Diabetes mellitus and inflammatory pulpal and periapical disease: a review.

Diabetes mellitus (DM) is one of the most common metabolic disorders. DM is characterized by hyperglycaemia, resulting in wound healing difficulties and systemic and oral manifestations, which have a direct effect on dental pulp integrity. Experimental and clinical studies have demonstrated a higher prevalence of periapical lesions in patients with uncontrolled diabetes. The influence of DM on periapical bone resorption and its impact on dental intervention of such patients are reviewed, and its aetiology and pathogenesis are analysed at molecular level. Pulps from patients with diabetes have

[Status of bacterial colonization in infected root canal].

OBJECTIVE: To investigate the morphology, distribution and relative position of the bacteria in infected root canals with chronic apical periodontitis and make some clinical inferences. METHODS: Seven extracted roots with chronic apical periodontitis were collected. Immediately after extraction, all specimens were rinsed in sterile saline solution. The remaining tooth crowns were cut off with carborundum disks under water spray. Longitudinal grooves following the root length axis were cut along the entire root by using tapered diamond burs under water spray, and the roots were then split with

Periapical repair after root canal filling with different root canal sealers.

The aim of this study was to evaluate periapical repair after root canal filling with different endodontic sealers. Sixty-four root canals from dog s teeth were filled, divided into 4 groups (n=16). Root canals were instrumented with K-type files and irrigated with 1% sodium hypochlorite solution. Root canals were filled in the same session by active lateral condensation of the cones and sealers: Intrafill, AH Plus, Roeko Seal and Resilon/Epiphany System. After 90 days, the animals were euthanized and the tissues to be evaluated were processed and stained with hematoxylin and eosin. For histop

Endodontic treatment of a maxillary canine with type 3 dens invaginatus and large periradicular lesion: a case report.

Dens invaginatus type 3 is an anomaly characterized by an infolding of enamel and dentin that can extend up to the root apex and might present difficulties with respect to its diagnosis and treatment because of canal morphology. This article describes an unusual case of the maxillary canine having a type 3 dens invaginatus, necrotic pulp, and an associated large periradicular lesion treated by conventional root canal therapy. After gaining access to the pulp chamber, the primary canal and the invagination were localized, negotiated, enlarged, and filled with calcium hydroxide. Three weeks late

Transitory pre-eruptive malposition of a maxillary canine: report of a case.

The ectopical eruption of the maxillary canine has been related to many causative factors. This paper shows a transient pre-eruptive abnormal position of a maxillary canine in a 9-year-old boy. The change from the normal position of the canine occurred coincidentally with the inflammation of the adjacent lateral incisor due to injury. The canine returned to a normal path after treatment and healing of the lateral incisor.

Host mediators in endodontic exudates. II. Changes in concentration with sequential sampling.

Exudate is often found in the root canal of teeth requiring endodontic therapy. The aim of this study was to evaluate the relationship of sequential changes of different host mediators in endodontic exudates to clinical and radiographic findings. Thirty-two nonvital teeth with periapical symptoms were evaluated. Exudates were collected with filter paper strips every 3 min after opening of the pulp chamber. The concentrations of beta-glucuronidase, IgG, IgA, IgM, and interleukin-1 beta in the exudates were analyzed. In general, the concentration of the mediators in exudates from less involved l

New perspectives on radicular cysts: do they heal?

During the past few decades several authors have perpetuated the notion that nearly half of all periapical lesions are radicular cysts. A few studies, based on meticulous serial sectioning of periapical lesions retrieved in toto, have shown that the actual incidence of radicular cyst is only about 15% of all periapical lesions. Equally significant was the discovery in 1980 and recent confirmation that radicular cysts exist in two structurally distinct classes namely, those containing cavities completely enclosed in epithelial lining (periapical true cysts) and those containing epithelium-lined

Surgical repositioning of unerupted anterior teeth.

The result of surgical repositioning of 12 unerupted anterior teeth was studied. The unerupted teeth were gently exposed, gradually repositioned by shifting them bodily toward the occlusal plane without disturbing the periodontal ligament, and splinted for 3 wk. Root canal therapy was performed at the end of the second week following repositioning. Obturation was done at the end of the third week just before splint removal. Clinical and radiographic examination (follow-up period: 6 months to 3.5 yr) revealed no root resorption with satisfactory apical healing and healthy supporting tissues.

Apical hard-tissue deposition in adult teeth of monkeys with use of calcium hydroxide.

The anterior teeth of five adult monkeys were divided into two groups. In one group the pulps were removed and the teeth were left open to oral fluids. The other group had vital pulps. Five teeth were designated as controls. The pulp canals of both groups were debrided, filed, and flushed thoroughly. In the two experimental groups (vital and pulpless), the apical third of the canal was filled with Ca(OH)2. In the control group no medication was placed in the canal. The coronal access of the canals of all groups was sealed with amalgam. After 270 to 330 days, the teeth were sectioned and examin

Odontogenic Factors Associated with Maxillary Sinus Schneiderian Membrane Thickness and their Relationship to Chronic Sinonasal Symptoms: An Ambispective Cohort Study.

Odontogenic sinusitis is a common maxillary sinus disease. It develops due to the violation of the Schneiderian membrane due to pathological, iatrogenic, or traumatic causes from dental and dentoalveolar structures. The aim of this cohort study was to investigate local and systemic factors associated with Schneiderian mucosal thickening (MT) in patients referred for evaluation of apical periodontitis (AP) and examine their relationship with chronic sinonasal symptoms. Cone-beam computed tomography (CBCT) scans of 197 patients referred for evaluation of endodontic diseases were reviewed. Mucosa

Higher expression of galectin-3 and galectin-9 in periapical granulomas than in radicular cysts and an increased toll-like receptor-2 and toll-like receptor-4 expression are associated with reactivation of periapical inflammation.

INTRODUCTION: Cysts and periapical granulomas are inflammatory reactions that develop in response to periapical infection by microbial species in dental root canal. It is known that toll-like receptors (TLRs) are pathogen recognition molecules and that galectins are lectins that can be associated with the inflammatory process, stimulating or inhibiting the immune system. The objective of this study was to evaluate the in situ expression of TLRs and galectins in radicular cysts and periapical granulomas. METHODS: We analyzed 62 cases (30 radicular cysts, 27 periapical granulomas, and 5 control

[General and local factors affecting periapical reparation after endodontic procedures].

Eighty-seven teeth with 100 periapical destruction foci have been treated in 78 patients from 19 to 60 years of age. In 28 patients calcium-containing preparation was administrated orally for 1 to 3 months. The dynamics of destruction foci changes was similar in all patients though among patients with satisfactory X-ray findings 64% took calcium-containing preparation. No considerable difference in the regeneration dynamics was revealed in patients with somatic diseases and healthy individuals. The residual destruction size 12 months later correlated positively with the initial size of destruc

Diagnosing inflammatory and non-inflammatory periapical disease.

Most central bony lesions appearing in a periapical location are inflammatory in nature secondary to pulpal degeneration. However, cysts, tumors, and other diseases may be easily confused with such endodontic lesions. Four clinical cases are presented, illustrating the importance of thorough evaluation and the role of biopsy in appropriate cases.

Diagnosis and Guided Surgical Management of a Mandibular Buccal Bifurcation Cyst: A Case Report.

Mandibular buccal bifurcation cyst (MBBC) is a rare inflammatory odontogenic cyst that frequently mimics chronic periapical pathosis, leading to potential misdiagnosis and unnecessary endodontic treatment. This case report describes the diagnosis and management of an MBBC associated with a vital mandibular left second molar (#18) in a healthy 16-year-old female, highlighting the value of pulp vitality testing, cone beam computed tomography (CBCT), and a custom 3D-printed surgical guide for conservative management. The patient presented with buccal bony expansion and a 12-mm periodontal probing

Molecular pathogenicity of 1-nonadecene and L-lactic acid, unique metabolites in radicular cysts and periapical granulomas.

Recently, 1-nonadecene and L-lactic acid were identified as unique metabolites in radicular cysts and periapical granuloma, respectively. However, the biological roles of these metabolites were unknown. Therefore, we aimed to investigate the inflammatory and mesenchymal-epithelial transition (MET) effects of 1-nonadecene, and the inflammatory and collagen precipitation effects of L-lactic acid on both periodontal ligament fibroblasts (PdLFs) and peripheral blood mononuclear cells (PBMCs). PdLFs and PBMCs were treated with 1-nonadecene and L-lactic acid. Cytokines' expression was measured using

Curved Planar Reformation: A Useful Method for Screening Dental Pathologies in Chronic Rhinosinusitis via Paranasal Sinus Computed Tomography.

(1) Background: Curved planar reformation (CPR) is a multiplanar reformatting technique of computed tomography (CT) commonly used during dental cone-beam CT (CBCT) to generate panorex-like images for dental evaluation. Here, we evaluated the utility of an additional CPR sequence in detecting dental pathologies in patients with chronic rhinosinusitis (CRS). (2) Methods: CRS patients who underwent paranasal sinus CT were enrolled retrospectively. The CT images featured three orthogonal sequences and a reconstructed CPR sequence. Additional dental CBCT was performed in patients with pathologies w
