A high tricuspid regurgitant jet velocity (TRV) signifies a risk for or established pulmonary hypertension (PH), which is a serious complication in thalassemia patients. The underlying pathophysiology in thalassemia subgroups and potential biomarkers for early detection and monitoring are not well defined, in particular as they relate to spleen removal. To better understand some of these unresolved aspects, we examined 76 thalassemia patients (35 non-transfused), 25 splenectomized non-thalassemia patients (15 with hereditary spherocytosis), and 12 healthy controls. An elevated TRV (>2.5 m/s) w

A 13-year-old male patient with a history of pre-B cell acute lymphoblastic leukaemia (ALL) with isolated central nervous system relapse on maintenance chemotherapy presented with severe thrombocytopenia refractory to platelet transfusions. The patient showed only modest responses to two courses of intravenous immunoglobulin and steroids. He was found to be positive for cytomegalovirus (CMV) with modest viral load. His thrombocytopenia normalised with rituximab therapy and CMV treatment supporting the diagnosis of CMV-associated immune thrombocytopenic purpura (ITP). Following treatment, the p…

A 45-year-old woman with previously diagnosed chronic type adult T-cell leukemia (ATL) presented with abdominal discomfort and red eruptions on her arms and legs. Anemia, thrombocytopenia, hypercalcemia, and splenomegaly indicated progression to acute-type ATL. Combined chemotherapy resulted in normalization of the serum calcium level and improvement in her symptoms. However, the severe anemia and thrombocytopenia persisted, necessitating transfusions of red blood cells (RBC) and platelets three times a week. We performed splenectomy in an attempt to reduce the total volume of malignant cells

Von-Willebrand disease (VWD) is one of the platelet dysfunction disorders that results from a deficiency of Von-Willebrand factor, which is essential for hemostasis. VWD patients typically have normal laboratory results on screening for bleeding disorders. To suspect and diagnose VWD, a careful review of past medical history and laboratory tests is critical. A 59-year-old male patient presented with intractable nasal bleeding after nasal polypectomy. The bleeding was controlled by platelet transfusion, and he was later diagnosed with VWD.

Fetal and neonatal alloimmune thrombocytopenia (FNAIT) is a major cause of severe thrombocytopenia, intracranial hemorrhage, and long-term neurological impairment in otherwise healthy neonates. Current management strategies are largely reactive, relying on maternal intravenous immunoglobulin (IVIG), intrauterine platelet transfusions, and postnatal supportive care. Although these interventions can be lifesaving, they are associated with considerable risks, inconsistent efficacy, and significant maternal and healthcare burdens, underscoring the need for a paradigm shift toward prevention. Recen…

The platelet-rich plasma (PRP) has proved promising regarding its applicability in dermatology, especially in the healing of chronic ulcers. The autologous platelet-rich plasma is obtained by centrifuging the blood, so that the components are separated by density gradient. The final product is a gel rich in growth factors that act in tissue repair by activating fibroblasts and inducing extracellular matrix remodeling.

Acute febrile illnesses (AFIs) with thrombocytopenia, such as dengue, leptospirosis, and scrub typhus, are often associated with bleeding and present challenges in transfusion therapy during outbreaks. Conventional bleeding predictors, which involve platelet counts, do not assess clot strength or fibrinolysis defects and may lead to inappropriate or delayed transfusions. To predict bleeding, we aimed to create a scoring system using point-of-care thromboelastography (TEG) and other conventional coagulation parameters. In this prospective cohort study, patients with AFI and platelet counts <100

OBJECTIVES: Transfusion of stored RBCs is associated with increased morbidity and mortality in trauma patients. Plasma hemoglobin scavenges nitric oxide, which can cause vasoconstriction, induce inflammation, and activate platelets. We hypothesized that transfusion of RBCs stored for prolonged periods would induce adverse effects (pulmonary vasoconstriction, tissue injury, inflammation, and platelet activation) in lambs subjected to severe hemorrhagic shock and that concurrent inhalation of nitric oxide would prevent these adverse effects.
DESIGN: Animal study.

Retinopathy of prematurity (ROP) is a retinopathy caused by abnormal proliferation of blood vessels in premature infants. It can lead to retinal detachment and, in severe cases, blindness, rendering ROP a critical condition. Advances in neonatal medicine have improved survival rates of low birth weight and low gestational age infants. However, this progress has also led to a rise in incidence of ROP. Currently, premature birth, low birth weight and high postpartum oxygen levels are independent risk factors for ROP. Other factors include mode of delivery, multiple births, anemia, blood transfus

BACKGROUND: The current study assessed a recently developed resuscitation protocol for bleeding trauma patients called the Targeted Transfusion Protocol (TTP) and compared its results with those of the standard Massive Transfusion Protocol (MTP).
METHODS: Per capita utilization of blood products such as packed red blood cells (RBCs), fresh frozen plasma (FFP), and platelet concentrates was compared along with mortality rates during two 6-month periods, one in 2011 (when the standard MTP was followed) and another in 2014 (when the TTP was used). In the TTP, patients were categorized into three

BACKGROUND: Bernard-Soulier syndrome (BSS) is a rare bleeding disorder caused by defects in the GPIb-IX-V complex, which is essential for platelet adhesion. We report a novel case of BSS in a Mauritanian patient with a history of chronic bleeding since childhood, accompanied by thrombocytopenia and macroplatelets.
METHODS: Diagnosis was confirmed by flow cytometry and molecular analysis. The variant identified in the GP1BB gene (glycoprotein Ib platelet subunit beta, HGNC:4440, OMIM:138720; 22q11.21). The variant identified was annotated according to HGVS nomenclature.

BackgroundOesophagectomy is a major oncological surgical procedure. Previous studies have shown a wide range of bleeding during and after surgery, and it is unknown if perioperative bleeding associated with oesophagectomy is purely surgical in nature, or if it is exacerbated by impaired haemostasis. We aimed to perform a detailed investigation of the perioperative coagulation in patients undergoing oesophagectomy due to cancer.MethodsThe study was a prospective study including adult patients with adeno- or squamous cell carcinoma referred for intended curative oesophagectomy. BACKGROUND: In the absence of low-titer group O whole blood, conventional components are often transfused to hemorrhaging trauma patients in a ratio designed to replicate whole blood. However, there is still confusion surrounding how conventional components should be used to support traumatically injured bleeding patients, particularly concerning how platelets should be counted in a ratio-based approach and when the resuscitation can switch from a ratio-based to a laboratory-guided approach.
CASE REPORT: A traumatically injured patient, who was resuscitated with 10 units of red blood cells (R

May-Hegglin anomaly (MHA) is a hematologic disorder defined by large platelets (mean platelet volume >12.5 fL) and thrombocytopenia. While it remains unclear whether these patients exhibit clinical coagulopathy, bleeding complications may be confounded in those undergoing cardiac surgery requiring cardiopulmonary bypass (CPB). There are no current guidelines for the perioperative management of patients with MHA. Although some reports favor no treatment, limited case reports have described desmopressin (DDAVP) and platelet transfusion as additional strategies. Thromboelastography® with Platelet…

BACKGROUND AND OBJECTIVES: Immune-mediated acute or delayed transfusion reactions occur when there is immunological incompatibility between transfused blood products and recipient's antibodies. Acute haemolytic transfusion reactions occur within 24 h and are delayed after 24 h up to 10 days following transfusion, whereas post-transfusion purpura (PTP) typically occurs 7-10 days post-transfusion. We present a case of a previously transfused and recently post-partum female who developed both delayed haemolytic transfusion reaction (DHTR) and PTP.

We describe a complete remission with cyclosporine A in a myelodysplastic syndrome (MDS) patient who had a 9-year history of nephrotic syndrome (NS) due to autoimmune nephritis. A 72-year-old woman with MDS and NS rapidly developed thrombocytopenia with multiple spontaneous bleeding episodes and profound proteinuria. She showed persistent platelet refractoriness to platelet transfusions. A flow cytometry examination strongly detected antiplatelet autoantibodies on the surface of her platelets. The treatment with high-dose corticosteroids and intravenous immunoglobulin did not lead to complete …

BACKGROUND: While Cell Saver technology effectively reduces red blood cell transfusion in the perioperative setting, its impact on the need for pro-hemostatic factors remains unclear. The use of Cell Saver could impact the requirements of pro-hemostatic factors.
OBJECTIVE: This study assesses whether the use of Cell Saver influences transfusion requirements for fresh frozen plasma, platelet concentrates, and fibrinogen in surgeries for placenta accreta spectrum (PAS) disorders.
METHODS: This retrospective, single-center, before-and-after study was conducted in a tertiary maternity hospital.

Most of the literature on massive transfusion concerns whole blood replacement, whereas clinically, packed red blood cells are commonly given. To determine when hemostatic abnormalities occur in patients resuscitated primarily with packed red blood cells and crystalloid, the cases of 39 consecutive patients who were transfused with 10 or more red blood cell units of any kind within 24 hours were reviewed. After transfusion with 20 or more units of red blood cell products of any kind (packed red blood cells, cell-saver units, or whole blood), 75% (3 of 4) of patients had platelet counts less th
