Advanced paramedics' restraint decision-making when managing acute behavioural disturbance (ABD) in the UK pre-hospital ambulance setting: A qualitative investigation.

Acute behavioural disturbance (ABD), sometimes called 'excited delirium', is a medical emergency. In the UK, some patients presenting with ABD are managed by advanced paramedics (APs), however little is known about how APs make restraint decisions. The aim of this research is to explore the decisions made by APs when managing restraint in the context of ABD, in the UK pre-hospital ambulance setting. Seven semi-structured interviews were undertaken with APs. All participants were experienced APs with post-registration, post-graduate advanced practice education and qualifications. The resulting

Learning from mothers: how myths, policies and practices affect the detection of subtle developmental problems in children.

BACKGROUND: Recent research has revealed increasing concerns over the number of children entering school with unidentified developmental problems, even though there are ostensibly comprehensive health services available for mothers and their children in the pre-school years. Recognizing that early detection and early intervention reduce the likelihood of long-term health and educational problems, it is important to understand why so many children are not detected with developmental problems in their pre-school years. METHODS: This doctoral study utilized the knowledge and experience of mother

Preparing the Nurses of the Future to Address Health Disparities.

Nurses need competence and confidence to assess for Social Determinants of Health (SDOH) and meaningfully mitigate the barriers they present to health. While acute care nurses are in an ideal position to address SDOH and optimize the continuum of care, evidence suggests they lack the necessary knowledge and confidence to address SDOH in acute care. The purpose of this project was to describe the frequency of SDOH topics encountered by undergraduate nursing students during clinical learning in acute care and whether those topics were addressed by the student independently or in collaboration wi

American Board of Medical Specialties and repositioning for excellence in lifelong learning: maintenance of certification.

The board certification movement was founded out of a concern for the quality of care, and today, more than 85% of all physicians licensed to practice medicine in the United States have been certified by an American Board of Medical Specialties (ABMS) member board. There is increasing evidence of a need for continuous monitoring and promotion of quality as well as for assessment and documentation that certified medical specialists are keeping up-to-date so that their continuing competence can be documented. To help, the ABMS established a program called Maintenance of Certification, a system t

Mothers' decision-making processes regarding health care for their children.

This research study explored mothers' decision-making processes regarding health care for their children. Identifying how decisions are made by mothers about health care for their children will assist health care professionals to be appropriate advocates for mothers, to improve quality of life, and to contain costs of health care for children. A sample of 114 rural mothers (51 with one child, 63 with more than one child) completed questionnaires identifying demographics, social support, client and professional interaction elements, self-determinism, competence in problem-solving skills, and re

The role of the immune system in anogenital human papillomavirus.

There is substantial evidence from experiments of nature that immune competence plays a major part in determining the outcome of anogenital human papillomavirus (HPV) infection. Cellular rather than humoral immunity would appear to be the key to the control and eradication of HPV-induced warts. It seems likely that the HPV early proteins, which are responsible for viral replication (E1 and E2) and for promoting tissue proliferation (E6 and E7) will be the target antigens recognized by antigen-specific T cells. Natural infection is slow to produce an appropriate therapeutic immune response to t

Phronesis and Nursing Clinical Judgment Revisited: A Reappraisal of Benner's Model of Expert Practice.

Within nursing, clinical judgment is widely recognized as an essential professional competency underpinning clinical care, educational formation, and research development. Recent scholarship has renewed attention to clinical judgment. However, its conceptual structure remains undertheorized with respect to its normative and evaluative dimensions. This article argues that phronesis, understood as the Aristotelian concept of practical wisdom, is constitutive of nursing clinical judgment. Drawing on philosophical and nursing virtue ethics perspectives, it examines Patricia Benner's account of exp
