Sonographic appearances of medial retinacular complex injury in transient patellar dislocation.

AIM: To describe the sonographic appearances of the medial retinacular (MPFR) complex of the knee in patients with acute and recurrent patellar dislocation. MATERIALS AND METHODS: Thirty patients were scanned within 2-4 weeks of an acute episode of lateral patellar dislocation. Eleven gave a history of recurrent patellar dislocation. Ten patients had examination under anaesthesia with arthroscopy and repair of the injury. The sonographic and operative results were compared. RESULTS: The normal sonographic appearance of the MPFR is described. Of the 10 patients who underwent examination under

Acute dislocation of the patella. A correlative pathoanatomic study.

The objective of our study was to elucidate the characteristic pathoanatomy associated with patellar dislocation and report the preliminary results of early surgical repair. Twenty-three patients with documented patellar dislocation had standard radiographs and a magnetic resonance imaging scan. Intraarticular lesions were evaluated and treated arthroscopically followed by an open exploration of the medial aspect of the knee in 16 patients. Twelve patients were observed for a minimum of 2 years after surgical repair (average, 34 months). Eleven patients returned for a follow-up examination. Ma

Unicompartmental Knee Arthroplasty Has Lower Infection, Conversion, and Complication Rates Compared to High Tibial Osteotomy.

INTRODUCTION: Isolated medial knee osteoarthritis can be surgically treated with either unicompartmental knee arthroplasty (UKA) or high tibial osteotomy (HTO). Proponents of UKA suggest superior survivorship, while HTO offers theoretically improved alignment and joint preservation delaying total knee arthroplasty (TKA). Therefore, we compared complications in a large population of patients undergoing UKAs or HTOs. We specifically assessed 90 days, 1 year, and 2 years: (1) periprosthetic joint infection (PJI) rates, (2) conversion to TKA rates, as well as (3) complication rates. METHODS: A re

Four-in-one Extensor Realignment for the Treatment of Obligatory or Fixed, Lateral Patellar Instability in Skeletally Immature Knee.

BACKGROUND: Congenital (fixed) and obligatory (habitual) patellar dislocations in children are a complex clinical and surgical challenge. Numerous individual surgical techniques have been described. This study aims to assess results, patient satisfaction, and complications after a combined Roux-Goldthwait procedure, vastus medialis obliquus advancement, Galeazzi procedure, and extensive, lateral release (4-in-1 extensor realignment) in the skeletally immature knee with obligatory, or fixed lateral patellar instability. METHODS: Records of children with congenital fixed or obligatory patellar

Patellofemoral chondral status after medial patellofemoral ligament reconstruction using second-look arthroscopy in patients with recurrent patellar dislocation.

BACKGROUND: Most patients with recurrent patellar dislocation show cartilage damage in the patellofemoral joint. Medial patellofemoral ligament reconstruction has become one of the most important surgical techniques for treating recurrent patellar dislocation. However, patellofemoral chondral status after this reconstruction has not been elucidated. The purpose of this study was to investigate the effects of medial patellofemoral ligament reconstruction on articular cartilage in the patellofemoral joint by comparing the arthroscopic chondral status at the time of reconstruction with that at se

Tibial tubercle-posterior cruciate ligament distance: a new measurement to define the position of the tibial tubercle in patients with patellar dislocation.

BACKGROUND: In patients with patellar instability, a pathological tibial tubercle-trochlear groove (TT-TG) distance is a risk factor. However, the TT-TG distance gives no information about the location of the malformation. HYPOTHESIS: Not all patients with a pathological TT-TG distance (≥20 mm) had lateralization of the tibial tubercle. STUDY DESIGN: Cohort study (diagnosis); Level of evidence, 2. METHODS: Fifty-eight knees in 49 patients with 2 or more patellar dislocations and 60 knees in 30 volunteers with no history of dislocation were analyzed using magnetic resonance imaging (MRI). Th

Epidemiology of painful knee after total knee arthroplasty in a tertiary care center: Assessment by decision tree.

BACKGROUND: Painful knee after arthroplasty concerns up to 21% of patients at six months. We aimed to evaluate: the application of a decision tree to explain painful knee after total knee arthroplasty (TKA), the rate of unexplained pain after complete algorithmic screening. The aim of the study was to evaluate the causes of painful TKA. Our hypothesis was that it is possible to find the cause of the pain in more than 90% of cases. METHODS: A single-center retrospective study analyzed all 1130 consultations between 1 April 2017 and 31 July 2018. We included all patients consulting for unexplai

Elmslie-Trillat procedure for the treatment of recurrent patellar instability.

PURPOSE: The purpose of this study was to evaluate the mid-term results of the Elmslie-Trillat procedure for the treatment of patella instability. Patella instability can be addressed by proximal, distal, and combined approaches. The Elmslie-Trillat procedure is a combined bony distal realignment with medial soft tissue technique offering a more rapid recovery than more extensive distal realignments. METHODS: A consecutive series of patients with recurrent patellar instability treated with a modified Elmslie-Trillat procedure were assessed subjectively by visual analog scale, Tegner scale, an

[Quadriceps dysplasia and patellar tilt in objective patellar instability].

PURPOSE OF THE STUDY: Dysplasia of the vastus medialis, a well-known landmark of patellar instability, is difficult to point out pre-operatively. We propose the measurement of patellar tilt on the CT scans in order to appreciate abnormality. MATERIAL AND METHODS: We studied 3 well defined groups: 143 knees operated on for a true dislocation of the patella, either for the first time or a recurrent episode, 67 asymptomatic and nonoperated contralateral knees and 54 control knees. The patellar tilt in extension was measured on the CT-scan with the quadriceps relaxed and contracted. RESULTS: The

The role of body mass index in high- and low-velocity trauma causing knee injury associated with popliteal artery lesions.

BACKGROUND: Among arterial traumas, osteoarticular traumas are particularly dangerous, and those involving the popliteal artery are associated with a high amputation rate. Despite representing a minority of arterial traumas, with an incidence that varies considerably by population and geographic location, traumatic lesions of the popliteal artery are challenging. This study aimed to verify the impact of body mass index (BMI) on arterial trauma damage and patient outcomes. METHODS: Data were retrospectively collected from the electronic medical reports of all patients with osteoarticular and v

[Diagnosis and treatment of trauma to blood vessels around the knee joint (author's transl)].

Eighteen arterial reconstructions after trauma in the region of the knee joint were performed between 1970 and 1976. In seven cases there had been fracture of the distal femur, in five of the head of the tibia, in three dislocation of the knee joint or severe contusion of the popliteal artery. In 12 cases referral had been eight hours or later after injury. The concomitant vessel injury was not recognised in six. Vascular reconstruction by direct suture with a venous patch was performed in eight, interposition of a venous autograft in five, and saphenous vein bypass in four. Results depended u

Distal Femoral Osteotomy and Medial Patellofemoral Ligament Reconstruction for Patellar Dislocation in Hereditary Multiple Exostoses: A Case Report.

CASE: A 44-year-old man with hereditary multiple exostoses (HME) presented with recurrent patellar dislocation of the left knee. Radiographs revealed valgus deformity and trochlear dysplasia. The patient underwent lateral opening-wedge distal femoral osteotomy with medial patellofemoral ligament reconstruction using FiberTape and knotless SwiveLock anchors. Postoperatively, patellar tracking normalized with no recurrence. CONCLUSIONS: This case highlights the need to address bony alignment and medial soft-tissue insufficiency concurrently in complex patellofemoral instability, particularly in

Distal realignment and medial patellofemoral complex reconstruction for the correction of patellofemoral instability in a patient with transtibial amputation.

Patellofemoral instability (PFI) is a multifactorial condition typically observed following initial traumatic patellar dislocation. PFI depends on various factors such as limb alignment, bony structure, and the integrity of static and dynamic stabilizers. Patients with below-knee amputation have a higher risk of experiencing PFI. This report describes a successful case involving a patient with patellofemoral instability and a transtibial amputation. The patient was effectively treated with distal realignment and patellar stabilization achieved by reconstructing the medial patellofemoral comple

Isolated medial patellofemoral reconstruction outcomes: A systematic review and meta-analysis.

BACKGROUND: Treatment of patellar instability remains up for debate, and a combination of tibial tubercle osteotomy and medial patellofemoral ligament reconstruction (MPFLr) of the medial patellofemoral ligament (MPFL) has become the mainstay treatment for recurrent lateral patellar dislocation. Due to limited small studies, there remains a variety of surgical techniques still being practiced. The use of MPFL reconstruction, in isolation, has demonstrated promise. PURPOSE: The purpose of this systematic review and meta-analysis is to investigate if isolated medial patellofemoral ligament reco
