Acute onset bilateral hearing loss in dorsomedial pontine hemorrhage: A Case report.

RATIONALE: Nontraumatic pontine hemorrhage represents approximately 10% of all cases of nontraumatic intracranial hemorrhage. The predominant cause and symptom of pontine hemorrhage are hypertension and disturbance of consciousness, respectively. PATIENT CONCERNS: A 64-year-old man was transported to hospital by ambulance for sudden articulation disorder and right leg paralysis. He was neurologically alert with mild dysarthria, right leg paralysis, and increased deep tendon reflex in all limbs. DIAGNOSIS: Plain head computed tomography showed an approximately 1.5 mL hemorrhage in the dorsome

Contact lenses for visual rehabilitation in Bilateral Nanophthalmos with Retinal Dystrophy- A case report.

BACKGROUND: Nanophthalmos is an uncommon developmental ocular disorder characterized by a small eye with short axial length, high hyperopia and high lens to eye volume ratio due to arrested development of the globe in all directions. Different types of fundus changes can rarely occur with nanophthalmos. OBSERVATIONS: This is a case report of a 17 years old female who presented with bilateral gradual progressive visual loss and photophobia at Tilganga Institute of Ophthalmology on 3rd July 2015. Her best corrected visual acuity was 20/300 with +12.00Ds/-1.00Dcyl@180 in the right eye and 20/200

Pisa-Like Syndrome Under Baclofen in a Patient With Spastic Hemiparesis due to Ischemic Stroke.

In its original description, Pisa syndrome was reported as an iatrogenic dystonia of the trunk caused by neuroleptic drugs. However, sometimes, not dystonic lateral flexion of the trunk is described as Pisa syndrome. These observations support the possibility of a drug-induced lateral flexion of the trunk with clinical presentation similar to Pisa syndrome, although with a different etiology and pathophysiology. Here, we describe the case of a male patient, with a previous ischemic stroke and residual spastic hemiparesis to the right side, who subacutely developed a dramatic lateral flexion of

When brain damage "improves" perception: neglect patients can localize motion-shifted probes better than controls.

When we look at bars flashed against a moving background, we see them displaced in the direction of the upcoming motion (flash-grab illusion). It is still debated whether these motion-induced position shifts are low-level, reflexive consequences of stimulus motion or high-level compensation engaged only when the stimulus is tracked with attention. To investigate whether attention is a causal factor for this striking illusory position shift, we evaluated the flash-grab illusion in six patients with damaged attentional networks in the right hemisphere and signs of left visual neglect and six age

[Hemicape-like sensory disturbance caused by cortical infarction in the postcentral gyrus].

We report a case of a small cortical infarction in the postcentral gyrus that presented an isolated hemicape-like sensory disturbance. A 47-year-old man suddenly developed numbness and paresthesia in the left neck, shoulder, arm, and upper trunk. Examination revealed hypoesthesia to touch and pain in these areas along with a hemicape-like distribution. The sensitivity to cold and vibration was normal, and two-point discrimination and graphesthesia were preserved. The patient had a normal visual field, muscle strength, and reflexes, and there were no neuropsychological deficits. Magnetic resona

[Thalamic dementia due to infarct of the left thalamus and genum of the right internal capsule].

INTRODUCTION AND OBJECTIVES: Thalamic dementia is the clinical consequence of a disorder of both thalami. It is generally secondary to bilateral paramedial thalamic infarcts due to disorders of small blood vessels or cardioembolism. We report a case of dementia of acute onset involving the left thalamus and the genum of the right internal capsule. CLINICAL CASE: A 33 year old man, HIV positive, category B2, admitted to hospital for tuberculous meningitis presented with the acute onset of somnolence, followed by marked bradypsychism, personality changes, marked disorder of executive explicit m

Guillain-Barre syndrome after antithymocyte globulin administration in a kidney transplant recipient: A case report and literature review.

This report describes a rare case of developing Guillain-Barre syndrome (GBS) following receiving rabbit antithymocyte globulin (ATG) after kidney transplantation to prevent acute allograft rejection in a 34-year-old man. The patient presented severe pain in the right temporomandibular joint, fever, chills, myalgia, polyarthralgia, and bone pain. Twelve hours later, he developed quadriplegia, paresthesia, and a limited range of active motions in all extremities. No antecedent viral or bacterial infection was identified. The EMG/NCV evaluation displayed acute inflammatory sensory-motor polyneur

Transesophageal echocardiography probe induced recurrent VF in a child with severe RV dysfunction: Report of a rare case.

The safety of transesophageal echocardiography (TEE) probes has been documented in pediatric patients (neonates, infants, and small children even < 2.5 kg). The overall safety profile of TEE probe is quite favorable with a reported incidence of complications of 1%-3%. However, insertion of the TEE probe can induce vagal and sympathetic reflexes such as hypertension or hypotension, nonsustained ventricular and supraventricular tachyarrhythmias, or bradyarrhythmias (third degree heart block), and even angina and myocardial infarction. We hereby document a repeated intraoperative ventricular fibr

[Case of neuralgic amyotrophy with anti GT1a antibody].

A 48-year-old-man had intense pain in the neck and muscle weakness in the left upper limb after he presented low grade fever and appetite loss for a week. Several days later, he developed intense pain and severe muscle weakness in bilateral upper limbs. Laboratory examination showed elevated liver enzyme levels. His muscle weakness was severe in the right upper limb and was moderate in the left upper limb. Deep tendon reflexes were decreased in the bilateral upper limbs. CSF showed albuminocytologic dissiciation. A diagnosis of neuralgic amyotrophy was made. His liver dysfunction improved grad

Inverse Argyll Robertson pupil in Burkitt's lymphoma.

We present a case of an 18 year old white male with Burkitt's lymphoma who was operated on for hydrocephalus and subsequently referred for evaluation of new onset diplopia. On examination, his visual acuity (VA) was 20/20 in both eyes with a right superior oblique palsy. His pupillary reaction to light was intact while on near gaze there was no constriction of the pupils, bilaterally. The other two responses of the near gaze triad ie, convergence and accommodation were present. These findings were suggestive of an Inverse Argyll Robertson pupil (IARP), a rare entity in the literature. We could

Psychiatric approach in the treatment of reflex sympathetic dystrophy in an adolescent girl: a case report.

Reflex sympathetic dystrophy (RSD) is an unusual diagnosis in the pediatric age group. It is a syndrome characterized by pain in one or more extremities with a significant morbidity in childhood. Patients with RSD have frequently undergone many unnecessary investigations such that the diagnosis and treatment may be considerably delayed. The pathophysiology remains unclear; however, a number of psychological problems were frequently suggested to play a role in this disorder. We describe a 13-year-old girl diagnosed as pediatric RSD who was admitted to a child and adolescent psychiatry unit with

Sleep disorder in alien hand syndrome.

A 63-year-old right-handed woman developed an alien hand syndrome (AHS) after an acute infarction in the territory of the left anterior cerebral artery. The uncontrolled hand movements were present during the daytime and eventually disturbed sleep. Polysomnography revealed that these motor actions only appeared when the patient was awake. These awakenings emerged mostly from NREM sleep stage 2 during the first half of the night. There was no evidence of any epileptiform activity, dyssomnia or parasomnia. These movements were controlled making her wear an oven mitt during sleep. The temporal di

The origin of hypsarrhythmia and tonic spasms in West syndrome: evidence from a case of porencephaly and hydrocephalus with focal hypsarrhythmia.

We report on a 3-year-old girl with West syndrome and with focal hypsarrhythmia. The left hemisphere of the patient was virtually completely defective and continuous hypsarrhythmia was only seen in the residual right frontal cortex, where an interictal single photon emission computed tomography (SPECT) showed hyperperfusion. Despite a focal epileptic pattern, the tonic spasms were quite symmetrical. In our patient, spasms might not require the sensorimotor cortex, but the brainstem containing the descending pathways that control spinal reflexes and other infratentorial structures seem to be es

Electrocochleographic response patterns in a group of patients mainly with presbyacusis.

The latency, amplitude and wave-form of the action potential were studied in a group of patients, mainly presbyacusics and a few with noise-induced hearing loss, sensorineural hearing loss of unknown etiology and conductive loss. Electrocochleographic recruitment with short latency of the AP "threshold" and very rapid increase in amplitude with increasing click intensity was seen in 22%. Partial recruitment with long latency at AP "threshold" but steep amplitude-intensity curves was present in 33%. Only 17% of the subjects showed both electrocpchleographic recruitment (or partial recruitment)

Does utricular stimulation elicit nystagmus in humans?

BACKGROUND: Stimulation of the utricle has been hypothesized to elicit nystagmus. However, the observation that nystagmus rarely occurs immediately following the Epley maneuver for posterior semicircular canal benign paroxysmal positional vertigo has raised questions about the existence of a robust otolith-ocular reflex. OBJECTIVE: To determine whether nystagmus can be elicited from the utricle by applying linear acceleration. MATERIALS AND METHODS: The subjects were 10 healthy individuals. Each subject was seated in a wheelchair, rotated their upper body 90° to the right, and moved side to

Unilateral Oil Droplet Cataract in a Child: Importance of Early Detection and Intervention.

Oil droplet cataract is a rare pediatric lens opacity that can cause significant amblyopia if not promptly diagnosed and treated. A 5-year-old male presented with intermittent exotropia and reduced vision in the left eye. Visual acuity was 6/6 in the right eye and 5/60 in the left eye, improving to 6/36 with correction. Retinoscopy revealed an oil droplet reflex in the left eye. Systemic evaluation ruled out galactosemia. Phacoemulsification with intraocular lens implantation was performed, followed by refractive correction and occlusion therapy. At 3 months, left eye vision improved to 6/24 u

Monocular Torsional Oscillopsia in Dentato-olivary Disconnection.

Monocular torsional eye oscillations are a rare form of disconjugate nystagmus and the underlying pathophysiology is not well understood. Here, we present and discuss a case with disabling torsional oscillopsia in one eye only. The patient exhibited (i) spontaneous pendular torsional nystagmus of the left eye and (ii) rhythmic involuntary movements of the soft palate and uvula, consistent with the syndrome of oculopalatal tremor with monocular nystagmus. Brain MRI revealed an infarct of the left dentate nucleus in the cerebellum and, more caudally, a secondary hypertrophic degeneration of the

Bilateral, vertical supranuclear gaze palsy following unilateral midbrain infarct.

A 60-year-old man recently admitted for bipedal oedema, endocarditis and a persistently positive COVID-19 swab with a history of anticoagulation on rivaroxaban for atrial fibrillation, transitional cell carcinoma, cerebral amyloid angiopathy, diabetes and hypertension presented with sudden onset diplopia and vertical gaze palsy. Vestibulo-ocular reflex was preserved. Simultaneously, he developed a scotoma and sudden visual loss, and was found to have a right branch retinal artery occlusion. MRI head demonstrated a unilateral midbrain infarct. This case demonstrates a rare unilateral cause of b

Audiological findings from an adult with thin cochlear nerves.

Reported here are audiological findings from an adult with thin cochlear nerves. Magnetic resonance imaging (MRI) revealed that he had a thinner cochlear nerve in the left ear than in the right ear. He had a higher degree of hearing loss in the left ear and poor speech recognition scores for both ears. He had normal middle ear and cochlear functioning. The auditory brainstem response and acoustic reflexes were absent, indicating a retrocochlear pathology. Long latency responses (LLR) revealed normal cortical functioning. Hence, implantation of an auditory brainstem implant might be an option,

Clinical Features of Spinal Cord Hemangioblastoma in a Dog.

A 2-year-old male, intact Yorkshire terrier presented with a 1-month history of progressive paraparesis. Neurological examination revealed paraplegia with absent deep pain perception, decreased right pelvic limb withdrawal reflex, and lumbar pain consistent with an L4-S2 neurolocalization. Magnetic resonance imaging (MRI) showed a single, well-demarcated, intramedullary mass centered over the L3-4 disk space. A hemilaminectomy was performed, and the mass was removed en bloc. Histopathological evaluation was consistent with a hemangioblastoma. Follow-up MRI 9 months after surgery showed no evid
