Paroxysmal nocturnal haemoglobinuria in pregnancy-a systematic review with meta analysis.

The co-occurrence of Paroxysmal nocturnal haemoglobinuria (PNH) and pregnancy has traditionally been associated with poor feto-maternal outcomes. Eculizumab, a terminal complement inhibitor, is often used in pregnant patients, but the drug is not universally available. We therefore present the first systematic review with meta-analysis of outcomes in 190 pregnancies from 135 PNH patients, comparing those receiving eculizumab and those not. We searched PubMed for studies reporting on pregnancy outcomes in PNH. Thirty-five papers were selected for inclusion, the majority of which were case studi

Consensus recommendations for optimising the diagnosis and treatment of paroxysmal nocturnal haemoglobinuria in Singapore.

INTRODUCTION: Paroxysmal nocturnal haemoglobinuria (PNH) is a rare haematologic disease characterised by intravascular haemolysis, thrombophilia and bone marrow failure. There is a lack of established clinical guidance on the screening, diagnosis and manage-ment of PNH in Singapore. A relatively low level of awareness among healthcare professionals regarding PNH manifestations further contributes to diagnostic delays. Additionally, limited access to complement inhibitors, like eculizumab, may delay treatment and impact patient outcomes. METHOD: Nine haematologists from different institu-tions

Detection of paroxysmal nocturnal haemoglobinuria clones in cases of deep vein thrombosis in a tertiary care centre, western Rajasthan.

BACKGROUND & OBJECTIVES: Paroxysmal nocturnal haemoglobinuria is a rare acquired disease characterized by bone marrow failure, intravascular haemolysis and thrombophilia. Thrombosis is the deadliest complication of paroxysmal nocturnal haemoglobinuria (PNH). The present study was conducted to study the prevalence of PNH in cases of deep vein thrombosis (DVT) which was previously undocumented from western Rajasthan. METHODS: In the present cross-sectional study, 61 adult patients with DVT were tested using flow cytometry to detect PNH clones. Blood samples were processed using fluorescein-labe

Pegcetacoplan: First Approval.

Pegcetacoplan (Empaveli™) is a PEGylated pentadecapeptide developed by Apellis Pharmaceuticals for the treatment of complement-mediated diseases. It binds to complement component 3 (C3) and its activation fragment C3b, controlling the cleavage of C3 and the generation of the downstream effectors of complement activation and thus both C3b-mediated extravascular haemolysis and terminal complement-mediated intravascular haemolysis. Pegcetacoplan is the first C3-targeted paroxysmal nocturnal haemoglobinuria (PNH) therapy to be approved (in May 2021) in the USA, where it is indicated for the treatm

[Inherited GPI deficiencies:a new disease with intellectual disability and epilepsy].

Glycosylphosphatidylinositol (GPI) is a glycolipid, which anchors 150 or more types of proteins to the cell surface. There are at least 26 genes involved in the biosynthesis and transport of GPI-anchored proteins (GPI-APs). Many inherited GPI deficiencies (IGDs) have been recently found using whole-exome sequencing. Patients with IGD have only a partial deficiency because complete GPI deficiency causes embryonic death. The major symptoms of IGDs include intellectual disability, epilepsy, coarse facial features, and multiple organ anomalies. These symptoms vary in severity depending upon the de

Complement inhibition to treat myocardial infarction?

The authors describe the case of a middle-aged women who presented with an acute myocardial infarction due to thrombotic occlusion of angiographically normal coronary arteries. Coronary thrombosis was caused by a hypercoagulable state related to a haemolytic crisis of paroxysmal nocturnal haemoglobinuria and the patient was treated conservatively with antithrombotic agents. The clinical course was complicated by both severe bleeding and thrombotic complications and the patient eventually died of a massive intracerebral haemorrhage. The rapid occurrence of complications inhibited a timely admin

Intravascular hemolysis secondary to aorto-atrial fistula presenting as red urine.

Intravascular hemolysis is a rare but potentially life threatening cause of red urine characterized by brisk hemolysis and release of large amounts of hemoglobin into the urine. We present an unusual case of red urine in a 20-year-old male who was subsequently diagnosed with intravascular hemolysis due to an aorto-atrial fistula. Fistula formation was likely secondary to a recently implanted atrial septal occluder, which is a reported but exceedingly rare complication of the device. We discuss the diagnostic approach to hemolytic anemia and conclude with a literature review of other cases of d

An evolutionary paradigm for carcinogenesis?

Mutations seem to be only one of the mechanisms involved in carcinogenesis; selection of mutated clones is a second crucial mechanism. An evolutionary (darwinian) theory of carcinogenesis can be useful to explain some contradictory observations of epidemiology, and to provide a common theoretical framework for carcinogenesis. In both the selection of species and in carcinogenesis (selection of mutated cells), mutation and selection can be interpreted as necessary and insufficient causes. Selection presupposes competition among clones-that is, survival advantage of the mutated species; without

Inhibition of complement-mediated haemolysis in paroxysmal nocturnal haemoglobinuria by heparin or low-molecular weight heparin.

Complement (C')-mediated haemolysis in paroxysmal nocturnal haemoglobinuria (PNH) is mainly due to the deficiency of glycosyl phosphatidylinositol-anchored membrane proteins with C'-regulatory activities CD55 and CD59 in PNH-affected red blood cells (RBCs). Hydrophobic insertion of C5b-7 to RBC membranes, initiating the formation of a membrane attack complex, readily results in lysis of PNH RBCs due to the deficiency of CD59. We studied the significance of the electrostatic interactions between C5b-6 and RBC membranes preceding the insertion of C5b-7. In vitro, C'-mediated lysis of PNH RBCs (a

Complement-induced procoagulant alteration of red blood cell membranes with microvesicle formation in paroxysmal nocturnal haemoglobinuria (PNH): implication for thrombogenesis in PNH.

Complement-induced procoagulant alteration of red blood cell (RBC) membranes in paroxysmal nocturnal haemoglobinuria (PNH) was examined. Microvesicles, deficient in acetylcholinesterase, were generated and released from PNH RBC upon complement activation. The microvesicles generated from complement-activated PNH RBC accelerated factor Xa-dependent plasma coagulation more than those generated from RBC by the treatment with ionophore A23187. When assessed by factor Xa-catalysed prothrombin activation, complement activation enhanced procoagulant properties of both normal and PNH RBC similarly, al

Idiopathic non-syphilitic paroxysmal cold haemoglobinuria in children.

Three examples of non-syphilitic paroxysmal cold haemoglobinuria (PCH) in children are described which occurred, within a period of 16 days, in association with a febrile illness. No definite viral aetiology or obvious epidemiological association could be established. A Donath-Landsteiner antibody of anti-P specificity was demonstrated in all three patients. The serological aspects of PCH are critically discussed.

Haemoglobinuria caused by propylene glycol in sheep.

HAEMOGLOBINURIA OCCURRED IN SHEEP ANAESTHETIZED BY AN INTRAVENOUS INJECTION OF PENTOBARBITONE SODIUM CONTAINING PROPYLENE GLYCOL: an equivalent dose failed to cause haemoglobinuria in rabbits. Intravenous injection of an aqueous solution of 20% propylene glycol caused haemoglobinaemia and haemoglobinuria in sheep. Neither distilled water nor 20% glycerol in water administered under identical conditions produced these effects.Haemoglobinuria occurred on some occasions when an aqueous 20% solution of propylene glycol was administered to sheep after an injection of saline: it never occurred when

Oncostatin M deficiency: A newly recognized inherited bone marrow failure syndrome with potential predisposition to myeloid malignancy.

Inherited bone marrow failure syndromes (IBMFSs) are rare, genetically heterogeneous disorders characterized by BMF with possible increased risk of myeloid malignancies. Recent advances in genomic diagnostic techniques have identified a new IBMFS, including Oncostatin M (OSM) deficiency, which is an autosomal recessive condition linked to early-onset BMF with possible predisposition to myelodysplastic syndrome (MDS). OSM is a cytokine in the interleukin-6 family and regulates hematopoiesis through the OSM receptor and leukemia inhibitory factor receptor-mediated signaling, activating pathways
