Extracapsular dissection of benign parotid tumors using a retroauricular hairline incision approach.

Extracapsular dissection has emerged as a more conservative approach to parotid surgery. The parotid surgery commonly begins with a modified Blair or facelift incision. Although minor, the incision scar from these incisions is visible on the face and neck. I initially developed a retroauricular hairline incision (RAHI) for the removal of benign lesions in the upper neck with a more esthetic look. The RAHI approach also may be used for selected patients with benign parotid neoplasms. Mobile benign tumors arising in the inferior superficial part of the parotid gland may be removed by the RAHI ap

The problem of advanced supraglottic carcinoma.

Sixty-four cases of advanced supraglottic laryngeal carcinoma, and the possible therapeutic errors in these cases, have been analyzed. The available time for the investigation and treatment in these cases is short because of the ease and rapidity of spread of these tumors; thus, it is of great importance: that the nature, site and extent of the tumor is determined quickly; that the patient's general condition is determined and improved as far as possible and the patient receives treatment also for other diseases he may have; and that the tumor is radically eliminated whenever possible without

[Spontaneous vertebral artery dissection complicated by a transient ischemic attack in a migraine patient].

The primary clinical manifestation of vertebral artery dissection may be an isolated neck headache, and therefore, dissection diagnosis may be challenging in patients with chronic headaches. A headache that has acutely developed and changed its character, localization, or trigger, an atypical aura in patients with migraine, requires a specialist's scrutiny.

A serious pain in the neck.

We describe a case of internal carotid artery dissection, an important cause of ischemic cerebrovascular disease. Presentations include a headache, which is usually unilateral to the side of carotid dissection, sharp, nonthrobbing in nature with radiation from the neck. The clinical presentations, investigations, and treatment of the disease are discussed.

A young pregnant woman with spontaneous carotid artery dissection--unknown mechanisms.

Spontaneous carotid artery dissection in pregnancy has not been reported before. We present a case of a 31-year-old Caucasian woman who was 11 weeks pregnant and presented with neck pain, headache, vomiting and left side Horner's syndrome. Subsequent investigations with MR angiography confirmed spontaneous left internal carotid artery dissection.

Atraumatic Vertebral Dissection in a Patient With Altered Mental Status.

The most common manifestations of vertebral artery dissection in approximately 80% of patients are headaches or neck pain. We discuss a case of a 34-year-old patient who presented to the emergency department with altered mental status and nonspecific symptoms. A CT angiogram with intravenous contrast revealed a dissection of the left vertebral artery, and the patient was found to have thromboembolism in the right occipital lobe with ischemia on MRI. This case demonstrates the importance of maintaining a broad differential diagnosis for patients who present with altered mental status and nonspe

Traumatic dissection of the vertebral artery: a case report.

This report concerns a patient suffering from a severe neck pain on the third day after a traffic accident. This condition was followed by a lateral medullary infarction on the next day. One week later, he was transferred to this hospital and dissection of the vertebral artery was disclosed. It should be emphasized that in case of acute cervical spine injury or severe neck pain after a major trauma, vertebral artery dissection should be considered, as its early diagnosis may have crucial therapeutic implications. In addition, the advantages of the combination of magnetic resonance imaging and

Isolated unilateral hypoglossal nerve palsy due to vertebral artery dissection.

We report the case of a patient with unilateral tongue weakness secondary to an isolated lower motor neuron hypoglossal nerve palsy that was caused by a right vertebral artery dissection in the lower neck. The patient had a boggy tongue with a deviation to the right side but an otherwise normal neurological examination. Magnetic resonance angiography showed a narrow lumen of the right vertebral artery in the neck. After initially treating the patient with aspirin in the emergency room and later with warfarin for three months, there was complete recanalization of the right vertebral artery. Onl

Evaluation of metastases in the submandibular gland in head and neck malignancy.

BACKGROUND: : Either excision of the submandibular gland during neck dissection or having the submandibular gland in radiation field can result in xerostomia, leading to reduced quality of life. The purpose of this study was to evaluate the prevalence of metastasis to the submandibular gland and to identify potential risk factors leading to the presence of metastases into the gland. PATIENTS AND METHODS: : Of 376 patients with head and neck malignancy who were treated between 1999 and 2008, 130 patients underwent a neck dissection, and in total, 171 submandibular glands were removed. The aver…

Infrahyoid Flap, a Convenient Alternative for Reconstruction of Tongue and Floor of Mouth Defects: Case Series.

OBJECTIVE: The aim of this study was to share our clinical experience with the neurovascular myofasiocutan infrahyoid flap (NMIHF), which was used for the reconstruction of the defects after oral cavity cancer surgery. METHODS: Records of five patients who were diagnosed with oral cavity cancer and underwent tumor resection, neck dissection, and defect reconstruction with NMIHF between 2012 and 2017 were analyzed retrospectively. RESULTS: The infrahyoid flap was used in five patients: four males and one female. The mean age of patients was 61.8 years. Four patients underwent tumor resection …

[Distribution of cervical lymph node metastasis in patients with cN1 supraglottic cancer].

OBJECTIVE: To study the characteristics of the cervical lymph node metastasis of patients with cN1 supraglottic cancer and to elucidate which levels should be involved during neck dissection. All the lymph nodes of the samples of 147 operation sides were examined pathologically to identify the metastatic levels. RESULTS: Among samples of 108 cases (147 sides), 126 lymph nodes were metastasis, among which 113 lymph nodes were located in levels II and III (89.7%, …

Case report: mucosal melanoma of the lip and the cheek.

Mucosal malignant melanoma arising from the mucosa of the head and neck region is a rare entity. We report one case of mucosal melanoma of the lower lip and the internal face of the cheek. Lymph nodes were not detected in the submandibular or cervical region. The patient had a large exerese of the lower lip and internal area of right cheek and an unilateral neck lymph node dissection. Histopathology showed superficial extended melanoma type SSM, index Clark III, fair metotic index and breslow index 1.5 mm. Lymph node metastasis was not found. Reconstruction was done after 15 days. Primary Closure With Posteriorly Based Lateral Tongue Flap Reconstruction After Transoral Videolaryngoscopic Surgery for Tonsil Cancer.

INTRODUCTION: Transoral surgery for head and neck cancer provides excellent oncologic outcomes while preserving speech and swallowing function. When neck dissection and resection of oropharynx are performed concomitantly, there is a risk of creating a communication defect or developing a pharyngocutaneous fistula. To prevent pharyngocutaneous fistula, we performed the reconstruction using a posteriorly based lateral tongue flap for communication defect. PATIENT: A 72-year-old male with oropharyngeal cancer (tonsil cancer) T2N1M0 underwent concomitant transoral videolaryngoscopic surgery and n…
