Pityriasis lichenoides et varioliformis acuta (PLEVA) is commonly thought of as a disease of young adults, yet we identified five cases, involving patients who were 3, 5, 6, 8, and 11 years of age, among 13,000 consecutive specimens submitted to a general dermatopathology laboratory during a 15-week period. The clinical and histologic features of PLEVA in our cases were similar to those reported for adults, except that no lesions were observed on the scalp or mucous membranes of children. A high index of suspicion and biopsy specimens of suspected lesions are often needed to differentiate PLEV…

The mycologic and clinical efficacy and the local tolerance for 1 and 2 weeks of once-daily treatment of sixty cases of pityriasis versicolor with bifonazole 1% cream were studied in a biometrically planned, controlled, double-blind group comparison. The mycologic findings after 2 weeks were negative in all the patients, with the exception of one from the 1-week group. Two weeks later the findings were negative in each case. The evolution of the clinical symptoms was similarly good: after 2 weeks, only two patients in the 2-week group still exhibited scaling. In one case the pruritus, which ha…

One hundred and twenty-one patients with tinea (pityriasis) versicolor, confirmed by pretherapeutic clinical and mycological examination, were treated with a single daily application of bifonazole (BAY h 4502) 1% gel for 2 weeks. Weekly clinical controls were performed during the treatment period and at least two clinical and mycological post-therapeutic controls were given. Overall results were very good in 98 patients, good in 15, moderate in 7 and failure in 1 patient. Local tolerance was very good; only 1 patient presented with slight erythema and pruritus during the treatment.

OBJECTIVES: Ultrasonography (US) and magnetic resonance imaging (MRI) are increasingly used imaging techniques for visualising entheses, however few studies have made a direct comparison of each. This study aimed to compare each technique for the detection of enthesitis in patients with spondyloarthritis (SpA) related knee swelling in order to make a lesion by lesion comparison.
METHODS: Consecutive SpA patients with knee synovitis were recruited: each had clinical assessment for enthesitis at 8 sites in the involved knee joint followed by US and MRI examinations. Inflammatory and structural

Pityrosporum orbiculare is an obligate lipophilic yeast in vitro, which suggests it possesses an extracellular lipase crucial for nutrition. If present in vivo, the enzyme would enable the yeast to utilize skin surface lipids, which may therfore play an important role in the pathogenesis of pityriasis versicolor. Cultured P. orbiculare and biopsy material from patients with pityriasis versicolor were investigated for the presence of lipase by electron microscope histochemistry. At sites of lipase activity, fatty acid hydrolyzed from Tween 80 substrate reacts with Ca++ ions to form an insoluble…

Unique cytolytic degeneration of keratinocytes adjacent to Langerhans cells was observed in five cases of pityriasis rosea (Gilbert). In the cytolytic area, the normal keratinocyte structures were absent except for free ribosomes. In addition, a similar cytolytic part of a keratinocyte was enveloped by the elongated dendrites of the Langerhans cell and was detected within its cytoplasm. The significance of these findings is discussed with respect to anticytoplasmic antibodies in the sera of patients with pityriasis rosea.

A 22-year-old homosexual male, newly diagnosed with human immunodeficiecy virus (HIV), presented with a rash and thin, scaly peeling on both knees and elbows. Multiple orange-brown, scaly plaques were observed on the flexor surface of the elbows. Histopathologic examination was consistent with a clinical diagnosis of pityriasis rubra pilaris (PRP). The CD4+ T lymphocyte count was 522/µL, and the viral load was 19,100 copies/µL. Treatment with acitretin and antiretroviral therapy was initiated, and the patient responded well within three months.

BACKGROUND: Malassezia globosa is a commensal basidiomycetous yeast occurring on the skin that causes pityriasis versicolor (PV) and seborrheic dermatitis, but that has also been implicated in other dermatoses. Cinnamaldehyde (CM) has antibacterial, antioxidant, and anti-inflammatory activities, but the effect of CM on M. globosa-infected PV has not been clarified.
PURPOSE: The study aimed to investigate the possible antifungal and antibiofilm activities of CM against M. globosa-infected PV in vivo and in vitro.
METHODS: The broth microdilution method was used to determine the minimum inhibi…

Pityriasis rubra pilaris (PRP) is a rare papulosquamous skin disorder that often presents with erythematous follicular-based hyperkeratotic papules that can become confluent and lead to erythroderma and electrolyte and thermoregulatory imbalances resulting from increased tissue perfusion and skin barrier breakdown. Due to this condition being uncommon, many specialties outside of dermatology are unfamiliar with this entity which poses unique diagnostic and management challenges. This case report involves a 55-year-old woman who presented to the emergency room with erythroderma secondary to PRP…

Confluent and reticulated papillomatosis (CARP) is a rare disorder mostly seen in young adults. It is characterized by persistent dull-brown, centrally confluent, peripherally reticulate macules and papules, which coalesce to form patches and plaques on the upper trunk and neck. It is commonly confused with pityriasis versicolor and acanthosis nigricans (AN). We report the case of a 15-year-old male with multiple pigmented confluent and reticulated patches and plaques on the neck, trunk, and chin for 3 years, which was successfully treated with oral minocycline, resulting in complete resolutio…
