Description of a Multi-faceted COVID-19 Pandemic Physician Workforce Plan at a Multi-site Academic Health System.

BACKGROUND: The evolving COVID-19 pandemic has and continues to present a threat to health system capacity. Rapidly expanding an existing acute care physician workforce is critical to pandemic response planning in large urban academic health systems. INTERVENTION: The Medical Emergency-Pandemic Operations Command (MEOC)-a multi-specialty team of physicians, operational leaders, and support staff within an academic Department of Medicine in Calgary, Canada-partnered with its provincial health system to rapidly develop a comprehensive, scalable pandemic physician workforce plan for non-ventilat

From Ebola to H5N1: Strengthening the U.S. Special Pathogen Response System.

The National Special Pathogens System (NSPS) stratifies U.S. healthcare facilities by their readiness level to care for patients with high-consequence infectious diseases (HCIDs). While NSPS Level 1 and 2 facilities possess advanced biocontainment capabilities to care for patients for the duration of their illness, most U.S. hospitals fall under a NSPS Level 3 or 4 designation, with limited resources to manage patients with a suspected or confirmed HCID. However, emerging zoonotic threats like H5N1 underscore the need to bolster HCID preparedness across all NSPS Levels. Beginning in March 2024

Multilevel regression and poststratification interface: an application to track community-level COVID-19 viral transmission.

PURPOSE: Public health surveillance systems require high-quality data to represent the population. In the absence of comprehensive or random testing throughout the COVID-19 pandemic, we have developed a proxy method for synthetic random sampling to estimate the actual community-level viral incidence, based on viral testing of patients who are asymptomatic and present for elective procedures within a hospital system. METHODS: The approach collects routine testing data on SARS-CoV-2 exposure among outpatients and performs statistical adjustments of sample representation using multilevel regress

Lifelines at Risk: Safety-net Hospital Closures in the U.S.

The U.S. healthcare safety net, a critical lifeline for millions of vulnerable individuals, is facing mounting financial and systemic pressures. Safety-net hospitals (SNHs) have supported the nation's safety net. In this perspective piece birthed from a Society of General Internal Medicine workshop, we examine SNH closures, why this matters for patients, trainees, and clinicians, and offer actionable steps forward. From 2013 to 2023, over 60% of SNH closures occurred in Medicaid non-expansion states and disproportionately impacted rural areas. The closures impact access and care quality in alr

Rapid discharge checklists for people at the end of life who present to the emergency department.

People often express a preference for dying in a home setting rather than in hospital. In emergency departments (EDs), system pressures and the complexity of organising discharges mean that people who are approaching the end of life may be admitted to and die in hospital, even when discharging them to their preferred place may have been indicated and feasible. A rapid discharge checklist can support the process of safely and efficiently discharging patients at the end of life from the ED. It outlines essential steps in the discharge process, including confirming the suitability of discharge, l

Evaluating Sepsis Bundle Compliance as a Predictor for Patient Outcomes at a Community Hospital: A Retrospective Study.

BACKGROUND: Clinicians are encouraged to use the Centers for Medicare & Medicaid Services early management bundle for severe sepsis and septic shock (SEP-1); however, it is unclear whether this process measure improves patient outcomes. PURPOSE: The purpose of this study was to evaluate whether compliance with the SEP-1 bundle is a predictor of hospital mortality, length of stay (LOS), and intensive care unit LOS at a suburban community hospital. METHODS: A retrospective observational study was conducted. RESULTS: A total of 577 patients were included in the analysis. Compliance with the SE

[Criteria for treating MRSA in sputum].

Methicillin-resistant strains of Staphylococcus aureus (MRSA) are of particular significance for the management of patients with airway infections, since the disease course is often complicated and treatment rendered difficult by multiple resistance. Their prevalence is now slowly declining, but still alarmingly high. Hospital-acquired infections are predominant, but hospital-associated and community-acquired infections do occur, as do rare infections with livestock-acquired strains. Non-nosocomial strains are characterized by different pathogenic factors and a different spectrum of antibacter

Wastewater-based epidemiology for tracking bacterial diversity and antibiotic resistance in COVID-19 isolation hospitals in Qatar.

BACKGROUND: Hospitals are hotspots for antimicrobial resistance genes (ARGs), and play a significant role in their emergence and spread. Large numbers of ARGs will be ejected from hospitals via wastewater systems. Wastewater-based epidemiology has been consolidated as a tool to provide real-time information, and represents a promising approach to understanding the prevalence of bacteria and ARGs at community level. AIMS: To determine bacterial diversity and identify ARG profiles in hospital wastewater pathogens obtained from coronavirus disease 2019 (COVID-19) isolation hospitals compared wit

What happens in hospitals does not stay in hospitals: antibiotic-resistant bacteria in hospital wastewater systems.

Hospitals are hotspots for antimicrobial-resistant bacteria (ARB) and play a major role in both their emergence and spread. Large numbers of these ARB will be ejected from hospitals via wastewater systems. In this review, we present quantitative and qualitative data of extended-spectrum β-lactamase (ESBL)-producing Escherichia coli, vancomycin-resistant enterococci and Pseudomonas aeruginosa in hospital wastewaters compared to community wastewaters. We also discuss the fate of these ARB in wastewater treatment plants and in the downstream environment. Published studies have shown that hospital

Prevalence of selected underlying health conditions among active component Army service members with coronavirus disease 2019, 11 February-6 April 2020.

The novel coronavirus (severe acute respiratory syndrome coronavirus 2, or SARS-CoV-2) that causes coronavirus disease 2019 (COVID-19) is exhibiting widespread community transmission throughout most of the world. Previous reports have evaluated the risk of serious illness in civilians diagnosed with COVID-19; however, similar reports have not been compiled for the Army active component (AC) population. COVID-19 has been a reportable condition for the Department of Defense since 5 February 2020, and, as of the morning of 6 April, a total of 873 cases were reported to the Disease Reporting Syste

Transport and Management of Patients With Confirmed or Suspected Ebola Virus Disease.

The foundation of safe care for patients with confirmed or suspected Ebola virus disease is effective infection control practice, which requires implementation of appropriate administrative policies, work practices, and environmental controls, accompanied by focused education, training, and supervision. In 2002, Emory University partnered with the Centers for Disease Control and Prevention to develop a capability for the evaluation and management of individuals with serious communicable disease. In 2005, the University of Nebraska developed a similar isolation capability. In each case, the hos

[Fifty years of public service for Quebec community psychiatry services. Part I].

This essay comprises 2 parts. It aims to recognize the public service of psychiatrists of the Département de psychiatrie de l'Université de Montréal who served at the provincial level of the Ministry of Health and Social Services for deinstitutionalisation of policies and organisation of services, at the service of people with severe mental disorders. First with Dr. Camille Laurin post-face of the 1962 book Les fous crient au secours! (Mentally ill patients cry for help); then the insight on the latest phase of differentiated specialised clinics by Dr. Denis Lazure, who participated in 1962 to

Benefits of a prehospital stroke code system. Feasibility and efficacy in the first year of clinical practice in Barcelona, Spain.

BACKGROUND AND OBJECTIVES: Hospital admission delay is a main limiting factor for effective thrombolytic therapy in stroke patients. We developed a stroke code system for rapid request of emergency transportation to the hospital and a priority availability of the attending neurologist on the patient's arrival at the Emergency Department (ED). METHODS: Over a 1-year period, a 24-hour telephone hotline between the attending neurologist and the Barcelona public emergency coordination service was established. Priority 1 (P1) was defined as a patient with symptoms suggestive of acute stroke with o

Nurses' perspectives on perioperative pulmonary rehabilitation for patients with lung cancer in the pre-implementation phase: a CFIR 2.0-guided study.

BACKGROUND: Despite strong evidence supporting perioperative pulmonary rehabilitation (PR) for patients undergoing lung cancer surgery, little is known about factors that may influence implementation prior to program rollout, particularly in resource-constrained settings. Guided by the updated Consolidated Framework for Implementation Research (CFIR 2.0), this study aimed to identify anticipated barriers and facilitators to inform program refinement and future implementation strategies. METHODS: A descriptive qualitative design was used. Semi-structured interviews were conducted with nurses f
