The clinical characteristics and adaptive functioning of preschoolers who met DSM-IV criteria for bipolar disorder versus psychiatric and healthy comparison groups were investigated. A community-based sample of 303 preschoolers (3-6 years of age) and their caregivers was ascertained. Diagnostic classification based on parent report of mania symptoms was made using an age-appropriate psychiatric interview. Myasthenia gravis and hypokalemic periodic paralysis are commonly related with hyperthyroidism but rarely occur together. Here, the authors reported a case of hypokalemic periodic paralysis in a Northeastern Thai woman accompanied with myasthenia gravis. She had motor weakness despite a normal level of serum potassium. Prostigmine test was positive. She dramatically improved after pyridostigmine treatment. Autoantibodies to nicotinic AchR-Ab and dihydropyridine receptor or L-type voltage gated calcium channel were postulated to explain these associated diseases.

We report on two cases of hypokaliemic periodic paralysis due to a potassium shift from the extracellular to the intracellular compartment of skeletal muscle cells. The first case occurred in a 15-year-old boy who experienced rapid onset flaccid tetraplegia without neurological abnormalities. Physical exam revealed facial dysmorphy, and EKG a long QT. BACKGROUND/PURPOSE: The aim of this study was to analyze the outcome of giant omphalocele repaired in the neonatal period.
METHODS: Twelve consecutive (1997-2004) neonates with giant omphalocele (defect >6 cm with liver herniation) were reviewed. A silo of Prolene mesh (Ethicon) was attached to the fascia and the defect was closed without opening the amniotic sac after sequential reduction. We report a case of an immunocompetent man with a 4-year history of persistent pruritic lesions along the long axis of his forearm. The symptoms began 3 years after the patient immigrated to New York from Mexico. A histopathologic evaluation demonstrated primary cutaneous nocardiosis. This rare case of Nocardia brasiliensis infection in a nontropical area raises the possibility that the incubation period for Nocardia may be longer than previously believed. Alternatively, cutaneous nocardiosis may occur in a wider geographic area than previously recognized.

INTRODUCTION: Restless legs syndrome and periodic leg movements in sleep are frequently observed, often in association. Misdiagnosis of these two conditions is common.
STATE OF ART: Physical examination can provide the diagnosis of restless legs syndrome but polysomnography recording must be performed to confirm the presence of periodic leg movements. Dopaminergic pathways are implicated in the pathophysiology of these two syndromes and the same treatment is given: dopaminergic agents, benzodiazepines, opioids and/or antiepileptic drugs.

Occasionally a case report can provide enormous insight into the pathogenesis of an infectious disease. In this instance, 2 Iowa boys were bitten by the same mouse on May 1, 1999. Three weeks later, both boys developed sin nombre hantavirus pulmonary syndrome. Thus, the incubation period of hantavirus infection has been documented. Both survived; one of the 2 boys continued to have a very high titer of hantavirus antibody 4 years after the mouse bite.

Over the last decade, a total of 301 patients were subjected to high amputation of lower limbs at City Clinical Hospital N5 in Nizhniy Novgorod. The authors report two cases of slow ventricular tachycardia presenting in the antenatal period. One foetus had anasarca at 38 weeks' gestation. We report a case of fetal congenital heart disease prenatally diagnosed as critical aortic valve stenosis at 25 weeks of gestation. Fetal echocardiography demonstrated severe mitral regurgitation, aortic valve stenosis, hypocontractility of the left ventricle, and showed retrograde flow in the aortic arch like HLHS (hypoplastic left heart syndrome). However, soon after delivery, improvement in the baby's hemodynamics and myocardial contractility were recognized without any treatment.
