Editorial Commentary: Too Many Questions Remain to Consider Slope-Correcting Osteotomy for Most Primary Anterior Cruciate Ligament Reconstructions.

Anterior cruciate ligament (ACL) reconstructions and meniscus repairs can have unacceptably high retear rates among certain cohorts of young athletes. Studies have showed that increased posterior tibial slopes are associated with increased risk of ACL and meniscus retears. Increasing evidence suggests that slope-correcting osteotomy can decrease retear rates following ACL reconstructions and meniscus tears in certain populations. However, preventing ACL graft failures and meniscus retears at all costs should not be the only consideration: risk of major complications and reoperations, ability t…

Dynamics of liquid rise in the interstices of circular capillaries under a non-inertial regime.

Capillary rise of liquids in confined geometries plays a vital role in natural and engineered systems such as microfluidic devices, porous m … minant, we use Onsager's principle of energy minimization and propose that the dimensionless corner meniscus height in later times must scale as the cubic root of the dimensionless time with a novel prefactor for the circular profiles. This result differs from earlier findings for general quadratic profile corners. The corresponding dimensionless velocity …

Studies of tibial subchondral bone density and its significance.

We examined the relationships between the density of the subchondral bone of the proximal end of the tibia and the location and condition of … llation of articular cartilage was observed only in the articular cartilage that was not covered by meniscus and lying mesial to it, which supports the concept that the meniscus confers a protective effect on the articular cartilage. The density of the bone underlying the meniscus was lower, and was inversely related to the thickness of the meniscus and di …

Spontaneous Recycling of Electrosprayed Sample by Retrograde Motion of Microdroplets.

Here, we discuss an interesting phenomenon occurring spontaneously near the sample liquid meniscus at the tip of the electrospray emitter. While most ejected droplets move from the emitter tip toward the counter electrode, some of the droplets decelerate and move backward to the liquid meniscus. When they hit the surface of the liquid meniscus, they either merge with the bulk liquid or get recharged during intermittent contact with the liquid meniscus and immediately reaccelerate toward the counter electrode. In some cases, while in contact with the meniscus they spontaneously form a secondary…

Updates in biological therapies for knee injuries: menisci.

The preservation of meniscal tissue is paramount for long-term joint function, especially in younger patients who are athletically active. Many studies have reported encouraging results following the repair of meniscus tears, including both simple longitudinal tears located in the periphery and complex multiplanar tears that extend into the central third avascular region. However, most types of meniscal lesions are managed with a partial meniscectomy. Options to restore the meniscus range from an allograft transplantation to the use of synthetic and biological technologies. Recent studies have…

The structural and compositional transition of the meniscal roots into the fibrocartilage of the menisci.

The meniscal roots, or insertional ligaments, firmly attach the menisci to tibial plateau. These strong attachments anchor the menisci and allow for the generation of hoop stress in the tissue. The meniscal roots have a ligament-like structure that transitions into the fibrocartilagenous structure of the meniscal body. The purpose of this study was to carry out a complete analysis of the structure and tissue organization from the body of the meniscus through the transition region and into the insertional roots. Serial sections were obtained from the meniscal roots into the meniscal body in fix…

Predicting risk of unplanned subsequent knee surgery following ACL reconstruction.

BACKGROUND: Following anterior cruciate ligament reconstruction (ACLR), unplanned subsequent knee surgeries are disruptive and costly. The purpose of this study was to identify risk factors associated with any subsequent knee surgery as well as specific surgical procedures, including revision, contralateral ACLR, surgery for meniscus injury, or procedure for loss of motion following primary ACLR using data from the electronic health record (EHR) of a large single-system health network. METHODS: Data for individuals (≥14 years old) who underwent primary ACLR between January 2013 and June 2021 …

Incidence of meniscal tears in skeletally immature patients with anterior cruciate ligament (ACL) rupture: Medial tears are more common in patients closer to skeletal maturity while lateral tears in those with more remaining growth.

PURPOSE: To report the prevalence of meniscal tears observed in patients with open physis and anterior cruciate ligament (ACL) rupture and to examine the relationship between meniscus tears and remaining growth. METHODS: Skeletally immature patients who underwent ACL reconstruction between June 2022 and June 2024 in a single institute were included. Patients were considered skeletally immature if the pre-operative knee magnetic resonance imaging (MRI) revealed an open physis. MRI was used to determine the bone age of the patients, and the patients were divided into two groups as >1-year and <…

Does Accelerated Rehabilitation Provide Better Outcomes Than Restricted Rehabilitation in Postarthroscopic Repair of Meniscal Injury?

CONTEXT: Meniscal injury is a common pathology, and the postoperative rehabilitation program is essential to patients after surgery. However, the optimal rehabilitation plan after meniscus suture is still controversial. OBJECTIVE: To compare the clinical outcomes between accelerated rehabilitation and restricted programs in patients with meniscus suture (with or without anterior cruciate ligament reconstruction, ACLR). EVIDENCE ACQUISITION: Four databases, including PubMed, Ovid, Embase, and the Cochrane Library, were searched up to November 2021. This study only included studies comparing t…

Tear evaluation by anterior segment OCT in dry eye disease.

Objective. Dry Eye Disease (DED) is a multifactorial disorder, centered by loss of tear homeostasis. The diagnosis represents a challenge in the absence of a gold standard, so different questionnaires and techniques are combined. Considering that a low lacrimal secretion and a high rate of evaporation can determine changes in the tear film, the question that arises is if there are differences between the tear film thickness and the tear meniscus values of patients with DED compared to healthy volunteers, and if so, if they can be proposed as an objective diagnosis technique using Optical Coher…

Partial medial meniscectomy leads to altered walking mechanics two years after anterior cruciate ligament reconstruction: Meniscal repair does not.

BACKGROUND: Partial meniscectomy dramatically increases the risk for post-traumatic, tibiofemoral osteoarthritis after anterior cruciate ligament reconstruction (ACLR). Concomitant medial meniscus surgery influences walking biomechanics (e.g., medial tibiofemoral joint loading) early after ACLR; whether medial meniscus surgery continues to influence walking biomechanics two years after ACLR is unknown. RESEARCH QUESTION: Does medial meniscus treatment at the time of ACLR influence walking biomechanics two years after surgery? METHODS: This is a secondary analysis of prospectively collected d…

Arthroscopic Meniscopexy for the Treatment of Nontraumatic Osteochondritis Dissecans in the Knee Joint of Adult Patients.

OBJECTIVE: The etiology of osteochondritis dissecans (OCD) is still uncertain. Recently, it has been hypothesized that instability of the anterior horn of the meniscus is an important cause for the development of a nontraumatic OCD in adolescents with good results after surgical stabilization. This case series aims to evaluate the treatment results after meniscus stabilization in adults with nontraumatic OCD. DESIGN: Ten patients with magnetic resonance imaging (MRI)-confirmed OCD of the knee joint, meniscal instability, and closed epiphyseal plates were enrolled in this study. The instable m…

[Causes of rearthrotomies of the knee joint].

Report on 178 rearthrotomies of the knee joint in 158 patients. Most of these rearthrotomies became necessary subsequent to meniscus operations. A review of the literature shows that these are mainly persisting or recurring complaints appearing usually under the overall pattern of "irritable knee". The principal causes were found to be leftover meniscus residues; on rarer occasions degenerative meniscus regenerates were found, or other causes such as floating cartilage, damaged cartilage of the patella, and instabilities of the condyles and of the ligaments.

Symptomatic bilateral discoid menisci in children: a comparison with unilaterally symptomatic patients.

BACKGROUND: In previous studies, 5% to 20% of patients with a discoid lateral meniscus eventually require surgery bilaterally for symptomatic discoid menisci. However, there are little published data specifically on children who require treatment for discoid menisci in both knees. The purpose of this study is to identify differences in clinical and arthroscopic findings between children who require bilateral versus unilateral treatment for symptomatic discoid lateral menisci. METHODS: We retrospectively reviewed the records of all patients aged 18 years or younger requiring treatment of disco…

Successful anterior cruciate ligament reconstruction and meniscal repair in osteogenesis imperfecta.

A case of anterior cruciate ligament (ACL) reconstruction with meniscal repair in an osteogenesis imperfecta patient is reported. A 24-year-old female with osteogenesis imperfecta type 1a suffered from a valgus extension injury resulting in tear of ACL and medial meniscus. She underwent an arthroscopic-assisted ACL reconstruction and medial meniscus repair. Meniscal tear at the menisco-capsular junction of the posterior horn of medial meniscus was repaired with three absorbable sutures via inside-out technique. ACL reconstruction was then performed with a bone-patellar tendon-bone allograft. T…

Proteoglycan concentrations in healthy and diseased articular cartilage by Fourier transform infrared imaging and principal component regression.

Fourier transform infrared imaging (FTIRI) combining with principal component regression (PCR) analysis were used to determine the reduction of proteoglycan (PG) in articular cartilage after the transection of the anterior cruciate ligament (ACL). A number of canine knee cartilage sections were harvested from the meniscus-covered and meniscus-uncovered medial tibial locations from the control joints, the ACL joints at three time points after the surgery, and their contralateral joints. The PG loss in the ACL cartilage was related positively to the durations after the surgery. The PG loss in th…
