The role of rapid molecular diagnostics in managing bloodstream infections: a microbiological insight.

Bloodstream infections (BSIs) represent a significant clinical challenge due to their high morbidity and mortality rates, compounded by the increasing prevalence of antimicrobial resistance (AMR). Even though they are regarded as the gold standard, traditional diagnostic techniques like blood cultures frequently have low sensitivity and delayed findings. Rapid molecular diagnostics (RMDs) have completely changed how BSIs are identified and treated. By using cutting-edge methods like next-generation sequencing (NGS), loop-mediated isothermal amplification (LAMP), polymerase chain reaction (PCR)

Neonatal microbiome dysbiosis decoded by mNGS: from mechanistic insights to precision interventions.

The neonatal period is a critical stage for microbial colonization and immune system development, with dynamic changes in the microbiome closely linked to the pathogenesis of various diseases. Traditional microbiological testing methods have low sensitivity and time-consuming limitations compared to metagenomic next-generation sequencing (mNGS), which makes it difficult to meet the diagnostic and therapeutic needs of critically ill neonates. mNGS analyzes the total DNA in a sample without bias, allowing comprehensive identification of bacteria, viruses, fungi, and parasites, and resolution of

Eosinophilic vacuolated tumor of the kidney can have distant metastasis: a histo-molecular case study.

Eosinophilic vacuolated tumor (EVT) is an emerging renal entity and further studies are required to characterize this neoplasm. EVT is considered generally indolent because metastasis or death from the disease has never been reported. Herein, we report the first case of EVT with mediastinal lymph node metastasis to validate its at least borderline behavior. Based on the morphological features, we additionally used a large panel of immunohistochemical antibodies and next-generation sequencing (NGS) to confirm the diagnosis of EVT and exclude other renal entities. This case report highlights the

Comparative Analysis of the Tumour Mutational Burden in Erosive and Reticular Oral Lichen Planus.

OBJECTIVE: Oral lichen planus (OLP) is a chronic inflammatory disease classified as an oral potentially malignant lesion. The erosive and reticular forms of OLP have the potential for malignant transformation, with no consistent data indicating that one form is more likely to undergo malignant transformation than the other. Tumour mutational burden (TMB) is a parameter that represents the number of somatic mutations in the DNA of neoplastic cells. This study aimed to compare TMB levels in the two primary clinical forms of OLP, the erosive and reticular sub-types. METHODS: Next-generation sequ

A New Next-Generation Sequencing Approach in Human Cytomegalovirus for the Identification of Antiviral Resistance Mutations and Genotypic Classification.

This study introduces a new procedure for antiviral resistance analysis and genetic classification of human cytomegalovirus (HCMV) using next-generation sequencing (NGS) adapted to existing methodologies, aiming for more targets due to the recent use of new antivirals. It expands the classical investigation of mutations in UL54 and UL97 genes, associated with resistance to (val)ganciclovir, foscarnet and cidofovir, to include UL27, UL56 and UL89 genes, which target newer antivirals like maribavir and letermovir. Additionally, it includes the genetic analysis of UL55 (glycoprotein B) for genoty

Re: "Severe angiostrongyliasis with neuropsychiatric symptoms in vulnerable adults: Early diagnosis via next-generation sequencing and successful treatment".

Tang et al. (2025) report on two cases of neuroangiostrongyliasis in vulnerable adults, diagnosed via metagenomic next-generation sequencing (mNGS). We highlight the clinical value of mNGS in detecting atypical presentations, particularly in patients without eosinophilia or classic meningitic signs. While mNGS demonstrates diagnostic potential, its limitations and feasibility in low-resource settings must be acknowledged. We propose a contextualized approach integrating mNGS with cost-effective tools like LAMP and rapid antigen tests, especially for underserved populations. The commentary also

Scrub typhus (Orientia tsutsugamushi)-associated intracranial infection: a case report and review of diagnostic and therapeutic challenges.

Intracranial infections caused by Orientia tsutsugamushi (O. tsutsugamushi), the causative agent of scrub typhus, are rare but potentially fatal. This case report describes a 45-year-old male patient presenting with persistent fever, headache, altered mental status, and skin lesions. High-throughput sequencing of cerebrospinal fluid confirmed O. tsutsugamushi, while respiratory samples detected Pseudomonas aeruginosa and Aspergillus fumigatus, suggesting a mixed bacterial and fungal infection. After timely adjustment of anti-infective treatment, the patient's condition significantly improved.

Metagenomic Sequencing for Diagnosing Listeria-Induced Rhombencephalitis in Patient and Contaminated Cheese Samples: A Case Report.

Among the various causes of rhomboencephalitis, Listeria monocytogenes infection is the most common. However, conventional microbiological methods often yield negative results, making diagnosis challenging and leading to extensive, often inconclusive, diagnostics. Advanced molecular techniques like metagenomic next-generation sequencing (mNGS) offer a powerful and efficient approach to pathogen identification. We present a case of life-threatening rhomboencephalitis in a 32-year-old immunocompetent patient where extensive microbiological, immunological, and biochemical tests were inconclusive.

The evolving role of interventional cytopathology from thyroid FNA to NGS: Lessons learned at Federico II University of Naples.

Fine-needle aspiration (FNA) guided by ultrasound (US) has emerged as a highly precise diagnostic method for managing thyroid nodules, significantly diminishing unnecessary surgeries. The effectiveness of US-guided FNA is high when a single specialist performs the FNA procedure and the microscopy. This paradigm has paved the way for the evolution of interventional cytopathology, a specialist with a pivotal role in the preoperative diagnostic process, encompassing patient history review, clinical examination, FNA execution under US guidance, preparation, and microscopic interpretation of cytolo

Comprehensive analysis of thalassemia alleles (CATSA) based on third-generation sequencing is a comprehensive and accurate approach for neonatal thalassemia screening.

Thalassemia is one of the most common and damaging monogenic diseases in the world. It is caused by pathogenic variants of α- and/or β-globin genes, which disrupt the balance of these two protein chains and leads to α-thalassemia or β-thalassemia, respectively. Patients with α-thalassemia or β-thalassemia could exhibit a severe phenotype, with no simple and effective treatment. A three-tiered strategy of carrier screening, prenatal diagnosis and newborn screening has been established in China for the prevention and control of thalassemia, of which the first two parts have been studied thorough
