BACKGROUND: The extent of intentional or accidental subcutaneous and intramuscular injections and the factors associated with these have rarely been studied among people who inject drugs, yet these may play an important role in the acquisition bacterial infections. This study describes the extent of these, and in particular the factors and harms associated with accidental subcutaneous and intramuscular injections (i.e. 'missed hits').
METHODS: People who inject drugs were recruited using respondent driven sampling. Weighted data was examined using bivariate analyses and logistic regression.

Metamphocin (methyl ether of amphotericin B), a new antifungal polyene antibiotic was used in the treatment of ocular mycoses. The procedure and results of its use are described. The clinical trials included 8 patients with keratomycosis and 2 patients with fungal endophthalmitis. Metamphocin was administered as drop-like intravenous injections in combination with local instillation of ketoconazole and dexamethasol. The clinical trials showed that metamphocin was an efficient antifungal drug useful in the treatment of ocular mycoses. Metamphocin proved to be less toxic than amphotericin B.

Arthritis caused by Sporothrix schenckii may not respond satisfactorily to a full course of intravenous amphotericin B therapy. Left untreated, the fungus continues to be recovered from cultures of joint fluid, and the patient typically has serious joint disability. We have shown in one patient with sporotrichosis of the knee that direct low-dose injections of amphotericin B can be performed safely, resulting in eradication of the fungus. The patient has had continued useful range of motion and weight bearing on the involved knee.

DSA is not suitable for the investigation of hepatic and splanchnic vessels by intravenous or non-selective arterial injections. On the other hand, its use in selective arterial injection offers many advantages compared with conventional arteriography. It can be particularly recommended for routine use in the pre-operative diagnosis of abdominal masses. In some cases additional conventional angiography may also be required and it is therefore desirable to perform both examinations at one unit in order to profit from the advantages of each technique.

A 30-year-old male taxi driver developed extensive purpuric necrotic lesions all over his body. He had a consolidated shadow in the right upper lung field, proteinuria with microscopic hematuria, and granulomatous change in the nasal mucosa. The standard regimen of prednisolone alone or in combination with cyclophosphamide showed only a limited effect on the lesion. However, weekly intravenous injections of a high dose of cyclophosphamide (500 mg/week) in combination with 60 mg/day of prednisolone exhibited dramatical suppression of the disease activity.

In five patients with subacute sclerosing panencephalitis (SSPE), human leukocyte interferon (IFN) therapy was tried. IFN was administered intramuscularly, intravenously, and intrathecally.
