Arthroscopic repair of type II superior labral tears using suture anchors has resulted in a good to excellent outcome in most cases. In throwing athletes, however, the results have not been as consistent. Although it has not been reported in the literature, it is also possible that, in a very small number of throwers, the limited space in the glenohumeral joint combined with the bulky knots used to secure the labrum to the anchor may be a cause of discomfort. In addition, the superior labrum is meniscoid in appearance, which traditional SLAP repair techniques do not recreate. We describe here

Arthroscopic rotator cuff repair (RCR) has been reported to have good clinical results but high retear rates by ultrasound. We prospectively assessed postoperative cuff integrity and outcome after arthroscopic RCR (40 patients) and compared these results with open RCR (32 patients). Evaluation preoperatively and at 1 year included a physical examination and magnetic resonance imaging. American Shoulder and Elbow Surgeons and Constant scores improved significantly in both groups (P < .0001). Overall, 69% of repairs in the open group and 53% in the arthroscopic group were intact by magnetic reso

PURPOSE: Large rotator cuff tears present a challenge to orthopaedic surgeons. Because tissue may be insufficient or of inadequate quality to undergo repair, a variety of materials have been used as adjuncts. Human dermal tissue may be processed to render it acellular, and thus less immunogenic, but with the extracellular matrix left intact, creating a collagen scaffold with favorable characteristics. Because of these traits, use in rotator cuff repair was proposed.
METHODS: A canine model for a full-thickness infraspinatus tendon tear was used. Tendon was excised from the bony interface to t

UNLABELLED: In a vitro study we evaluated the influence of surgical technique on the postoperative tension of the supraspinatus tendon in rotator cuff ruptures. In ten shoulder specimens Hall-Effect Strain Gauge (HEST) were implanted in the supraspinatus tendon close to the insertion at the humerus. We documented the tension in specimens with intact rotator cuff as well as after reconstruction of small and large tears in different joint positions. In the specimens with an intact rotator cuff there was almost no tension registered. Only in adduction with external rotation and flexion we found s

Coracoid fractures are an uncommon injury and typically occur in the setting of high-energy trauma. Isolated injury to the coracoid is rare; therefore, a high suspicion for concomitant shoulder injuries should exist. These associated injuries have been shown to be acromioclavicular dislocations, clavicular and acromial fractures, scapular spine fractures, rotator cuff tears, and anterior shoulder dislocations. Although most of these shoulder injuries respond to nonsurgical treatment, there are case reports and literature reviews that present more complicated injuries requiring surgical interve

BACKGROUND: The benefits of acromioplasty in treating rotator cuff disease have been debated. We systematically reviewed the literature regarding whether acromioplasty with concomitant coracoacromial (CA) release is necessary for the successful treatment of full-thickness rotator cuff tears.
MATERIALS AND METHODS: We identified randomized controlled trials that reported on patients who underwent rotator cuff repair with or without acromioplasty and used descriptive statistics to summarize the findings.
RESULTS: Four studies fulfilled the inclusion criteria. They reported on 354 patients (mea

Once the almost exclusive domain of the orthopaedic surgeon, sports injuries are now being seen with increasing frequency by other specialists, including rheumatologists. It is therefore important for rheumatologists to be able to diagnose and manage the various musculoskeletal conditions that are associated with physical activity. Soft tissue injuries are a very common cause of morbidity in both competitive and recreational athletes. Most of these conditions are provoked by muscle-tendon overload (or overuse) that is usually the result of excessive training or improper training techniques. Ho

BACKGROUND: Multiple patient-reported outcome measures (PROMs) have been used to assess shoulder function, but it is unknown which are the most effective.
PURPOSE/HYPOTHESIS: The purpose of this study was to report the multiple PROMs used after rotator cuff repair (RCR) and to compare the responsiveness between them. It was hypothesized that the Western Ontario Rotator Cuff PROM would be the most responsive and commonly used in patients undergoing RCR.
STUDY DESIGN: Meta-analysis; Level of evidence, 4.
METHODS: A systematic review was conducted in accordance with PRISMA (Preferred Reporting

BACKGROUND: Subscapularis muscle strains can develop in professional baseball players, rendering the players unable to continue playing for a certain period. However, the characteristics of this injury are not well known. The purpose of the present study was to investigate the details of the injury and the postinjury course of subscapularis muscle strain in professional baseball players.
METHODS: Of 191 players (83 fielders and 108 pitchers) who were members of a single Japanese professional baseball team between January 2013 and December 2022, 8 players (4.2%) had subscapularis muscle strain

Surgical repair is a common treatment for rotator cuff tear; however, the retear rate is high. A high degree of suture repair strength is important to ensure rotator cuff integrity for healing. The purpose of this study was to compare the mechanical performance of rotator cuffs repaired with a mesh suture versus traditional polydioxanone suture II and FiberWire sutures in a canine in vitro model. Seventy-two canine shoulders were harvested. An infraspinatus tendon tear was created in each shoulder. Two suture techniques-simple interrupted sutures and two-row suture bridge-were used to reconnec

Traumatic rotator cuff tears in patients younger than 25 years are rare events, with few reports in the literature. When compared with the more mature shoulder, the young, healthy supraspinatus tendon is a robust tendon that is able to absorb a significant amount of energy before tendon failure. Therefore, the diagnosis of a rotator cuff tear can be often overlooked in this population due to the patient's age. This is a report of traumatic supraspinatus repairs in patients younger than 25 years. Nine patients younger than 25 years were identified with a posttraumatic supraspinatus tear as visu

The author presents his short-term experience with reconstruction operations in ruptures of the rotator cuff which were performed according to principles accepted at present in twenty five patients (26 shoulders) during 1993-1996. Men predominated in the group (16 men) and the majority of patients were in the age bracket from 50-70 years. The follow-up period was 6-36 months. Massive ruptures with a lesion of the cuff exceeding 5 cm (10 times) predominated, followed by ruptures with a lesion under 2 cm (9 times). In the majority of patients the operatoin was made using the Kessel-Gschwend appr

Rotator cuff tears are a common injury that affect a significant percentage of the population over age 60. Although it is widely believed that the rotator cuff's primary function is to stabilize the humerus against the glenoid during shoulder motion, accurately measuring the three-dimensional (3D) motion of the shoulder's glenohumeral joint under in-vivo conditions has been a challenging endeavor. In particular, conventional motion measurement techniques have frequently been limited to static or two-dimensional (2D) analyses, and have suffered from limited or unknown in-vivo accuracy. We have

High frequency ultrasound is a valuable tool for assessing soft tissue injuries about the shoulder. It has advantages over other imaging modalities including relatively low cost, portability, and dynamic real-time visualization. It has a high sensitivity for identifying tendon degeneration, bursitis, and rotator cuff tears. Ultrasound is also an excellent modality for diagnostic and therapeutic needle-guided procedures. A detailed knowledge of shoulder anatomy and transducer positioning is required for obtaining adequate diagnostic value from ultrasound. This Monograph is designed to help deve

BACKGROUND: Despite its main function as abductor, the role of the supraspinatus as stabilizer and rotator cannot be neglected. A supraspinatus tear may not only influence humeral head rotation during abduction but also the strength and loading of the acting (intact) rotator cuff muscles. The purpose of this study was to investigate the effect of constrained humeral rotation and elevation on rotator cuff loading, strain and kinematics with intact and torn cuff conditions.
METHODS: Active humeral elevation until 30° was simulated in twelve fresh-frozen cadaver shoulders with free humeral rotat

Rotator cuff tears (RCT) in older individuals may compound age-associated physiological changes and impact their ability to perform daily functional tasks. Our objective was to quantify thoracohumeral kinematics for functional tasks in 18 older adults (mean age=63.3±2.2), and compare findings from nine with a RCT to nine matched controls. Motion capture was used to record kinematics for 7 tasks (axilla wash, forward reach, functional pull, hair comb, perineal care, upward reach to 90°, upward reach to 105°) spanning the upper limb workspace. Maximum and minimum joint angles and motion excursio

Shoulder injuries are common in contact athletes and vary in severity because of the required complex interplay of shoulder stability and range of motion for proper function. Pathology varies based on sport but most commonly includes shoulder instability, acromioclavicular injuries, traumatic rotator cuff tears, and brachial plexus injuries. Acute management ranges from reduction of shoulder dislocations to physical examination to determine the severity of injury. Appropriate radiographs should be obtained to evaluate for alignment and fracture, with magnetic resonance imaging commonly being n
