We describe a patient with cholecystoduodenal fistula treated by a laparoscopic approach. Use of a flexible videoscope, flexible retractor, and endoscopic transecting stapler allows for laparoscopic treatment of cholecystoenteric fistulae.

Umbilical anomalies arise from fetal structures such as the omphalomesenteric duct (OMD) or urachus or from failure of closure of the umbilical fascial ring. Persistence of the OMD may lead to several anomalies including umbilical sinus, umbilical cyst, Meckel's diverticulum, or patent OMD (POMD). A POMD is usually associated with the ileum, but rarely may be with the caecum or appendix. We describe a POMD of the vermiform appendix and discuss the possible pathogenesis and management.

Aortoenteric fistula is a rare complication of abdominal trauma. We present a case of a patient with multiple gunshot wounds to the abdomen and thorax in whom control of injury required staged operations. An aortoenteric fistula developed, presenting a diagnostic and therapeutic challenge. The operative control of aortic bleeding was by a retroperitoneal approach to the aorta and facilitated by the use of percutaneous aortic balloon catheters. The patient survived to leave hospital. This case highlights a rare trauma complication, the use of "damage control" for the severely injured abdomen an

When the clinical manifestations of colovesical fistulae lack diagnostic features, significant diagnostic delay may result. The literature contains conflicting recommendations regarding the optimal method of imaging such fistulae. We describe the transabdominal ultrasound appearances of a colovesical fistula and review the relevant literature. The identification of the ureteric orifices with their associated urinary jets and the use of abdominal compression are essential components of the technique.

Acute appendicitis, particularly pelvic, may cause a suppurative process that spontaneously drains to the bladder and an appendicovesical fistula may result. The pelvic appendix and the bladder may occasionally become fused in an inflammatory and necrotic focus, directly creating a fistula. A case of vesicointestinal fistula secondary to appendiceal pelvic peritonitis is described and the cases reported in the literature are reviewed.

From 1984 to 1988, 173 patients with 190 external gastrointestinal fistulae and given total parenteral nutrition (TPN) were reviewed. The advent of TPN has enabled the patients to be operated in a better condition under a well-planned procedure, and has therefore modified the principle of surgical intervention. However, no significant improvement has been noted in the mortality rate and spontaneous closure rate as compared with the years without TPN. The diseases have changed in pattern to afflict older population and increase the incidence of complicated fistula because of the more sophistica…

Fistula formation between a segment of colon and the uterus is an unusual complication of diverticulitis; only 17 cases have been reported in the world literature. We describe a 69-year-old woman with a colouterine fistula secondary to diverticulitis. She presented with a malodorous vaginal discharge that grew multiple enteric organisms on culture. A barium enema revealed colonic diverticula but no fistula tract. Orally administered activated charcoal was seen flowing from the cervical os during a pelvic examination the following day, establishing a diagnosis of colouterine fistula. Pathologic

A rare case of cholecysto-duodenal-colic fistula presenting with non-faeculent vomiting, diarrhoea and loss of weight and appetite is reported. This fistula was demonstrated both by barium and endoscopic studies. She was treated with a simple closure of the fistulous tract and a cholecystectomy with good results. The surgical management of this condition is simple and rewarding.

Sixteen patients were treated for gastrocolic fistula arising as a complication of peptic ulcer (11 cases), colonic perforation (2), gastric cancer (1), colonic cancer (1) or pancreatitis (1). The predominant symptoms were diarrhoea, weight loss and abdominal pain. Barium meal and barium enema were the most reliable means of diagnosis, and no fistula was gastroscopically demonstrable. A one-stage en bloc resection of the involved gastrocolic region was performed in eight cases. Other operations were simple excision (3), gastric resection with closure of the colonic wall (2) and colectomy with
