BACKGROUND: Ohtahara syndrome is a progressive developmental and epileptic encephalopathy that manifests in the early infantile period. This rare condition is characterized by intractable seizures, psychomotor retardation, and poor prognosis. To date, there are a handful of case reports regarding the anesthetic management of children with Ohtahara syndrome. However, limited reports exist of patients with Ohtahara syndrome who present with difficult airways. This report describes our airway findings and general anesthetic management of a pediatric patient with Ohtahara syndrome undergoing diagn

BACKGROUND: Fibrodysplasia ossificans progressiva (FOP) is a rare genetic condition characterized by progressive heterotopic ossification of skeletal muscle and soft connective tissues, leading to progressive ankylosis of all joints of the axial and appendicular skeleton. Cervical spine fusion, ankylosis of the temporomandibular joints, thoracic insufficiency syndrome, restrictive chest wall disease, and sensitivity to oral trauma complicate airway management and anesthesia and pose life-threatening risks.

OBJECTIVE: To describe the role of optical coherence tomography (OCT) in the evaluation and surgical planning of pediatric patients with complex airway pathology.
METHODS: We conducted a retrospective case series of four pediatric patients who underwent OCT imaging of the trachea and mainstem bronchi between 2020 and 2024 at a tertiary children's hospital. Imaging was performed intraoperatively using a microcatheter-based OCT catheter under general anesthesia. OCT findings were reviewed in conjunction with bronchoscopy, preoperative imaging, and operative reports.
RESULTS: In each case, OCT

Emergence delirium remains a clinically significant issue, which often leads to distress among pediatric patients, parents, and staff in the short term; and may also result in postoperative maladaptive behaviors persisting for weeks to months. Although several diagnostic tools are available, the Pediatric Anesthesia Emergence Delirium Scale is most often utilized. Many risk factors contributing to the likelihood of a pediatric patient developing emergence delirium have been identified; however, its accurate prediction remains challenging. Recently, intraoperative electroencephalographic monito…

BACKGROUND: Adequate perioperative analgesia is a key component of paediatric urologic surgery. Several regional anaesthesia techniques are commonly used for inguinal and genital procedures; however, prospective comparative data across different block types remain limited.
METHODS: This prospective, single-center, pragmatic comparative cohort study included 160 children undergoing outpatient inguinal or genital urologic surgery. Regional anesthesia was provided using caudal block (CB), quadratus lumborum block (QLB), pudendal block (PDB), or penile block (PB) (n = 40 per group), allocated in

OBJECTIVES: Pediatric trigger thumb (PTT) is characterized by flexion deformity and interphalangeal joint locking caused by A1 pulley constriction. Open A1 pulley release is the standard surgical method, whereas percutaneous release under local anesthesia offers a minimally invasive outpatient alternative. This study compared the outcomes of these two techniques.
METHODS: A retrospective cohort of children aged 2-10 years undergoing A1 pulley release between 2012 and 2024 was analyzed. Patients were assigned to open release under general anesthesia or percutaneous release under local anesthes

PURPOSE: Periocular lesions in pediatric patients usually require general anesthesia for surgical intervention. The US Food and Drug Administration (FDA) warns against multiple exposures to anesthesia in children younger than 3 years due to the increased risk of learning disabilities in this population. This study aimed to evaluate risk factors associated with chalazion recurrence after surgery.
METHODS: A retrospective chart review over a five-year period identified 649 patients at our institution undergoing surgical intervention for chalazion. The primary outcomes examined were as follows:

A former premature infant (1,795 g) with chronic lung disease underwent pyrolomyotomy under spinal anesthesia. She had been managed with artificial ventilation for 2 months after birth and had developed chronic lung disease. She showed frequent apnea with desaturation several times per day and 21 x min(-1) of oxygen had been administered. She began projectile vomiting 1 month after extubation and then was diagnosed as hypertrophic pyrolic stenosis by ultrasonography. She was transferred to our hospital to have pyrolomyotomy. After admission to pediatric intensive care, she was managed with nas

IMPORTANCE: Vocal fold paralysis (VFP) results from the disruption of neural motor outputs to laryngeal muscles. Children with VFP manifest various degrees of difficulties in phonation, breathing, and swallowing. Although the etiologic characteristics and symptoms of VFP are well established in adults, corresponding clinical profiles are notably different in children. Clinical management of VFP is particularly challenging in children because their larynges are still actively developing and the recovery of disrupted laryngeal nerves is often unpredictable. This review discusses the neurologic c

BACKGROUND: Intraarticular injections (IAI) were first reported in adult rheumatology in the 1950s and subsequently gained acceptance as a safe and efficacious treatment in Juvenile idiopathic arthritis (JIA). IAIs are now widely performed and recommended as the initial or only treatment of oligoarticular JIA and ancillary treatment of actively inflamed joints in other varieties of JIA. However, the performance of the procedure is currently not guided by standardized recommendations, and several practice variations are observed.
METHODS: This worldwide survey of pediatric rheumatologists (wit

PURPOSE OF REVIEW: There have been significant advancements in depth of anesthesia (DoA) technology. The Anesthesia Patient Safety Foundation recently published recommendations to use a DoA monitor in specific patient populations receiving general anesthesia. However, the universal use of DoA monitoring is not yet accepted. This review explores the current state of DoA monitors and their potential impact on patient safety.
RECENT FINDINGS: We reviewed the current evidence for using a DoA monitor and its potential role in preventing awareness and preserving brain health by decreasing the incid

BACKGROUND: Dexmedetomidine (DEX), a highly selective α2-adrenoceptor agonist, may produce arousable sedation through locus coeruleus-mediated noradrenergic suppression while preserving respiratory drive. Its clinical applications have expanded beyond intensive care unit (ICU) sedation to procedural sedation, anesthesia adjunction, and non-operating room settings. However, substantial heterogeneity in dosing regimens has impeded the formulation of standardized clinical protocols.
OBJECTIVE: This narrative review synthesizes evidence identified through a MeSH-based search of PubMed, Embase, an

INTRODUCTION: Current ethical practice allows for adult patients with decision-making capacity to refuse blood transfusion, even at the cost of high morbidity or mortality. However, for an adult patient who is of the Jehovah's Witness faith, an unwanted blood transfusion confers a psychospiritual cost to the patient and a financial cost to health care entities. The ethical boundaries are increasingly ambiguous with minors who are members of the Jehovah's Witness faith. This simulation experience intends to identify and address knowledge gaps in the care of minors in an emergent setting using a

BACKGROUND AND OBJECTIVES: Tracheal intubation associated with airway operations can cause complications such as laryngospasm, bronchospasm and periods of reduced oxygen saturation. Such complications are frequently reported during adenotonsillectomies, a procedure that by nature increases the incidence of airway complications. The objective of this study was to compare the occurrence of respiratory problems during adenotonsillectomies while using either a disposable laryngeal mask airway (LMA) or an endotracheal tube (TT).
METHODS: We evaluated 204 pediatric patients undergoing general anest

Skin biopsies provide a valuable source of culturable cells for a broad range of clinical and biomedical research applications. However, conventional methods for establishing primary cell cultures typically require relatively large tissue specimens obtained through invasive procedures that involve intradermal anesthesia, suturing, and subsequent wound care. These factors are of particular concern when working with pediatric populations. To address these limitations, we developed a simplified protocol that reduces patient discomfort by minimizing the punch biopsy diameter to as little as 1 mm.

Awake craniotomy enables the maximal resection of lesions near the eloquent cortex while preserving neurological function. However, its use in the pediatric population remains limited by challenges of cooperation and anesthetic management. We describe a 12-year-old boy with refractory epilepsy due to the recurrence of a left parietal ganglioglioma previously resected under general anesthesia. Preoperative assessment included neuropsychological testing and familiarization with the operating room environment. An asleep-awake-asleep technique was used, with propofol and remifentanil for induction

Acute appendicitis is the most common abdominal surgical emergency today. Blunt abdominal trauma is a rare etiology of appendicitis, with a reported incidence of 0.3-0.9 %. Traumatic appendicitis is more common in the pediatric population, with a median age of 11.5 years. We present the case of a 13-year-old healthy female who presented as a trauma activation after a motor vehicle collision. Clinical examination revealed ecchymoses and abrasions over bilateral hips consistent with a seatbelt sign, with tenderness in the periumbilical region and right lower quadrant. Imaging was consistent with
