[The limping child with a painful hip].

Hip pain is a frequent cause for limping in children. Etiology work out is oriented by the age of the child, the existence of a traumatic or infectious background, the child's morphology and ethnic origin, and most of the diagnoses will require only a standard X-ray and an ultrasound to be confirmed. In this way, the three main causes to be considered in children of less than 10 years of age are septic arthritis, transient synovitis and Perthes disease. Septic arthritis will require an urgent treatment, whereas Perthes disease will need a long care and follow up in order to watch for a poor ou…

Hip pain in the young adult.

Hip pain in the adult between 20 and 40 years of age may present as a diagnostic and treatment dilemma. Recently, adult developmental dysplasia of the hip and femoroacetabular impingement have been identified as important pathologic causes. These disorders often present with subtle initial findings. However, early diagnosis and treatment are critical to avert or delay the need for reconstructive surgery.

Screening for hip abnormality in the neonate.

Clinical screening policies for the detection of hip instability or dysplasia of the hip vary internationally. There is general agreement in the Western world that at birth all hip joints should be clinically assessed by the Ortolani and Barlow tests. Currently, there is no consistency regarding who should undertake the examination, the results being worse when inexperienced personnel are used. These clinical tests have poor sensitivity and should be regarded as surveillance, not screening methods. Since the 1980s ultrasonographic assessment of the hip has become a valuable diagnostic tool. Ho…

Resurfacing arthroplasty in osteonecrosis of the hip.

Approximately 10% of the total hip replacements performed in the United States are for osteonecrosis. Resurfacing arthroplasty has received renewed interest, with several new designs being implanted worldwide. Proponents of resurfacing arthroplasty describe the advantages of bone conservation, preservation of joint mechanics, more physiologic loading of the bone, lower incidence of perioperative complications, and easier conversion to a secondary procedure if failure occurs. Critics recite previous poor results including high failure rates with femoral and acetabular loosening, osteonecrosis o…

Hip osteoarthritis: where is the pain?

BACKGROUND: Pain radiating below the knee is typically thought to originate from the lumbosacral spine rather than degenerative hip pathology. We investigated the lower limb distribution of pain using body image maps in 60 patients awaiting primary hip arthroplasty and in 60 patients awaiting spinal decompression for confirmed spinal stenosis. The perception of 33 orthopaedic registrars regarding distribution of hip pain was also assessed. RESULTS: Groin and buttock pain are significantly more common in hip osteoarthritis. The presence of groin pain is 84.3% of those sensitive for hip dysfunc…

Height loss and osteoporosis of the hip.

Osteoporosis of the hip is associated with hip fracture, a devastating outcome on both an individual and aggregate basis. Height loss is a frequent manifestation of vertebral osteoporosis and is simple to evaluate in the clinical setting. The goal of this study was to determine whether height loss is significantly associated with low bone mineral density at the femur, using a retrospective review of cross-sectional data from 2108 women referred for a bone density scan. Collected data included self-reported maximum adult height, current height, and total hip bone mineral density, along with oth…

Acute osteochondral injuries of the hip.

Acute osteochondral injuries of the hip are rare in the absence of associated dislocation. A series of cases is presented in which these injuries were sustained after relatively minor trauma to the hip. Radiologic studies revealed evidence of mild hip dysplasia and, in one case, an osteochondral fracture of the right femoral head. Scintigraphy revealed the fracture and unexpected uptake in the acetabulum, which was subsequently confirmed as being due to labral tears or acetabular osteochondral injuries. In addition, the scan showed evidence of a possible avulsion-type injury of the iliofemoral…

Management of periprosthetic fractures: the hip.

Treatment of periprosthetic fractures around a total hip arthroplasty is based on fracture timing (intraoperative or postoperative) and fracture location (acetabulum or femur). Most intraoperative fractures are treated by ensuring that the implant and fracture are stable and employing minor technique modifications to ensure that the fracture remains stable postoperatively. Major intraoperative fractures require more complex reconstruction. Postoperative fractures associated with well-fixed implants typically are treated with open reduction and internal fixation. Postoperative fractures associa…

Imaging of hip disorders in athletes.

Normal hip joint function is fundamental in running-, jumping-, and kicking-based sporting activities. Hip disorders do not account for a large portion of exercise-related injuries, but they can pose a clinical dilemma since symptoms tend to be non-specific. Conventional radiographs may demonstrate some causes of hip pain, such as stress fractures and degenerative joint disease. Magnetic resonance (MR) imaging of the hip has proven valuable in the diagnosis of radiographically occult osseous abnormalities and periarticular soft tissue disorders such as stress fractures, avulsion injuries, musc…

Limited hip abduction in the infant.

This article addresses two important questions: What is the relationship between limited abduction and hip dysplasia in infants? Does untreated limited abduction in the infant ultimately lead to developmental dysplasia of the hip? In the authors' population of 683 babies, limitation of abduction had a sensitivity for the diagnosis of hip dysplasia of 69%, a specificity of 54%, a positive predictive value of 43%, and a negative predictive value of 78%. In the group with sonographically proven hip dysplasia, 70 of 226 babies (31%) showed no limitation of abduction, and in the group without dyspl…

Hip and groin injuries in athletes.

Although athletic injuries about the hip and groin occur less commonly than injuries in the extremities, they can result in extensive rehabilitation time. Thus, an accurate diagnosis and well-organized treatment plan are critical. Because loads of up to eight times body weight have been demonstrated in the hip joint during jogging, presumably even greater loads can occur during vigorous athletic competition. The available imaging modalities are effective diagnostic tools when selected on the basis of a thorough history and physical examination. Considerable controversy exists as to the cause a…

Cone prosthesis for the hip joint.

The shape of the proximal segment of the femur must be taken into account when implanting femoral endoprostheses, especially those intended for cementless anchorage. Numerous femoral prostheses are available for the proximally broadly extending, "trumpet-shaped" morphology. However, the femur often has a narrow, more cylindrical configuration, as is frequently seen with dysplastic hip joints, but variants of the anatomical constitution or ethnic variants are also found. Conventional femoral prostheses with a proximal transverse oval or rectangular cross-section are often incorrectly positioned…

Development of the hip in diastrophic dysplasia.

We examined 50 patients with diastrophic dysplasia both clinically and radiologically. Two legally aborted fetuses were dissected. The mean age of the patients was 16.2 years (newborn to 38) and the mean follow-up was 11.4 years (3 months to 34 years). The fetal hips and MRI of newborn infants showed congruity and no significant joint deformity. Flexion contracture of the hip became evident later in 93% and was progressive. The radiological appearance of the proximal femoral ossific nuclei was delayed and in 17% of males and 28% of females the ossific nuclei had not appeared by the age of 12 y…

Ultrasonography of nondevelopmental dysplasia of the hips.

We examined 45 infants with nondevelopmental dysplasia of the hip (non-DDH), including congenital deficiency of the femur (CDF) (20 infants), neuromuscular syndrome (five infants), skeletal dysplasia (15 infants), and infection (five infants), and present the sonographic findings. Dysmorphic proximal femora, coxa vara, and abnormal echogenicity of the soft tissue and cartilage are features which differentiate patients with non-DDH from those with typical DDH.
