Pyelo-hepatic abscess is a rare complication of upper urinary tract infections (UTIs). We describe a case of polymicrobial pyelo-hepatic abscess in an immunocompetent patient. A 71-year-old male patient with a double-J stent for right ureteral lithiasis was admitted in our Infectious Diseases Department for a pyelo-hepatic abscess. Despite a targeted antibiotic therapy against an extended spectrum betalactamase-negative Escherichia coli, the patient did not improve. Further examinations revealed a possible polymicrobial aetiology, including Candida spp. and E. coli resistant to piperacillin/ta…

Context: Ketoprofen is widely used to remove pain. A steady increase on allergic reactions and photoallergic contact dermatitis related to ketoprofen has been reported when there is topical use. However, there are few documented cases of hypersensitivity when it is administered systemically. Objective: Present a case of hypersensitivity reaction after systemic administration of ketoprofen for pain control in nephritis crisis.Case description: A 43-years-old Caucasian man diagnosed with renal colic (kidney lithiasis), who was initially treated with 100 mg of tramadol (IV), followed by 4 mg of th…

INTRODUCTION: Dieulafoy lesion of the gallbladder is very rare. It can cause hemobilia and, consequently, acute pancreatitis. After extensive research, the authors found six reported cases of this entity, none of them presenting as acute pancreatitis.
CASE PRESENTATION: A 78-year-old woman presented to the emergency department a right upper-quadrant pain and nausea. Abdominal ultrasound showed a gallbladder full of a heterogenic content with no signs of lithiasis or acute cholecystitis. Blood lipase was significantly elevated. She was admitted for acute pancreatitis. During hospitalization sh…

BACKGROUND: Extracorporeal shock wave lithotripsy (ESWL) was first introduced in paediatric population in 1986. Given the more frequent recurrence in children, compared to adults, urinary stones treatments should require minimal invasive treatment methods. In this study, we aimed to evaluate the profile of the young patient with lithiasis who can benefit from ESWL, analysing the experience of 2 clinical departments.
MATERIALS AND METHODS: We have retrospectively reviewed the medical records of 54 children who underwent ESWL for urolithiasis. ESWL success rate was defined as stone-free status …

Down syndrome is the most common human chromosomal disorder. Among clinical findings, one constant concern is the high prevalence of gastroi … gia that resolved before the second year of life. Peptic ulcer disease, celiac disease, and biliary lithiasis were less prevalent with 3% each. Awareness of the high prevalence of gastrointestinal disorders promotes outstanding clinical follow-up as well as adequate development and greater quality of life for patients with Down syndrome and their families.

Myiasis is caused by the larval stages of flies feeding on live or dead mammalian tissues and fluids. Human cases involving the genitourinary system are very rare and present with unusual symptoms. We report the case of a male, 25-year-old shepherd who underwent a surgical intervention on his urinary tract for renal lithiasis and was catheterized with a drain dwelling in his left kidney. A few days after the patient showed signs of dysuria, polyuria and pollakisuria. Lower urinary tract infection with ciprofloxacin susceptible E. coli was diagnosed and treated accordingly. The symptoms persiste…

Somatostatinoma are rare well-differentiated endocrine tumors with malignant behavior arising from the pancreas and duodenum. They are defined by somatostatin positive immunostaining of the majority of tumor cells. The main clinical features are diabetes, diarrhea and biliary lithiasis related to somatostatin production. Somatostatinoma secreting both calcitonin and somatostatin may be unrecognized as a small number of such observations have been published. We report the case of a 57- year-old woman referred for weight loss, diarrhea and worsening diabetes. Computer tomography scan revealed mu…

We found pathological fractures and rachitism in two, bone rarefaction and delay of bone age in all of the patients. One patient presented a reduction in the rhythm of glomerular filtration. BACKGROUND: Cholesterolosis is frequently observed in cholecystectomies performed for lithiasis or chronic cholecystitis.
AIM: To determine the degree of association between cholesterolosis and gallbladder cancer.
MATERIAL AND METHODS: In a prospective study of gallbladder cancer, all gallbladders obtained during cholecystectomies were processed for pathological study, following a special protocol. As part of this study, 23,304 surgical samples obtained between 1993 and 2002 were studied, looking for a relationship between cholesterolosis and chronic cholecystitis, adenomas, dysplasia and ga…

BACKGROUND: Acute cholecystitis is the major complication of biliary lithiasis, for which laparoscopic treatment has been established as the standard therapy. With longer life expectancy, acute cholecystitis has often been seen in elderly patients (>65 years old) and is often accompanied by comorbity and severe complications. We sought to compare the outcome of laparoscopic treatment for acute cholecystitis with special focus on comparison between elderly and nonelderly patients.
METHOD: This study was a prospective analysis of 190 patients who underwent laparoscopic cholecystectomy due to ac…

The excision of the choledochal cyst with bile drainage through intestinal conduit is a standard operative procedure in the surgical managem … ere without abnormalities. The routine control which followed did not show episodes of cholangitis, lithiasis, lipid malabsorption, blood clotting abnormalities or growth failure. The complete excision of the cyst with Roux-Y hepaticoenterostomy is an operative treatment with good results in infancy and childhood.

BACKGROUND: Appendicoliths are recognized risk factors for complicated appendicitis and adverse surgical outcomes. However, current preoperative assessments lack a standardized method to evaluate appendicoliths beyond simply documenting their presence. This study aimed to develop and validate a CT-based appendicolith scoring system incorporating both location and morphological characteristics to predict postoperative outcomes in patients with acute appendicitis.
METHODS: We retrospectively analyzed 241 adult patients with acute appendicitis and radiologically confirmed to have appendicoliths

OBJECTIVE: Acute appendicitis is a common presentation in most emergency departments, and the presence of appendiceal fecolith on CT is indicative of earlier progression to complicated disease. This study aimed to assess the association between appendiceal fecoliths identified on preoperative computed tomography (CT) and the severity of appendicitis, using integrated clinical, radiological, surgical, and histopathological data.
METHODS: We conducted a retrospective observational study of 529 adult appendicectomy patients diagnosed with acute appendicitis via CT between 2010 and 2020 at two te

BACKGROUND: Residual and recurrent stones remain one of the most important challenges of hepatolithiasis and are reported in 20 to 50% of patients treated for this condition. To date, the two most common surgical procedures performed for hepatolithiasis are choledochojejunostomy and T-tube drainage for biliary drainage. The goal of the present study was to evaluate the therapeutic safety and perioperative and long-term outcomes of choledochojejunostomy versus T-tube drainage for hepatolithiasis patients with sphincter of Oddi laxity (SOL).
