We report a case of lipoid pneumonia complicated by supra-infection with Mycobacterium fortuitum. The diagnosis of a lipoid pneumonia was suggested by the CT Scanner which showed a negative density in the pneumonic tissue. Since some previous studies have reported elevated total serum IgE levels in leprosy, that may be associated with the existence of a state of generalized T cell anergy, we undertook a carefully controlled study of this immunoglobulin in such patients before and after treatment with a Mycobacterium leprae-BCG vaccine. A rare case of hepatic granulomatosis due to Mycobacterium scrofulaceum is presented. The prolonged clinical course before liver biopsy was characterized by a disproportionate rise in alkaline phosphatase, moderate hepatomegaly, tiredness, and low-grade fever. Liver biopsy confirmed primary liver granulomatosis in the absence of evidence of pulmonary or systemic involvement. The patient was treated with INH, rifampin, and cycloserine with amelioration of clinical symptoms and return of serum alkaline phosphatase levels to normal.

A case of traumatic osteomyelitis of the leg yielded on culture a branching partially acid-fast organism that failed to respond to therapy directed at Nocardia asteroides. Subsequent laboratory investigation revealed the organism to be Mycobacterium fortuitum. A tuberculous lesion was found in the prepuce of a 5-year-old tuberculin-positive herd bull at necropsy. Microscopic examination revealed granulomas with Langhans'-type giant cells; Mycobacterium bovis was isolated. The bull had been used in artificial insemination procedures and for natural breeding.

Eight individual antigens were detected in soluble antigen preparations from Mycobacterium leprae bacilli by using pools of serum samples from lepromatous leprosy patients as antibody reagents in crossed immunoelectrophoresis.
