Sarcoidosis, granulomas and myopathy syndromes: A clinical-pathology review.

Muscle involvement in sarcoidosis is common by pathologic analysis, but symptomatic disorders are less frequent. Sarcoidosis-related muscle pathology includes non-caseating granulomas, muscle fiber changes that are diffuse or anatomically related to granulomas, and perimysial connective tissue with histiocyte-associated damage. The mechanisms by which granulomas form, enlarge and damage muscle tissues are incompletely understood. Sarcoidosis-related clinical syndromes with muscle involvement include: chronic myopathies with proximal weakness; nodular disorders; subacute onset disorders involvi

Comparison of diaphragmatic breathing exercises and incentive spirometry on the functional status of heart failure patients: a randomized controlled trial.

BACKGROUND: Respiratory muscle weakness in heart failure (HF) can deteriorate its symptoms such as fatigue, dyspnea, and impaired functional status. Pulmonary rehabilitation can strengthen these muscles. This study aimed to determine the impact of breathing exercises on fatigue severity, dyspnea, and functional classification in HF patients. METHODS: A three-arm single-blind randomized controlled trial was conducted on 90 hospitalized HF patients in three 30-participant groups including diaphragmatic breathing group (DG), incentive spirometry group (SG), and control group (CG). The interventi

The assessment of muscle strain with surface electromyograms during simulated mushroom picking.

Muscle strain was assessed with surface EMG during simulated mushroom picking. Nine female subjects performed five periods of work (W1-W5). The duration of each period was about 20 min. W1, W2 and W3 were separated by a short break of several minutes. W3, W4 and W5 were separated by a rest period of about 43 min. EMGs were recorded from the muscles of trapezius, infraspinatus, deltoid and erector spinae muscles. Amplitude of EMG (AEMG) and mean power frequency (MPF) of EMG were calculated every minute. Ratings of perceived exertion (RPE) were recorded for every 5 min. RPE increased during each

Central and peripheral fatigue: exemplified by multiple sclerosis and myasthenia gravis.

Fatigue is a feature of several chronic diseases of the central and peripheral nervous system. The pathophysiology of central fatigue is complex and often not well-defined. In contrast, peripheral fatigue is more objectively defined and measured. Fatigue can be part of the primary disease process, but there are often contributions from comorbid factors such as depression, sleep disturbance, medication, or deconditioning. Multiple sclerosis (MS) offers an example of central fatigue. More than 40% of MS patients complain of fatigue. Validated questionnaires are used to assess fatigue severity an

Impact of exercise-induced fatigue on the risk of stress fractures in the tibia during smash landing in female badminton players.

This study aimed to investigate the effects of exercise-induced fatigue on the risk of tibial stress fractures by comparing biomechanical parameters during backhand-side smash landings in elite female badminton players before and after fatigue. Thirteen elite female badminton players performed smash-landing trials before and after fatigue. The AnyBody Modeling System was used for inverse dynamics analysis, and finite element analysis was conducted using the AnyFE2Abq plugin to compute tibial stress and strain. After fatigue, the backhand rearcourt jump smash (BRJS) and the backhand lateral jum

The Physicochemical Properties and Anti-Fatigue Efficacy of a Polysaccharide-Rich Fraction Derived From Millettia speciosa Champ.

The root of Millettia speciosa Champ has been traditionally and widely used for anti-fatigue properties in folk culture. However, the active components and underlying mechanisms of M. speciosa's anti-fatigue efficacy are still unclear. Notably, polysaccharides are considered the primary component for anti-fatigue action in many traditional Chinese medicines. This study aimed to characterize the physicochemical properties of a polysaccharide-rich fraction from M. speciosa (MSCP) and investigate its anti-fatigue efficacy and underlying mechanisms. The results showed that MSCP was primarily compo

Effects of additional external weight on posture and movement adaptations to fatigue induced by a repetitive pointing task.

Fatigue and additional weight are risk factors of injuries by falls. Posture and trunk movement changes occur with fatigue induced by a repetitive pointing task. These changes facilitate arm movement but they may also jeopardize postural stability. When equilibrium is challenged, e.g. with additional weight, strategies that represent less postural threat could develop with fatigue. Nineteen participants performed two sessions (without, with 20% body weight added load (Load)) of a repetitive pointing task until shoulder fatigue (8 on Borg CR-10). There was no difference in time to fatigue betwe

Respiratory Muscle Contraction Characteristics and Potential Mechanisms in Severely Burned Rats.

Postburn hypermetabolism remains an important clinical problem. During this phase, there is a significant loss of diaphragmatic proteins. Better understanding of respiratory muscle dynamics and potential mechanisms affecting respiratory muscle function is necessary for the development of effective therapeutic approaches. Male Wistar rats were subjected to 50% TBSA burns and sham injuries, and respiratory muscle function was assessed with 0, 1, 4, 7, and 14 days postinjury, including pulmonary function, blood gas analysis, transdiaphragmatic pressure, diaphragm ultrasonography, isolated diaphra

Strength training and aerobic exercise training for muscle disease.

BACKGROUND: Strength training or aerobic exercise programmes might maximise muscle and cardiorespiratory function and prevent additional disuse atrophy in patients with muscle disease. However, over-exerting might cause more rapid disease progression. OBJECTIVES: To examine the efficacy and safety of strength training and aerobic exercise training in patients with muscle diseases. SEARCH STRATEGY: We searched the Cochrane Neuromuscular Disease Group register (October 2002 and May 2004), the Cochrane Collaboration Rehabilitation and Related Therapies Field register (October 2002), MEDLINE (Ja

Stress fractures in the elderly: different pathogenetic features compared with young patients.

Stress fractures mainly occur in the lower limb as a result of cyclic submaximal stresses. Most commonly affected by this specific type of fractures are young athletes, military or elderly subjects with metabolic bone diseases like osteoporosis. In consideration of the heterogeneity of affected patients is presumable that there are different pathogenic mechanisms. In young person bone tissue, although metabolically intact, is not able to withstand the stresses to which it is chronically subjected, also because of muscle fatigue. This leads to a macrostructural failure and to the development of…

Calcium Fluxes in Work-Related Muscle Disorder: Implications from a Rat Model.

INTRODUCTION: Ca2+ regulatory excitation-contraction coupling properties are key topics of interest in the development of work-related muscle myalgia and may constitute an underlying cause of muscle pain and loss of force generating capacity. METHOD: A well-established rat model of high repetition high force (HRHF) work was used to investigate if such exposure leads to an increase in cytosolic Ca2+ concentration ([Ca2+]i) and changes in sarcoplasmic reticulum (SR) vesicle Ca2+ uptake and release rates. RESULT: Six weeks exposure of rats to HRHF increased indicators of fatigue, pain behaviors

Idiopathic combined adrenocorticotropin and growth hormone deficiency mimicking chronic fatigue syndrome.

A 42-year-old man who had suffered from severe fatigue for 5 years was diagnosed as having chronic fatigue syndrome (CFS) and fibromyalgia. Endocrinological workup using combined anterior pituitary function tests showed that the patient had adrenocorticotropin hormone (ACTH) deficiency, with a normal pituitary MRI. Treatment with a physiologic dose of oral hydrocortisone replacement physically ameliorated his general fatigue. A secondary workup using a growth hormone-releasing peptide-2 test revealed that he also had growth hormone (GH) deficiency, and GH replacement therapy was started. His m

[Intravenous immunoglobulin in postpolio syndrome].

BACKGROUND: Postpolio syndrome is characterised by new muscular weakness, pain, and fatigue several decades after the acute polio, and affects approximately 1/4 of patients with previous paralytic polio. MATERIAL AND METHODS: A 47-year-old woman with a previous history of acute poliomyelitis developed progressive muscular weakness in her left arm and right leg with muscular pain and fatigue. Clinical examination, MRI, and electromyography gave no other explanation to her progressive muscular weakness and fatigue than postpolio syndrome. She was treated with 400 mg/kg immunoglobulin intravenou

Second Episode of Rhabdomyolysis: A Case Report.

Rhabdomyolysis is a serious medical condition resulting from skeletal muscle breakdown, leading to the release of intracellular contents-most notably myoglobin-into the bloodstream. The accumulation of myoglobin can result in acute kidney injury and electrolyte disturbances. Common etiologies include trauma, strenuous physical activity, infections, genetic predisposition, and certain medications. Clinically, patients may present with muscle pain, weakness, fatigue, dark-colored urine, and decreased urine output. Creatine kinase, an enzyme predominantly located in skeletal muscle, serves as a h
