosthetic valve replacements in Egypt. Findings were validated through an expert meeting with cardiothoracic surgeons. To capture real-world practice, an online survey was administered to cardiothoracic surgeons across Egy

Between May 1995 and May 2004, 197 ATS valves were implanted in 182 patients: 120 males and 62 females with an average age of 58 +/- 13 years. 149 cases were for aortic valvular replacement and 48 cases were for the mitral valve. Fifteen patients had a double mitral and aortic replacement. Twelve tricuspid procedures were necessary, 17 patients underwent coronary revascularisation and 58 underwent an aortic procedure (Bentall, aortic sub-coronary, aortic cross). The in-hospital mortality (31 days) was 1.6%. The long term mortality at up to 9 years included 23 deaths. No death was attributed to

In the total pre-clinical evaluation of new heart valve substitutes, there is an absolute need for chronic experimental valve testing in different animal models. Also fundamental research towards mechanisms of calcification, tissue degeneration and valve thrombosis requires standardized and well-controlled animal models. Possible clinical use of a new experimental valve type and/or future developments and improvements in prosthetic heart valves, all depend on such research activities. Our recent studies concerning prosthetic heart valves resulted in following conclusions. 1. Photo-oxidation, a

AIMS: Prosthetic valve endocarditis (PVE) is the most severe form of infective endocarditis associated with a high mortality rate. Whether PVE affects biological and mechanical aortic valves to the same extent remains controversial. This study aimed to compare the incidence of re-intervention because of PVE between bioprosthetic and mechanical valves.
METHODS: Patients undergoing isolated surgical aortic valve replacement (AVR) or combined AVR in a single cardiac surgery centre between January 1998 and December 2019 were analysed. All patients who underwent re-intervention because of PVE were

Transcatheter pulmonary valve replacement (TPVR) is commonly performed in patients with congenital heart disease as a safe alternative to replacement via open heart surgery. Intracardiac echocardiography (ICE) is a useful technique for evaluating multiple structures that are difficult to assess by other echocardiographic techniques, particularly the pulmonary valve. To our knowledge, the use of three-dimensional (3D) ICE catheters to evaluate prosthetic valves after TPVR has not been reported. Three-dimensional ICE catheters offer a comprehensive evaluation of transcatheter-deployed pulmonary

Sixty years ago, 2 cardiac operations dramatically influenced the survival of patients with valvular heart disease. The replacement of an aortic valve by Dwight Harken and of a mitral valve by Albert Starr with mechanical caged-ball valves, both in 1960, was a true milestone in the history of cardiac surgery and the beginning of a long journey toward prosthetic valve replacement full of expectations, hopes, and dreams fulfilled. Caged-ball prostheses underwent numerous modifications in design and materials to improve reliability and prevent specific mechanical and thrombogenic complications. C

BACKGROUND: Aortic stenosis in older adults can lead to symptoms of breathlessness, dizziness, syncope and angina. While surgical AVR is the optimum treatment, it is not suitable for frail older people who may have other significant comorbidities. TAVI provides a safer alternative. There is evidence that aortic valve replacement reduces mortality and morbidity, but less is known about its effect on quality of life (QoL) in older adults.
AIMS AND OBJECTIVES: To carry out a narrative review of the existing literature on quality of life (QoL) after surgical aortic valve replacement (AVR) or tran

During the past decade, interest in the development of less-invasive cardiac valve surgery has undergone a steady increase, with many surgeons having supplanted full sternotomy with minimal-access incisions. While the minimally invasive approaches for the treatment of mitral disease have been standardized, the preferred route for aortic valve replacement (AVR) remains a matter of debate. Although AVR through a right minithoracotomy avoids opening the sternum, it does require a greater surgical ability and may be a challenging procedure, even for expert surgeons. By simplifying the valve implan

BACKGROUND: The ATS Open Pivot Heart Valve was first introduced in 1992 and has been implanted routinely at our institution since 1993. Valve selection was based on surgeon preference. The objective of this study is to retrospectively analyze our 11-year clinical results with ATS prostheses.
METHODS: Between January 1993 and December 2003, 601 ATS valves (393 aortic valve replacement [AVR], 168 mitral valve replacement [MVR], 20 aortic plus mitral valve replacement [DVR]) were implanted in 581 patients (377 male, 204 female; mean age, 63.7 years; range 18 to 89). Preoperatively, 47 (8%) were

OBJECTIVES: Pulmonary valve implantation (PVI) is common in children with congenital heart disease particularly as they get older. While surgical PVI (SPVI) was the mainstay of treatment for years, percutaneous PVI (PPVI) has evolved into a popular treatment option. The aim of our study was to evaluate the rate of prosthetic pulmonary valve infectious endocarditis (PPVIE) between these two modes of intervention.
METHODS: Retrospective, single-centre review of paediatric patients at a tertiary referral centre from 1993 to 2022 who underwent PVI. Method of intervention (SPVI vs PPVI) and type o
