PURPOSE: To examine normal anatomic relationship of the globe to the bony orbit, and relate variations in this relationship to clinical parameters.

The authors report the rare occurrence of an orbital roof fracture associated with extradural and subperiosteal orbital hematoma in a 53-year-old man. This case highlights the clinical association of intracranial extradural hematoma and subperiostal orbital hematoma. Traumatic fractures of the orbit are fairly frequent injuries both in wartime and in peacetime. Most often they are part of facial injuries but they can also be isolated. They are followed by numerous symptoms such as eye dislocation, enophthalmos, double vision, and often by various injuries of eye accessory organs. Carcinoid tumors arise from the gastrointestinal tract or the bronchial tree, and tumors of either origin produce symptoms locally or secrete amines that produce a carcinoid syndrome systemically. Metastatic carcinoid tumors to the orbit are rare and, thus, present a special problem in diagnosis and treatment. We describe a case of metastatic orbital carcinoid that presented itself 6 years prior to the discovery of the primary ileal lesion. This case report describes the unusual presentation, reviews previously published data, and addresses controversial opinions regarding therapy.

We examined three patients with arteriovenous malformation of the orbit. At initial examination, all patients had a slowly progressive mass effect combined with congestive symptoms and clinical signs typical of an orbital vascular lesion. A case of monostotic fibrous dysplasia involving the orbit, presenting with a short history of painful, progressive proptosis and a lacrimal fossa mass is reported. There were no clinical or radiological clues to the diagnosis which was made on frozen section at the time of open surgical biopsy. The features of orbital fibrous dysplasia are reviewed and a possible mechanism for this rare clinical presentation of fibrous dysplasia is discussed.

Sixty-five patients with lymphoproliferative disease of the orbit were treated at the Joint Radiation Oncology Center of the University of Pittsburgh. An analysis of these patients was stratified by their initial tumor histopathology: benign lymphoid hyperplasia (BLH) in 28, malignant lymphoma (ML) in 20, and lymphoid infiltrate of indeterminate histology (IH) in 17. The results of the conservative approach in the management of patients with orbital blow-out fractures were studied at the Orbital Centre, Amsterdam. In a prior study we reported that monotypic immunoglobulin expression significantly correlated with an increased risk of dissemination by small lymphocytic infiltrates (SLIs) of the orbit and conjunctiva. Injectable glutaraldehyde cross-linked collagen (Zyplast) has been used to augment the soft-tissue volume of the anophthalmic sockets of five patients. We have reported the case of a 19-year-old black man with sickle cell disease who had swelling over the right frontal and periorbital areas. Plain roentgenograms and CT scans were consistent with frontal sinus disease. At trephination, however, sterile liquescent blood clot was found in the frontal sinus. Awareness of the orbital apex syndrome and infarction of the orbit and sinuses in patients with sickle cell disease is necessary to prevent misdiagnosis of these conditions as suppurative sinusitis.

Findings from orbital computed tomography scans have shown inflammatory enlargement of the levator palpebralis superioris complex to be a likely cause of eyelid retraction in dysthyroid (Graves's) ophthalmopathy. Fifty patients (55 incisions) requiring exploration for repair of zygomatico-orbital trauma were admitted to a prospective study using a uniform and standardised technique for the approach to the orbital rim and floor--the lower blepharoplasty incision combined with a post-orbicularis dissection. A post-operative 'Frost suture' was used in the majority of cases. The incidence of post-operative ectropion is compared to the incidence noted in a previous retrospective study (Pospisil & Fernando, 1984) and factors relating to the occurrence of ectropion are discussed.

Four cases of orbital cellulitis following the extraction of maxillary molars are presented. The time interval between dental extraction and development of orbital symptoms ranged from two hours to 13 days.
