While shoulder dislocations are a common injury, dislocation in the posterior direction is unusual and often missed clinically. A traumatic posterior shoulder dislocation can be caused by a direct blow to the anterior shoulder or if a posteriorly directed force is axially applied through the forward flexed arm. A posterior shoulder dislocation can occur after a violent muscle contraction induced by an electrical shock or grand mal seizure. Posterior shoulder dislocations are commonly associated with posterior glenoid rim fractures and anterior compression fractures of the humeral head. This di…

Two groups of RA patients were compared: 102 patients with osteonecrosis of the head of the femur (group 1) and 50 patients free of it (group 2). Group 1 patients had bilateral lesion of the hip joints (28% of cases), unilateral osteonecrosis of knee joint in 14.5% and bilateral in 2.9% of the patients. Unilateral humeral osteonecrosis occurred in 17.6%, bilateral in 4.9% of patients. Extra-articular manifestations of RA (lymphadenopathy, renal lesions and Cushing's syndrome) were diagnosed more frequently in group 1. Progression of osteonecrosis is related to a mean maximal day dose of predni

Sickle-cell disease (SCD) is probably the commonest cause of avascular necrosis worldwide, and its prevalence appears to be rising in developed countries. Avascular necrosis of the humeral head is a common complication but has not been previously studied in detail. We have reviewed 138 patients with SCD for clinical, radiological and functional abnormalities of the shoulder, using a radiological classification designed for avascular necrosis of the shoulder. Radiographic lesions, frequently bilateral, were found in 28% and only 53% of patients had normal shoulder function. The management of th…

Among 41 adult haemophiliacs 15 suffered from shoulder symptoms. We examined 12 patients by radiography and ultrasound. Four had bilateral symptoms making a total of 16 symptomatic shoulders. Of these, 10 had abnormal ultrasound scans with eight having evidence of rotator cuff tears. Evidence of bicipital tendonitis was found in two. Pain with loss of range of movement and a positive impingement sign was the most reliable clinical indicator of a cuff tear. Joint incongruity and superior migration of the humeral head were the best radiographic indicators. Rotator cuff tears are a common compone…

Field and laboratory studies of arm-swinging in gibbons reveal its singularity even compared to spider monkeys. On the basis of principal components and discriminant analyses of size-corrected forelimb variables, this study confirms their morphological uniqueness and the more generalized nature of the spider monkey forelimb. Long forearms, well-developed scapular spines, and sagittally thicker radial shafts are features associated strictly with gibbon arm-swinging. On the other hand, large humeral heads, projecting medial epicondyles, and axially elongated scapulae, traditionally regarded as a…

A new radiologic sign is described which seems to diagnose an early presubluxation phase of glenohumeral malalignment in hemiplegic shoulders. The sign consists of a V-shaped widening of the upper part of the space between the humeral head and the glenoid cavity on anteroposterior shoulder films in the erect position. Twelve of 14 patients showing this sign went on to develop chronically painful shoulders, and four of them developed radiologically evident subluxation within several months. The sign may be helpful in diagnosing shoulder pathology following stroke at an early stage, when orthoti…

INTRODUCTION: Elbow dislocation is usually treated conservatively with a good outcome. When there is associated injury to the capitellum, "Osborne-Cotterill Lesion," then the clinical course is less predictable and controversial. In this article, we attempt to define the critical point on the capitellum surface that, when breached, forebodes potential engagement of the radial head in the defect and imminent dislocation.
CASE REPORT: We present two cases of fracture and fracture dislocation of the elbow with "Osborne-Cotterill lesion." They were treated with fracture reduction and fixation. In

OBJECTIVE: To investigate the effectiveness of proximal humerus internal locking system (PHILOS) plate combined with rotator cuff reinforcem … bacromial impingement, fracture redisplacement, and screw removal during follow-up. One patient had humeral head necrosis, but the basic function of the shoulder joint was acceptable, the symptoms were mild, and no treatment was performed. At 3 months after operation, the upper limb function of the patients basically recovered. The VAS score, Constant-Murley s …

With increasing age, rotator cuff tears are common in symptomatic and asymptomatic patients. Yet, there remains a limited understanding of the etiology of pain associated with rotator cuff pathology. Recent research has associated patient perception, resilience, repair integrity, tear size, biomechanics of the humeral head, and muscle kinematics with pain levels. However, the evidence remains largely inconclusive in answering the fundamental question regarding why some patients experience symptoms severe enough to warrant surgery and others remain completely asymptomatic. A deeper understandin…

A 67-year-old woman with rheumatoid arthritis presented with an untriggered hematoma in the right shoulder joint. Radiographic findings showed humeral head collapse and destruction of the glenoid fossa with ectopic calcification. Calcium pyrophosphate deposition (CPPD) in the synovial fluid was observed using a polarizing microscope. Histopathological findings revealed chronic inflammatory cell infiltration and giant cells surrounded by CPPD. The patient was diagnosed with rapid destructive arthropathy (RDA). Endoscopic shoulder joint debridement was performed. Postoperatively, active flexion …

AIM: We aimed to study luxatio erecta humeri using advanced imaging modalities.
METHOD: Patients with luxatio erecta humeri and a subsequent magnetic resonance imaging (MRI) and/or computed tomography (CT) scan were included in this study.
RESULTS: Among 10 identified cases, we detected 2 rotator cuff, 4 labral, and 2 inferior glenohumeral ligament tears as well as 2 glenohumeral cartilage defects. We observed six comminuted displaced greater tuberosity fractures, four anterior inferior glenoid fractures, and four impaction fractures of humeral head.
CONCLUSION: This study provides detailed…

Roentgenographic evidence of bone and soft tissue abnormalities may be noted in the upper extremities of diabetics. Major shoulder changes, typically associated with peripheral neuropathy, include humeral head deformity due to bone resorption, joint space narrowing, subchondral cysts and sclerosis, subluxation, and juxtra-articular soft tissue bone fragments. Charcot-type joints, characterized by severe joint destruction, sclerosis, multiple bone fragments, and soft tissue swelling may occur. Diabetic neuropathy can produce flexion contractures of the hand. Abnormalities not necessarily associ…

The posterior instability of the shoulder is a more difficult diagnostic and therapeutic challenge than the anterior instability. There are many etiologies and causes of posterior instability. Most studies in the literature are retrospective and yield a great variation in therapeutic recommendations. Generally it has to be separated in traumatic and atraumatic instabilities. Most of the traumatic dislocations are impaction fractures of the humeral head against the dorsal glenoid. Therapy is depending on the size of the humeral defect, the duration of dislocation and the functional demand of th…

OBJECTIVE: The aim of surgical stabilization of proximal humerus fractures is to restore the anatomical relations between the greater and lesser tubercle, to reconstruct the joint and preserve the vascular supply of the fragments.
INDICATIONS: Approximately 80% of proximal humeral fractures can be treated conservatively. Surgical treatment is indicated based on the fracture pattern, patient-related factors and the risk of avascular head necrosis. Two-part fractures with a metaphyseal comminution zone and 3/4-part fractures can benefit from near to anatomic reconstruction depending on the pati

INTRODUCTION: The purpose of this study was to investigate whether a safe zone rule could be applied to prevent iatrogenic injuries to the radial nerve (RN); and determine whether there is a relationship between the diameter of the radial head and capitellum and the distance of the posterior interosseous nerve (PIN) to the radiocapitellar joint.
METHODS: Ten fresh-frozen cadaveric specimens were used to measure the distances between the RN and the lateral epicondyle; the PIN and the radiocapitellar joint; the lateral epicondyle and the PIN as it crossed the ulnohumeral joint; the diameter of

A 22-year-old African male with known sickle cell anaemia was referred by a Congolese medical centre with a request to improve his poor physical condition. He was unable to walk, stand or sit because his large joints and his spine were either ankylosed or very rigid. Radiographs showed joint fusion from the third to the fifth cervical vertebrae, of both hips, of the left knee, and a bilateral osteonecrosis of the humeral head. There was no scintigraphic evidence for an active osteomyelitis (99mTc-MDP (methyldiphosphonate) bone scan, Tc monoclonal antigranulocyte scan and 99mTc sulphur colloid …

Subscapularis tenotomy for anatomic total shoulder arthroplasty has been the standard approach for shoulder surgeons that use the deltopectoral approach. The risk of subscapularis insufficiency after this approach has been well documented. In order to avoid subscapularis complications, subscapularis sparing approaches through the rotator interval have been developed. We present two alternative subscapularis preserving techniques that are performed through the deltopectoral interval and allow more complete osteophyte excision and accurate humeral head sizing. These techniques require modified i…
