Socioeconomic, institutional, environmental, and demographic determinants of population health in 90 developing countries: panel data evidence.

Anchored in Grossman's health production framework, this study examines how socioeconomic, institutional, environmental, and demographic conditions are associated with population health in developing countries. Population health is measured through three complementary indicators-life expectancy, infant mortality, and maternal mortality-to capture broader survival conditions, early-life vulnerability, and maternal health risks. Using panel data for 90 developing countries from 2002 to 2018, the study estimates country fixed-effects models and uses Driscoll-Kraay standard errors to strengthen in…

Neonatal outcomes according to different degrees of maternal morbidity: cross-sectional evidence from the Perinatal Information System (SIP) of the CLAP network.

BACKGROUND: The burden of maternal morbidity in neonatal outcomes can vary with the adequacy of healthcare provision and tool implementation to improve monitoring. Such information is lacking in Latin American countries, where the decrease in severe maternal morbidity and maternal death remains challenging. OBJECTIVES: To determine neonatal outcomes according to maternal characteristics, including different degrees of maternal morbidity in Latin American health facilities. METHODS: This is a secondary cross-sectional analysis of the Perinatal Information System (SIP) database from eight heal

Incidence, risk factors, and associated complications of eclampsia.

OBJECTIVE: To estimate trends in incidence and identify risk factors and maternal and neonatal consequences of eclampsia in Canada. METHODS: We conducted a population-based cohort study of all women and their newborns (N=1,910,729) delivered in the hospital in Canada (excluding Quebec) from 2003 to 2009. The data were obtained from the Canadian Institute for Health Information. Logistic models were used to examine the association with potential determinants and consequences of eclampsia. RESULTS: The incidence of eclampsia declined dramatically from 12.4 per 10,000 deliveries in 2003 to 5.9

Treatment cost and costing model of obstetric complications at a hospital in Myanmar.

Maternal health still remains a major challenge in almost all developing countries. In Myanmar, the country met only 62% of its target for the maternal mortality rate (130 per 100,000 live birth) even though proportion of skilled birth attendant (SBA) and antenatal care (ANC) coverage was 80% in 2015. Despite the estimated large maternal complications, most maternal healthcare program ignored the burden of those morbidity because of limited understanding of the incidence and prevalence of morbidity conditions and cost of those morbidity burdens on society. The present study provides a general …

Fetomaternal Outcome in Eclampsia in Relationship with Gravidity.

Eclampsia is a well-recognized major cause of maternal and perinatal morbidity and mortality. Patient's age, occupational status, socioeconomic status, educational status, regular ANC (antenatal care) and gravidity may affect the outcome of mother and foetus. The purpose of this study is to see the fetomaternal outcome in eclampsia in relationship with gravidity. This prospective observational cross-sectional study carried out among the alternate cases of primigravid and multigravid eclamptic patients. Total number of patients was 100 eclamptic patients among them, 50 patients were primigravid

Analysis of emergency peripartum hysterectomy in Northern Tasmania.

OBJECTIVE: There is a need for emerging Australian data on emergency peripartum hysterectomy (EPH) especially in rural areas due to the associated high maternal morbidity and mortality. The aim of this study is to review the incidence and complications of EPH in the northern region of Tasmania. DESIGN: A retrospective cohort study at a single health care institution during a 10 year period. SETTING: Launceston General Hospital, the main maternity referral centre for the northern region of Tasmania. PARTICIPANTS: Case notes of women coded with hysterectomy during childbirth were included and

Tick-borne relapsing fever and pregnancy outcome in rural Tanzania.

OBJECTIVE: To assess the impact of tick-borne relapsing fever (TBRF) on the outcome of pregnancy. DESIGN: Case control study of 137 pregnant women (cases) and 120 non-pregnant women (controls) with TBRF between 1985 and 1995. SETTING: A rural hospital in Tabora Region, Tanzania. RESULTS: Risk of birth during the attack of TBRF was 58.0%, with an extremely high perinatal mortality of 436 per 1000 births. The total loss of pregnancies including abortions was 475. per 1000. Case-fatality rate in pregnant women was 1.5%, compared to 1.7% in the non-pregnant women. A Jarisch-Herxheimer reaction

Peer review through perinatal statistics.

The purpose here is to demonstrate that perinatal statistics serve as a powerful tool for peer review, thereby improving the standard of maternity care. These data were used as an instrument for peer review, first by comparing perinatal mortality between one country and another. More detailed statistics, rapidly fed back to the delivery units, acted as a strong inducement to practitioners at all levels who wanted to do at least as well as their neighbors. Examples are given from Sweden, Scotland and Germany.

Management of sickle cell disease in pregnant patients.

Data from a total of 74 pregnancies in 42 patients with sickling disorders seen at Johns Hopkins Hospital are compared with similar data from other centers. Although risks are still higher than those for women without sickle cell disease, they have diminished significantly from those reported earlier. Prophylactic transfusion therapy may decrease these risks further, but benefits of transfusion are unproved at present. Patients with sickling disorders should receive meticulous individualized treatment until further data are made available.

Addressing Health Care Gaps During the Fourth Trimester and Through One Year Postpartum.

BACKGROUND: Maternal mortality in the United States has risen over the past three decades, with over half of deaths occurring between one week and one year postpartum. Despite this, most clinical guidelines and care-delivery models focus on the initial 12 weeks after birth. NARRATIVE: This narrative review examines barriers to care beyond the fourth trimester and highlights policy, health-system, and clinical strategies for optimizing maternal health across the first postpartum year, recognizing that structural challenges-including insurance loss, workforce shortages, and adverse social deter…

Characteristics and preventability of obstetric intensive care unit admissions in Far North Queensland.

BACKGROUND: The rarity of maternal deaths in developed countries has increased interest in auditing cases of severe maternal morbidity or maternal 'near miss'. The assessment and preventability of cases of maternal 'near misses' are important in improving the provision of maternity care. AIMS: To describe the epidemiology and determine aspects of preventability in care among women admitted to an obstetric intensive care unit (ICU) in Far North Queensland. MATERIALS AND METHODS: A retrospective observational study of pregnant and postpartum patients admitted to the Cairns Hospital ICU between
