The Opioid Epidemic and Perinatal Health.

Opioid misuse, opioid use disorder (OUD), and opioid overdose continue to be significant contributors to perinatal morbidity and mortality, including in the postpartum period. Medications for OUD including methadone and buprenorphine reduce the risk of pregnancy-associated overdose and improve perinatal outcomes. Newer medications and medication formulations are currently being investigated to provide additional treatment options. Modifications to obstetric and pediatric management may be required to best support maternal, fetal, and neonatal outcomes. Harm reduction tools, including Narcan an

Maternal Sildenafil vs Placebo in Pregnant Women With Severe Early-Onset Fetal Growth Restriction: A Randomized Clinical Trial.

IMPORTANCE: Severe early onset fetal growth restriction caused by placental dysfunction leads to high rates of perinatal mortality and neonatal morbidity. The phosphodiesterase 5 inhibitor, sildenafil, inhibits cyclic guanosine monophosphate hydrolysis, thereby activating the effects of nitric oxide, and might improve uteroplacental function and subsequent perinatal outcomes. OBJECTIVE: To determine whether sildenafil reduces perinatal mortality or major morbidity. DESIGN, SETTING, AND PARTICIPANTS: This placebo-controlled randomized clinical trial was conducted at 10 tertiary referral cente…

Effects of short inter-pregnancy/birth interval on adverse perinatal outcomes in Asia-Pacific region: A systematic review and meta-analysis.

BACKGROUND: Short inter-pregnancy or birth interval is associated with an increased risk of adverse perinatal outcomes. However, some emerging evidence questions this association and there are also inconsistencies among the existing findings. This study aimed to systematically review the evidence regarding the effect of short inter-pregnancy or birth intervals on adverse perinatal outcomes in the Asia-Pacific region. METHODS: A comprehensive search of five databases was conducted targeting studies published between 2000 to 2023. Studies that reported on short inter-pregnancy or birth interval

[Various problems and prospects of improving the outcome of cesarean section].

A review of the outcomes of 819 cesarean sections and the causes of intranatal and early neonatal loss of full-term newborns has demonstrated that postcesarean perinatal mortality and morbidity remains fairly high. Perinatal losses are largely seen in cases of late gestosis and placental disorders, i.e. where cesarean section is performed urgently for maternal reasons. Possibilities for improving perinatal parameters are in the optimization of indications for surgical delivery and timely as well as technically correct performance of the operation and high-quality anesthesiologic support at a s…

Obstetric and neonatal outcomes in women who live in an urban resettlement area of Delhi, India: a cohort study.

AIM: To study the pregnancy outcome, namely mode and place of delivery, attendant at birth and perinatal mortality in an urban resettlement area of Delhi, India, and to determine factors that affect the outcome. METHODS: All the pregnant women (n = 909) in the area were enrolled and followed until 7 days after delivery. We calculated the crude and adjusted odds ratios for predictors of pregnancy related obstetric and neonatal outcomes, using logistic regression analysis. RESULTS: A total of 884 (97.3%) women could be followed up. Approximately two-thirds of deliveries took place at home. Pri…

Management of other complications specific to monochorionic twin pregnancies.

Monochorionic (MC) twins have a 3-10-fold higher perinatal mortality and morbidity than dichorionic twins. This is largely attributable to their common vascular architecture and the high rate of discordant fetal growth, growth restriction and congenital abnormalities. In the event of a single intrauterine death (IUD), intertwin agonal transfusion results in up to a 38% risk of death and a 46% risk of neurological injury to the co-twin. This chapter addresses the management of complications unique to MC twins. The primary aim of management is to prevent single IUD or, if inevitable, prevent ago…

Comparison of elective and empiric cerclage and the role of emergency cerclage.

We describe the maternal, obstetric, and neonatal outcomes of patients undergoing elective, empiric, and emergency cervical cerclage at our institution in an attempt to determine predictive factors for adverse perinatal and maternal outcomes. A retrospective chart review was conducted on patients who underwent cervical cerclage placement over a 7-year time span. Of 55 charts, 40 contained complete peripartum data satisfactory for review; 7 elective, 15 empiric, and 18 emergency cerclages were analyzed. There was no perinatal mortality in the elective group, and 5/7 patients delivered at term. …

Hypothermia in Preterm Newborns: Impact on Survival.

Background. Globally, prematurity is the leading cause of neonatal mortality, and hypothermia is one of its contributing factors. The goal of this study was to determine the association between hypothermia and mortality. Methods. A prospective, multi-center, descriptive clinical study was conducted in 5 hospitals in Ethiopia. Axillary temperatures were taken at the time of admission to the newborn intensive care units (NICU) and followed during the NICU stay. Results. A total of 3852 premature neonates (<37 weeks) were admitted to the NICUs from July 2016 to May 2018. Of these infants, 1109 (2

Outcome of low birth weight infants in Diyala province of Iraq.

OBJECTIVES: To measure birth weight-specific mortality rates in the Diyala Province of Iraq and to determine if the causes of neonatal death could identify interventions needed to reduce neonatal mortality rates. METHOD: We retrospectively compared the outcome of 196 neonates with birth weight 500-2499 g admitted in 2003 with 252 such neonates admitted in 2009. RESULTS: The mortality rate in very low birth weight infant (VLBWI) increased from 12/80 (15%) in 2003 to 51/152 (33.6%) in 2009, (p = 0.003). In LBWI, 10/116 (8.6%) died in 2003 compared to 33/152 (13%) in 2009 (p = 0.29). Sepsis acc

A review of primary caesarean sections in Tasmania.

In the years 1975-1982 inclusive there were 55,095 deliveries in Tasmania and of these 2,738 patients (4.9%) were delivered by primary Cesarean section. The primary Caesarean section rate increased from 4.3% in 1975 to 6.6% in 1982. This increase has been due to the increased incidence of primary Caesarean section associated with breech presentation and to a lesser extent fetal distress. Although the overall perinatal mortality rate fell from 19.5 per 1,000 births in 1975 to 10.8 per 1,000 births in 1982, there has been no corresponding fall in the perinatal mortality rate associated with prim…

[Perinatal outcome and cardiac dysfunction in preterm growth-restricted neonates in relation to placental impairment severity].

INTRODUCTION: Intrauterine growth restriction (IUGR) and prematurity have been associated with increased perinatal morbidity and mortality and also with cardiovascular foetal programming. However, there are few studies on the impact of placenta-related IUGR on perinatal outcomes and cardiovascular biomarkers in pre-term infants. OBJECTIVES: To determine differences in neonatal morbidity, mortality and cord blood biomarkers of cardiovascular dysfunction between pre-term placenta-related IUGR and non-IUGR new-borns, and to analyse their relationship with the severity of IUGR according to foetal
