BACKGROUND: Serological markers for inflammatory bowel disease (IBD), including anti-neutrophil cytoplasmic antibody (ANCA) and anti-Saccharomyces cerevisiae antibody (ASCA), have a high specificity and positive predictive value in diagnosing IBD. However, neither indication nor use in clinical practice has been clearly established. This study aimed to determine the clinical impact of ANCA/ASCA testing by evaluating how these tests were used in an academic referral center.
METHODS: Retrospective chart review to classify the indications for testing and effect on diagnosis or management.

Monocyte preparations were obtained from the peripheral blood by adherence to plastic dishes. The number of monocytes obtained was significantly greater than the number of B cells recovered from similar samples. Monocytes could be frozen and thawed with relatively little loss in cell numbers or viability. Cytotoxicity tests were performed most reliably at 20 degrees C. Under these conditions, autoantibodies were rare, and normal sera were consistently negative. We report a 9-year-old boy with X-linked hyper-immunoglobulin M syndrome and invasive Cryptococcus laurentii infection identified on the basis of morphologic and biochemical characteristics and serologic tests. Fluconazole treatment resulted in complete recovery. We believe that C. laurentii should be included in the differential diagnosis of opportunistic fungal infections in patients with primary T cell immunodeficiencies.

Cervical lymphadenopathy is the most common presentation of granulomatous inflammation of the neck in children and is usually caused by NTM infection. Although certain granulomatous infections have characteristic imaging features, there is considerable overlap in the imaging appearance of the various disorders. The diagnosis is usually based on a combination of clinical features, histopathologic examination, serologic tests, and culture results.

Advances in blood donor screening have advanced the concept that a zero-risk blood supply may be possible in the future. Although the application of a large number of serologic tests and more stringent donor questioning has certainly aided our ability to screen out donors, risks still remain. Several protozoa including Cryptosporidium sp. and Isospora belli were found in a stool specimen from a patient not suspected of having Acquired Immunodeficiency Syndrome (AIDS). As a result of the parasitologic findings, serologic tests were ordered that verified the diagnosis of AIDS. All stool specimens are routinely tested for presence of acid fast organisms with a modified acid fast technique.

Osteoarticular sporotrichosis was diagnosed in a dog referred for evaluation of hindlimb lameness. There was radiographic evidence of osteopenia of the fourth tarsal and proximal aspects of the metatarsal bones. The diagnosis was based on histologic findings and results of physical examination, radiography, fungal culturing, and serologic tests. The dog was treated successfully with ketoconazole for 3 1/2 months.

A total of 706 native blood sera of birds representing 44 most common bird species in the southern parts of the Kirov region were examined for antibodies against tick-borne encephalitis virus. Some serologic tests were found positive, but the birds appeared to constitute a secondary and nonessential link in the circulation of virus in the natural environment of this area.

A case of erythrocytic polyagglutination in a healthy blood donor is reported. After a review of current literature, the results of clinical and serologic tests which led to the diagnosis of Tn-red cells polyagglutination are presented. This is only the sixth case of Tn-activation in a healthy blood donor. The clinical and forensic significance of this rare phenomenon is discussed.

Amniotic fluid is rarely used in the evaluation of syphilis during pregnancy but was available from a woman with positive serologic tests in the 30th week of gestation. Nonmotile intra-amniotic spirochetes were seen by dark-field microscopy 20 hr after treatment with benzathine penicillin G.
